Book excerpt: With the United States and other developed nations spending as much as 14 percent of their GDP on medical care, economists and policy analysts are asking what these countries are getting in return. Yet it remains frustrating and difficult to measure the productivity of the medical care service industries. This volume takes aim at that problem, while taking stock of where we are in our attempts to solve it. Much of this analysis focuses on the capacity to measure the value of technological change and other health care innovations. A key finding suggests that growth in health care spending has coincided with an increase in products and services that together reduce mortality rates and promote additional health gains. Concerns over the apparent increase in unit prices of medical care may thus understate positive impacts on consumer welfare. When appropriately adjusted for such quality improvements, health care prices may actually have fallen. Provocative and compelling, this volume not only clarifies one of the more nebulous issues in health care analysis, but in so doing addresses an area of pressing public policy concern.

Firms in manufacturing industries are influenced by the market-oriented liberalization reform policies in many developing countries since the late eighties. However, studies applying appropriate methodology to appropriate data seldom analyze the impact of reforms on the performance of production units such as manufacturing firms. The central point of this book is to address this issue by comparing firms’ achievement with 'best practice' performance before and after reforms. This form of analysis is not new but it emphasizes a new focus or realignment of thinking within neoclassical economics to develop an analytical framework. This book examines the productivity growth of Bangladesh manufacturing firms as component measures of changes in capacity realization and technical progress. The significant feature of this approach is that it allows for the inefficiency of firms, and thus productivity growth is estimated rather than taking it as a residual as is usually measured in the traditional growth accounting approach. High rates of technological progress, on the one hand, can co-exist with low rates of capacity realization. On the other hand, relatively low rates of technological progress can co-exist with an improving capacity realization. As a result specific policy actions are required to address the difference in the sources of variation in productivity. In this respect this book would provide invaluable insights for policy makers, development practitioners, academics and students of economics.

A significant improvement in the Malaysian financial system, coupled with rapid economic growth and a rich history of financial sector reforms, makes Malaysia an interesting case study for this subject. The author shows that some government interventions seem to have impacted negatively on economic growth, whereas repressionist financial policies such as interest rate controls, high reserve requirements and directed credit programmes seem to have contributed positively to financial development. The analysis concludes that financial development leads to higher output growth via promoting private saving and private investment. Book excerpt: The book develops and applies a modelling framework that enables consumers' expenditure and portfolio behaviour to be analysed in an integrated manner. It extends and applies the Yale approach to financial model building associated with James Tobin. The basis of this approach is surveyed with particular emphasis on the empirical implementation of portfolio models in the UK, US and other countries, appropriate econometric estimation techniques and comparisons with other monetary modelling strategies. Empirical application of the model is particularly addressed to testing whether the composition of wealth holdings is important for expenditure and asset demands, whether concentration on 'money' at the expense of other assets and liabilities is acceptable, and whether the interrelationships stressed by Tobin's general equilibrium approach are statistically important. The results have important implications for the recent literature on wealth effects on consumption and the overemphasis on 'money' at the expense of other financial assets and liabilities.
