Over the past decade, its government has introduced such ground-breaking schemes as Integrated Environmental Zoning, the City-Environment Project, the Bubble Concept and Policy Concepts and new approaches to coping with noise, odours, soil pollution, air pollution and safety issues. These initiatives and policy tools reflect a rapidly changing and decentralising environmental policy, which contrasts with more conventional environmental ideologies. However, at present little is known of these policies in the international arena. De Roo shows how and why, in recent years, the country's planning system has moved away from its traditional 'top-down' structure. The resulting changes have had far-reaching consequences for the traditional principles of Dutch environmental policy. In addition, measures for compensating excessive environmental loads are now open to discussion and environmental quality is a subject of negotiation among stakeholders. All these developments mean that environmental policy-making has become more closely integrated with local initiatives that focus on general location-specific qualities. In this book, this development is referred to as 'tailor-made comprehensive planning', which relates closely to the local context, is area-specific, situation-dependent, and embraces shared governance.

Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. These goals are an integral part of the European Charter on Environment and Health, adopted by 29 European Member States and the Commission of the European Communities in December 1989. The Charter stresses the shared responsibility of everyone to protect the environment, to be given adequate and accurate information, and to be involved in decision-making. It outUnes the principles for public policy as well as what needs to be done to transform them into action. In this, strong information systems have a vital role to play by helping to monitor the effectiveness of measures taken, of trends analysed, of priorities set and of decisions made.

Book excerpt: Accurate assessment of environmental hazards and related risks is a primary prerequisite for effective environmental health protection, at both the individual and collective level. National and regional policies on environmental health need to be guided by knowledge about the risks to the populations involved; as the Environmental Action Plan for Europe notes, 'priority setting requires the comparative assessment of risks to health of different environmental factors against the cost of controlling them.' In recent years this has assumed particular importance, for with the encouragement of the World Health Organisation (WHO), all countries in Europe are committed to producing National Environmental Health Action Plans (NEHAPs), which will define priorities and targets for environmental health and the actions needed to achieve them. Reliable information on risks is clearly fundamantal to this process. Individual risk assessment is no less important in this context. Much of the responsibility and capacity to improve public health lies ultimately in the choices (e.g. about diet, smoking, alcohol consumption, sexual activities, sporting activities, travel mode, place of residence and occupation) which we make as individuals. If we are to improve and protect our own health, therefore, and in so doing play our personal role in achieving the targets set by these Plans, we need to be guided by a clear understanding of the risks involved.
