Book excerpt: We examine how policymakers should react to a pandemic when there is significant uncertainty regarding key parameters relating to the disease. In particular, this paper explores how optimal mitigation policies change when incorporating uncertainty regarding the Case Fatality Rate (CFR) and the Basic Reproduction Rate (R0) into a macroeconomic SIR model in a robust control framework. This paper finds that optimal policy under parameter uncertainty generates an asymmetric optimal mitigation response across different scenarios: when the disease's severity is initially underestimated the planner increases mitigation to nearly approximate the optimal response based on the true model, and when the disease's severity is initially overestimated the planner maintains lower mitigation as if there is no uncertainty in order to limit excess economic costs.

In doing so, it furthers the policy studies scholarship through an examination of the effects of policy styles on national responses to the pandemic. Despite governments being faced with the same threat, significant variation in national responses, frequently of contradictory nature, has been observed. Implications about responses inform a broader class of crises beyond this specific context. The authors argue that trust in government interacts with policy styles resulting in different responses and that the acute turbulence, uncertainty, and urgency of crises complicate the ability of policymakers to make sense of the problem. Finally, the book posits that unless there is high trust between society and the state, a decentralized response will likely be disastrous and concludes that while national responses to crises aim to save lives, they also serve to project political power and protect the status quo. This text will be of key interest to scholars and students of public policy, public administration, political science, sociology, public health, and crisis management/disaster management studies.

And because they are shot through with uncertainty, they often become sites of contestation and conflict. This book presents research on the 2009 pandemic and other public health crises in an attempt to describe and analyze the distinctive challenges that such diseases pose today. Thanks to vaccines, more reliable provision of medical services, more effective means of communication, and a more educated public, some argue we will not see a new Black Plague – or even Spanish Flu – in our time. Today we face new challenges, however, which can both enable diseases to reach pandemic scales and affect our ability to enact an appropriate response. Those include fragmentation of media, tribalization of “knowledge regimes,” the increasingly troubled status of scientific and political expertise, growing cross-continental mobility, as well as the globalization and commercialization of pandemic response systems. These distinctive complexities make the need to stage public action in response to pandemics and other public health crises a crucial problem, on which thousands of human lives hinge. This volume consists of a handful of social science and humanities studies of precisely such complexities, and thus offers a much-needed supplement to existing research on pandemics and pandemic response.

The consequences and lethality of Ebola have increased interest in coordinated global response to infectious threats, many of which could disrupt global health and commerce far more than the recent outbreak. In order to explore the potential for improving international management and response to outbreaks the National Academy of Medicine agreed to manage an international, independent, evidence-based, authoritative, multistakeholder expert commission. As part of this effort, the Institute of Medicine convened four workshops in summer of 2015 to inform the commission report. The presentations and discussions from the Workshop on Research and Development of Medical Products are summarized in this report.

In addition to the number of patients infected with the SARS virus, the disease had profound economic and political repercussions in many of the affected regions. Recent reports of isolated new SARS cases and a fear that the disease could reemerge and spread have put public health officials on high alert for any indications of possible new outbreaks. This report examines the response to SARS by public health systems in individual countries, the biology of the SARS coronavirus and related coronaviruses in animals, the economic and political fallout of the SARS epidemic, quarantine law and other public health measures that apply to combating infectious diseases, and the role of international organizations and scientific cooperation in halting the spread of SARS. The report provides an illuminating survey of findings from the epidemic, along with an assessment of what might be needed in order to contain any future outbreaks of SARS or other emerging infections.

When the pandemic is over, investigators, political and scientific, will mull over what went wrong and what went right. Different responses will be compared to determine effectiveness, not just on lives saved, but on incomes lost. The economic fallout will become clearer. The unforeseen derivative impact of responses on human health will also emerge - at some future time.But maybe there are lessons to be learned right now: lessons relating to the evolution of thought and behavior during times of danger and uncertainty, lessons relating to evolution of responses, lessons relating to how policy is made, and identification of deficits arising when failing to consider significant, but perhaps not obvious, outcomes, such as effects on the elderly and on their mental health.The first part of the book contains early essays reflecting public confusion and terror; the second part reflects initial responses generated by the early panic. As the earliest essays indicate, initial focus centered on conspiracy theories rather than masking and social distancing, diverting and deflecting resources from more promising and productive responses.The third section analyzes recently emerging data. These data reflect another key - and erroneous theme of the early panic - based on early and stagnant research: the virus preyed especially on the elders. Ill-thought-out policy, manifesting in isolation directives, had additional and disastrous consequences on mental health, which, in turn resulted in significantly excess dementia-related deaths. These results cry out for consideration of epidemic responses from a legal "therapeutic jurisprudential" perspective, where concern for mental health ramifications is considered before policy implementation.The relatively new field of therapeutic jurisprudence traditionally involves considering mental health effects of legislation. As mental health consequences of CoVid 19 responses continue to proliferate, even, as I show in the final chapter, perhaps disproportionately affecting minority populations, involvement of the field becomes even more important.

Understanding and responding to these often unpredictable events have become major challenges for local, national and international bodies. All too often, responses can become restricted by implicit assumptions about who or what is to blame that may not capture the dynamics and uncertainties at play in the multi-scale interactions of people, animals and microbes. As a result, policies intended to forestall epidemics may fail, and may even further threaten health, livelihoods and human rights. The book takes a unique approach by focusing on how different policy-makers, scientists, and local populations construct alternative narratives-accounts of the causes and appropriate responses to outbreaks- about epidemics at the global, national and local level. The contrast between emergency-oriented, top-down responses to what are perceived as potentially global outbreaks and longer-term approaches to diseases, such as AIDS, which may now be considered endemic, is highlighted. Case studies-on avian influenza, SARS, obesity, H1N1 influenza, HIV/AIDS, tuberculosis, and haemorrhagic fevers-cover a broad historical, geographical and biological range. As this book explores, it is often the most vulnerable members of a population-the poor, the social excluded and the already ill-who are likely to suffer most from epidemic diseases. At the same time, they may be less likely to benefit from responses that may be designed from a global perspective that neglects social, ecological and political conditions on the ground. This book aims to bring the focus back to these marginal populations to reveal the often unintended consequences of current policy responses to epidemics. Important implications emerge - for how epidemics are thought about and represented; for how surveillance and response is designed; and for whose knowledge and perspectives should be included. Book excerpt: Abstract: We quantify the macroeconomic effects of COVID-19 for a small open economy by calibrating a SIR-multi-sector-macro model to Turkey. Sectoral supply shocks are based on the proximity requirements in each sector and the ability to work from home. Physical proximity determines the supply shock through its effect on infection rates. Sectoral demand shocks incorporate domestic and foreign demand, both of which adjust with infection rates. We calibrate demand shocks during COVID-19 using real-time credit card purchase data. Our results show that the optimal policy, which yields the lowest economic cost and saves the maximum number of lives, can be achieved under a full lockdown of 39 days. Economic costs are much larger for an open economy as the shocks are amplified through the international production network. A decline in foreign demand leads to losses in domestic sectors through international input-output linkages, accounting for a third of the total output loss. In addition, the reduction in capital flows deprives the network from its trade financing needs, where sectors with larger external finance needs experience larger losses. The policy options are limited given sparse fiscal resources to fight the pandemic domestically, while serving the external debt. We present historical evidence from 2001 crisis of Turkey, when fiscal, monetary and exchange rate policies were employed altogether to deal with a triple crisis of balance of payments, banking, and sovereign debt
