Patients et méthodes : 94 patients ont été inclus. La FEVD était mesurée par contourage endocardique manuel pour le ventricule droit par deux radiologues en aveugle. Les caractéristiques cliniques, fonctionnelles, anatomiques et étiologiques étaient recueillies. La VIO relative était définie comme la différence entre les mesures des deux observateurs divisée par leur moyenne et exprimée en pourcentage. Résultats : La VIO relative moyenne pour la FEVD était 12±14% (n=94). Les facteurs responsables d'une augmentation significative de la VIO relative était la présence d'une régurgitation droite (20%; n=27), d'une cardiomyopathie dilatée (20%; n=24), une FEVD45% (20%; n=32), un DTSVG41mm (18%; n=30), ou d'une régurgitation gauche (18%; n=27). Conclusion : pour le suivi des cardiopathies acquises ou congénitales, un changement de la FEVD doit être interprété avec précaution du fait d'une augmentation de la VIO relative quand il existe une valvulopathie droite ou gauche, une dilatation du VG ou une dysfonction systolique.

Book excerpt: Objectifs : 1.Évaluer la variabilité de la mesure des paramètres fonctionnels du ventricule droit (VD) en IRM cardiaquechez les patients atteints d'une cardiopathie acquise, avec un accent particulier sur l'effet de l'expérience de l'observateur et des étapes du post-traitement. 2.Évaluer la précision diagnostique et la variabilité de la mesure de 3 paramètres semi-quantitatifs comme outil d'évaluation de la fonction systolique du VD : TAPSE = Excursion systolique du plan de l'anneau tricuspide ; RVFS = Fraction de raccourcissement du VD; RVFAC = Fraction de changement de surface du VD. Matériels et méthodes : Soixante patients consécutifs atteints de cardiopathie acquise ayant une indication clinique à la réalisation d'une IRM cardiaque ont été inclus de façon prospective. Les volumes, fraction d'éjection et masses VD et VG ont été calculés avec des séquences CINE de type balanced steady state free precession. Les paramètres semi quantitatifs ont été calculés sur une coupe ciné 4-cavités. Toutes les analyses ont été effectuées 2 fois par 3 observateurs ayant des niveaux d'expérience différents en IRM cardiaque. La variabilité intra et inter observateur a été évaluée par la méthode de Bland-Altman, le coefficient de variation et le coefficient de corrélation intraclasse. L'effet des étapes de post-traitement suivantes sur la variabilité des paramètres quantitatifs a été évalué: expérience des observateurs, choix des coupes basale et apicale, choix de la phase télésystolique, et contourage de l'observateur. Une corrélation entre les paramètres semi-quantitatifs et la FEVD et une analyse Receiver Operating Characteristic (ROC) ont été réalisés pour chaque paramètre semi-quantitatif. Résultats : Le temps de segmentation moyen variait de 9.8 à 19.0 min pour VD et de 6.4 à 9.2 min pour le VG. La variabilité de la mesure des paramètres fonctionnels du VD en IRM était fortement influencée par l'expérience préalable de l'observateur, le choix de la coupe basale et le contourage de l'observateur. L'évaluation de la fonction VD en IRM restait 2 fois plus longue et 2 à 3 fois plus variable que pour le VG. La corrélation entre le RVFAC et la FEVD était bonne pour tous les observateurs et peu dépendante de l'expérience préalable de l'observateur. Au contraire, le RVFS et le TAPSE étaient moins corrélés avec la FEVD et dépendants de l'expérience préalable. L'analyse ROC a démontré qu'un RVFAC

Book excerpt: Pré requis : La dysfonction ventriculaire droite est un marqueur de mauvais pronostic à long terme dans les tétratologies de Fallot et les transpositions des gros vaisseaux opérées à l'étage atrial. L'évaluation de la fonction du ventricule droit (VD) est difficile du fait de sa forme géométrique complexe. L'IRM est la méthode de référence actuelle. Objectifs : L'objectif principal de ce travail était d'étudier les relations entre le pic systolique de déformation du VD en 2D-strain, les indices échocardiographiques classiques de fonction VD et la fraction d'éjection du VD mesurée par l'IRM chez les patients adultes opérés de tétralogie de Fallot ou de transposition des gros vaisseaux (TGV) corrigée à l'étage atrial. Les objectifs secondaires étaient d'évaluer la faisabilité et la reproductibilité inter et intra observateur du 2D-strain chez ces patients, ainsi que la reproductibilité intra observateur de l'IRM. Méthodes : Dix sept patients ont été inclus dans l'étude sur une période de 18 mois et ont été répartis en 2 groupes : un groupe opéré de tétralogie de Fallot et le second TGV opéré à l'étage atrial. L'analyse du 2D-strain par la technique du suivi de points a été réalisée à partir d'une coupe apicale 4 cavités par 2 opérateurs indépendants. Pour chaque patient, en plus de la recherche de critères de fonction VD classiques, les pics systoliques de strain longitudinal global, pour la paroi latérale, pour la paroi septale et pour chaque segment apical, médial et basal de chaque paroi ont été déterminés. Chaque patient a également eu une IRM cardiaque avec mesure de la fraction d'éjection du VD (FEVD). Résultats : Les patients étaient âgés de 11 à 61 ans (27 + ou -13 ans), et avaient été opérés depuis 25 + ou - 9 ans, 11 d'une tétralogie de Fallot et 6 avaient eu une intervention de Mustard ou de Senning pour une TGV. Tous les paramètres d'évaluation de la fonction VD étaient diminués : fraction de raccourcissement de surface du VD à 37 + ou - 7 %, Tapse à 15 +ou - 5 mm, pic de strain systolique global à -17,25 + ou - 3,43 % avec 15,46 + ou - 4.31 pour la paroi septale et -19,04 % pour la paroi latérale en échographie, FEVD à 40 + ou - 11 % en IRM. Seule la régression linéaire entre le pic de strain systolique global du VD et la FEVD IRM était significative dans les tétralogies de Fallot opérées avec un coefficient à 0.65 (p=0.003). Les variabilités intra et inter observateur de la mesure du pic de 2D-strain étaient faibles, de même que la variabilité intra observateur en IRM. La faisabilité du 2D-strain sur le VD est également bonne avec seulement 4 segments non analysables sur 204. Conclusion : Le 2D-strain est une méthode récente, intéressante et facile à réaliser pour évaluer la fonction ventriculaire droite dans la cardiopathie congénitale.

L’influence de l’utilisation d’un espace inter-coupes (EIC) constant de Omm (méthode de référence), 5mn, 10mn ou variable de la base du VD à l’apex, ainsi que les variations inter-observateur ont été étudiées concernant le calcul des volumes et des fractions d’éjection des deux ventricules. Il apparaît que l’évaluation fonctionnelle des deux ventricules en IRM peut être considérablement raccourcie par l’utilisation d’un EIC constant de 5mn ou variable. Par contre, un EIC de 10mn ne paraît pas approprié pour l’étude fonctionnelle du VD.
