Book excerpt: The overall objective of this research was to identify genes that are aberrantly methylated in epithelial ovarian cancer. Our approach was to treat or mock treat primary normal or tumor cultured cells with drugs that inhibit DNA methyltransferase activity and then perform microarray analysis to identify genes that are likely to exhibit methylation-mediated silencing. We also employed similar analysis of 43 ovarian cell lines. Two major criteria identified genes likely to be methylated: maximum fold-change in expression following drug treatment, and a high standard deviation in expression among untreated cells. These criteria were validated using a gene set known to exhibit methylation in other cancers and led to prediction of a total of 360 methylated genes in ovarian cancer, among which 32 were validated. Genes involved in TGF-beta pathway activity were overrepresented among the candidates, and their methylation status was confirmed. Pathway activity was found to increase when DNA methyltransferase activity is inhibited, indicating coordinate epigenetic suppression of this pathway in ovarian cancer.

Researchers now know that ovarian cancer is not a single disease-several distinct subtypes exist with different origins, risk factors, genetic mutations, biological behaviors, and prognoses. However, persistent questions have impeded progress toward improving the prevention, early detection, treatment, and management of ovarian cancers. Failure to significantly improve morbidity and mortality during the past several decades is likely due to several factors, including the lack of research being performed by specific disease subtype, lack of definitive knowledge of the cell of origin and disease progression, and incomplete understanding of genetic and non-genetic risk factors. Ovarian Cancers examines the state of the science in ovarian cancer research, identifies key gaps in the evidence base and the challenges to addressing those gaps, considers opportunities for advancing ovarian cancer research, and examines avenues for translation and dissemination of new findings and communication of new information to patients and others. This study makes recommendations for public- and private-sector efforts that could facilitate progress in reducing the incidence of morbidity and mortality from ovarian cancers.

Book excerpt: Epithelial ovarian cancer (EOC), and all other solid tumor types investigated to date, contain a variable proportion of tumors characterized by global DNA hypomethylation (GDHO). This phenotype is known to involve loss of DNA methylation at repetitive elements and at single copy gene promoters of cancer germline (CG)/cancer testis (CT) antigens. GDHO in EOC has also been linked to tumor progression and reduced survival. Despite the apparent clinical importance of DNA hypomethylation, we currently have a limited understanding of the origin, the targets, and the consequences of GDHO in EOC. To reveal the underlying mechanisms and molecular pathology of GDHO in cancer, we used gene expression profiling and methylome sequencing to determine the molecular differences between hypomethylated EOC samples and normally methylated EOC samples. The gene expression pattern was distinct in hypomethylated EOC, with more than half of differentially expressed genes (DEGs) upregulated. A large number of epigenetic related genes and histones were differentially expressed in hypomethylated EOC. In addition, many CG antigens were upregulated in hypomethylated EOC, including those not previously known to be regulated by DNA methylation in EOC, for example PRAME and CT45. Notably, one of the most significantly altered pathways in hypomethylated tumors was cellular growth and proliferation, including the FOXM1 pathway, implicating enhanced proliferation in phenotype. Methylome analysis revealed that DNA hypomethylation is not evenly dispersed across the genome, but is regionally localized, including prominent alterations at nuclear lamina associated domains (LADs). Taken together, our data show that DNA hypomethylation in EOC is linked to global gene activation, proliferation, and nuclear disorganization. These mechanisms may account for the poor prognosis and reduced survival of patients showing DNA hypomethylation. Our study is the first detailed investigation of the genomic and the epigenomic characteristics of GDHO in human epithelial EOC, and sheds light on the connection of this important epigenetic phenotype with carcinogenesis.

The high mortality rate of this cancer is due to the poor prognosis and that the majority of patients are diagnosed at late stage. Therefore, a combination of cytoreduction and adjuvant chemotherapy is the only choice for this disease at late stage. Platinum-based chemotherapy regimens are the first line treatment for ovarian cancer. However, the repetitive challenges of platinum-based agents and the frequent relapse cause acquired chemoresistance which is the major obstacle for clinical management of this disease. The underlying molecular mechanism for acquired chemoresistance remains largely unclear. Therefore, understanding the molecular mechanism in acquired chemoresistance of ovarian cancer is urgently needed. Emerging evidence has suggested that dysregulation of miRNAs is significantly involved in acquired chemoresistance in human cancers. In this study, the expression status and functional roles of miR-199b-5p were characterized in the chemoresistance of ovarian cancer. By miRCURY LNA(TM) miRNA array, miR-199b-5p was one of the downregulated miRNAs identified in acquired chemoresistant ovarian cancer cells. Further Q-PCR analysis found that miR-199b-5p exhibited a pattern of progressive reduction from early to advanced stage and from low to high grade ovarian cancer tissue samples (N=79). Interestingly, the expression of miR-199b-5p could be restored upon 5-Aza-2''-deoxycytidine (5-Aza-dC) treatment in ovarian cancer cells. Methylation-specific PCR and bisulfite genomic sequencing revealed that the promoter region of miR-199b-5p (Chromosome9q34) was frequently hypermethylated in chemoresistant ovarian cancer cells. Functionally, enforced expression of miR-199b-5p remarkably diminished the cisplatin-resistance in miR-199b-5p low expressing ovarian cancer cells, whereas depletion of miR-199b-5p by siRNA approach augmented cisplatin-resistance in miR-199b-5p high expressing ovarian cancer cells. A tumor xenograft mouse model further confirmed that miR-199b-5p could sensitize ovarian cancer cells to cisplatin-mediated tumor growth inhibition in vivo. On the other aspect, computational analysis plus luciferase reporter assay and western blotting identified JAG1, a key ligand of Notch1 signaling frequently associated with chemoresistance of human cancers, as a direct target of miR-199b-5p in ovarian cancer cells. Intriguingly, an inverse relationship between the downregulation of miR-199b-5p and the upregulation of JAG1 was found by in situ hybridization (ISH) and immunohistochemical (IHC) analyses in a commercial ovarian cancer tissue array (OVC1021). Functionally, enforced expression of JAG1 increased cisplatin resistance of ovarian cancer cells. In contrast, depletion of JAG1 by siRNA approach abrogated the cisplatin-resistance of ovarian cancer cells. This finding was in consistent with the effect of enforced expression of miR199b-5p in cisplatin-resistant ovarian cancer cells. Mechanistically, the Notch1 luciferase reporter assay and western blotting analysis demonstrated that the Notch1 signaling activity could be activated by JAG1 or conversely be inhibited by enforced expression of miR-199b-5p. We have set up the infrastructure at Mayo for an Ovarian Cancer Research Program utilizing the rich resources of clinical material. This Program Project focuses specifically upon a genetic analysis of ovarian cancer using the transcriptional profiling strategies of cDNA microarrays and subtraction suppression hybridization. We have linked the work of cancer geneticists with cell biologists to begin to understand the functional role that some of the consistently aberrantly-regulated genes play in ovarian cancer development. There are three projects in this grant. The first project focuses on the identification of consistently down-regulated genes in ovarian cancer. The second project focuses on the role of gene amplification in familial versus sporadic ovarian cancer. The third project is to characterize genes that are consistently down-regulated in ovarian tumors that are derived from within common fragile site regions. This Program Project is centered within a larger institutional effort to better understand the biology of the development of ovarian cancer and to devise better strategies for the prevention, early detection, and treatment of this lethal disease.
