Book excerpt: L’évaluation de la fonction myocardique a des conséquences importantes sur la prise en charge et le pronostic des patients suivis pour une cardiopathie ischémique. L’objectif était de comparer les paramètres de déformation myocardique obtenus par le suivi de marqueurs acoustiques en 2 Dimensions (2D) et en 3 Dimensions (3D), au rehaussement tardif en IRM. Dix huit témoins (51±10 ans) ainsi que 25 patients (62±16 ans) suivis pour une cardiopathie ischémique avec dysfonction ventriculaire gauche (fraction d’éjection ventriculaire gauche [FEVG] : 41±9%) ont été inclus dans l’étude. Pour chacun une IRM cardiaque avec recherche de viabilité a été réalisée. Les paramètres de déformation longitudinale, radiale et circonférentielle ont été obtenus par suivi de marqueurs acoustiques en 2 D et en 3D. Les segments avec une nécrose transmurale ont été définis comme ceux ayant un rehaussement tardif supérieur à 50%. Au final, 661/688 (96%) des segments ont été analysés en IRM et en échographie 3D. Les valeurs des paramètres de déformation obtenus en 3D des segments avec une nécrose transmurale (n=154) étaient inférieures à celles des sujets témoins (n=288) et à celles des segments avec une nécrose non transmurale (n=219).
