L'utilisation d'implants mandibulaires en titane poreux permet de compléter une reconstruction par lambeau libre pour améliorer les résultats fonctionnels et esthétiques. Ils peuvent aussi être utilisés seuls pour traiter une asymétrie faciale. Néanmoins leur utilisation peut s'accompagner de complications et de déposes. L'objectif de notre étude était d'identifier les causes d'échecs de ces implants afin de sécuriser leur utilisation. Notre étude a inclus 17 patients ayant bénéficiés de la pose de 19 implants en titane poreux sur une durée de 5 ans au CHU de Caen. Ces implants étaient imprimés sur mesure après planification virtuelle. Les causes de dépose et les facteurs qui leur étaient associés ont été analysés. Au total, il y a eu 10 déposes dont 8 sont imputables à l'implant. Les causes de ces déposes étaient l'exposition (4), l'infection (2) ou l'erreur de planification (2). La localisation symphysaire de l'implant et l'association à une chirurgie majeure osseuse étaient les deux facteurs qui ont montré une association significative avec l'échec de l'implant. Les expositions sont souvent dues à des tissus peu propices à l'implantation pouvant être améliorés notamment par le lipofilling. La planification de l'implant en fonction des tissus mous et l'éviction des portes d'entrées infectieuses sont primordiales. Les échecs de ces implants sont donc partiellement évitables.
