I develop alternative quasi-experimental approaches to quality estimation that accommodate nonlinear causal effects, institutional specialization, and unobserved selection-on-gains. I use this framework to compute empirical Bayes posteriors of the quality of 4,821 U.S. hospitals, combining estimates from ambulance referral quasi-experiments with predictions from observational risk-adjustment models. Higher-spending, higher-volume, and privately-owned hospitals are of higher quality, and most healthcare markets exhibit positive Roy selection-on-gains. I then simulate Medicare reimbursement and consumer guidance programs based on different hospital quality measures. Higher-spending providers tend to see moderately larger performance-linked subsidies when quality posteriors replace conventional rankings, while teaching hospitals are reimbursed relatively less. Admissions policy simulations highlight limitations of consumer guidance programs in settings with unobserved Roy selection: redirecting patients to top-ranked hospitals may worsen expected survival when based on observational rankings, while quasi-experimental rankings appear to generate modest gains.

Book excerpt: We study the costs of hospitalizations on patients’ earnings and labor supply, using the universe of hospital admissions in Denmark and full-population tax data. We evaluate the quality of treatment based on its ability to mitigate the labor market consequences of a given diagnosis and propose a new measure of hospital quality, the "Adjusted Earning Losses" (AEL). We find a 4 percentage points difference in lost earnings between the best and worst large Danish hospitals, all else equal. We show that AEL contains significant additional information relative to traditional measures and does not suffer from worse selection issues. We also document a large decline in the labor cost of hospitalizations over time, with large variations across diseases. We find that the average post-hospitalization reduction in labor earnings declined by 25 percent (50 percent) on the intensive (extensive) margin between 1998 and 2012.

The method aggregates any number of mortality outcome measures observed over several years into a single indicator. We begin with the supposition that there exists an abstract quality index which drives all observed mortality outcomes in each hospital. This abstract quality index is not directly observable but manifested via the observed mortality outcomes, which we make use of to provide an estimate of the abstract quality index. The method is applied to a sample of heart disease episodes extracted from hospital administrative data from the state of Victoria, Australia. Using the quality estimates, we show that teaching hospitals and large regional hospitals provide higher quality of care than other hospitals and this superior performance is related to hospital size.

Book excerpt: In response to unsustainable growth in health care spending, there is enormous interest in reforming the payment system to "pay for quality instead of quantity." While quality measures are crucial to such reforms, they face major criticisms largely over the potential failure of risk adjustment to overcome endogeneity concerns. In this paper we implement a methodology for estimating the causal relationship between hospital quality measures and patient outcomes. To compare similar patients across hospitals in the same market, we xploit ambulance company preferences as an instrument for patient assignment. We find that a variety of measures used by insurers to measure provider quality are successful: assignment to a higher-scoring hospital results in better patient outcomes. We estimate that a two-standard deviation improvement in a composite quality measure based on existing data collected by CMS is causally associated with reductions in readmissions and mortality of roughly 15%.

To this end a carefully selected group of experts in medicine, epidemiology, and health econom ics was invited to present papers. They were selected for their reputations either as conceptualizers or as empirical evaluators, or - the rarest breed of expert - as both. The context was to be empirical. Three procedures were selected that had been subject to evaluation but that posed rather different types of problem. The first was the treatment of renal failure by dialysis of various kinds. This has a relatively long history of evaluation, with a large literature, and particularly raises broad policy is sues within the health services of Western societies as to - the size of programmes to be provided; the type, location, and mix of treatments; the selection of patients to receive treatment; and the measurement of the success of various strategies. The second was the treatment of duodenal ulcer by a new species of drug - the hista mine Hrreceptor antagonists (specifically, cimetidine).
