Book excerpt: Contexte : Le cancer du sein est un des cancers les plus fréquents dont l'incidence chez les femmes enceintes est en progression depuis ces 30 dernières années. Jusqu'à peu, très peu de données existaient sur cette pathologie. Méthode : Cette étude rétrospective, non interventionnelle et multicentrique a inclus au sein de 28 centres, entre le 1er janvier 2000 et le 31 décembre 2006, des patientes enceintes au moment du diagnostic. L'objectif principal était de comparer la survie globale de ces patientes à celle d'un groupe de témoins non enceintes appariées sur des facteurs pronostiques. Les objectifs secondaires étaient la comparaison de la survie sans récidive à 5 ans par rapport au groupe témoin ainsi que la description des caractéristiques tumorales et de la prise en charge de ces patientes. Résultats : La survie globale et la survie sans récidive étaient comparables entre les deux groupes. Nous avons mis en évidence en analyse univariée puis multivariée des facteurs pronostiques péjoratifs de SG et SSR à savoir le statut T4, l'envahissement ganglionnaire, et le statut Triple Négatif. Les caractéristiques tumorales étaient comparables à celles des femmes jeunes. Les patientes ont été prises en charge selon les recommandations actuelles en dehors d'une faible proportion de chimiothérapie par taxanes. Conclusion : Les progrès faits dans la prise en charge des patientes atteintes d'un cancer du sein au cours de la grossesse sont importants. La prise en charge de ces patientes doit avoir lieu dans des Centres Expert Régionaux en collaboration étroite avec les obstétriciens.
