Book excerpt: Contexte. En 2014, le département de médecine générale de Saint-Étienne a mis en place un nouvel outil pédagogique : le jeu de rôle filmé avec acteur patient. Cet enseignement a été plébiscité par les internes. En 2015, l'expérience a été tentée avec les externes. Objectif. Faire évaluer cet enseignement par les externes. Méthode. Une étude quantitative prospective par questionnaire a été menée au cours de l'année universitaire 2014-2015 auprès d'externes en stage de médecine générale. L'enseignement s'est déroulé en petits groupes. Deux thèmes étaient abordés : première consultation et annonce d'une mauvaise nouvelle. Après chaque entrevue, une rétroaction avait lieu en groupe avec analyse vidéo, sous la supervision de l'enseignant. Le critère de jugement principal correspondait au taux de satisfaction des internes sur le dispositif d'enseignement. Les réponses entre externes filmés et observateurs ont été comparées. Résultats. 119 externes, 19% (n = 23) filmés, 81% (n = 96) observateurs, ont rendu 1 428 réponses. Le taux de satisfaction global était de 95.9% (n = 1 369), sans différence significative entre les externes filmés et les observateurs. 99.2% (n = 118) des externes étaient satisfaits du recours à l'acteur, 98.3% (n = 117) de la discussion en petit groupe et 96.6% (n = 115) de la vidéo. La principale difficulté observée était l'appréhension de la rétroaction. Conclusion. Cet enseignement a rencontré un franc succès chez les externes. Le cadre d'une évaluation formative permet un apprentissage de la compétence communicationnelle en toute sécurité.

L'objectif de cette étude était de vérifier si cet enseignement améliore la compétence communication des étudiants en médecine qui y participent. Matériel et Méthode : C'était une étude observationnelle d'une cohorte d'étudiants de Diplôme de Formation Générale en Sciences Médicales 3 tirés au sort parmi les participants à l'enseignement en 2017. Leur compétence communication a été comparée au début et à la fin de l'enseignement, par l'échelle de Calgary-Cambridge simplifiée. Résultats : 34 étudiants ont été tirés au sort, 27 ont été évalués. Dans une première partie, ils présentent un état des lieux des connaissances concernant la relation médecin-patient, présentent les méthodes d'évaluation par la simulation des compétences relationnelles au niveau national et international ainsi que leurs limites et l'état de la recherche au niveau local ayant conduit à la création de l'échelle SURF. Dans une seconde partie, ils présentent un article de recherche original, intitulé « Assessment of medical student's communication skills using simulation : Reliability and validity of the SURF scale » dont l'objectif principal est d'étudier la fidélité de la grille SURF pour l'évaluation des compétences relationnelles des étudiants en médecine lors d'un exercice de simulation en calculant le coefficient de corrélation intra-classe entre deux évaluateurs. Les résultats de cette étude montrent une très bonne fidélité de cette échelle. La troisième partie est consacrée à la relecture des résultats obtenues lors de cette étude et leurs limites. Ces données sont confrontées à celles de la littérature et intègre les différentes difficultés rencontrées lors de la réalisation de ce projet ainsi que les bénéfices attendus pour les étudiants en médecine se faisant évaluer par l'échelle SURF. Puis ils concluent sur les perspectives possibles à l'avenir concernant de nouveaux projets d'études sur l'enseignement des compétences en communication par la simulation.

In this classic of French cultural studies, Georges Didi-Huberman traces the intimate and reciprocal relationship between the disciplines of psychiatry and photography in the late nineteenth century. Focusing on the immense photographic output of the Salpetriere hospital, the notorious Parisian asylum for insane and incurable women, Didi-Huberman shows the crucial role played by photography in the invention of the category of hysteria. Under the direction of the medical teacher and clinician Jean-Martin Charcot, the inmates of Salpetriere identified as hysterics were methodically photographed, providing skeptical colleagues with visual proof of hysteria's specific form. These images, many of which appear in this book, provided the materials for the multivolume album Iconographie photographique de la Salpetriere. As Didi-Huberman shows, these photographs were far from simply objective documentation. The subjects were required to portray their hysterical "type"—they performed their own hysteria. Bribed by the special status they enjoyed in the purgatory of experimentation and threatened with transfer back to the inferno of the incurables, the women patiently posed for the photographs and submitted to presentations of hysterical attacks before the crowds that gathered for Charcot's "Tuesday Lectures." Charcot did not stop at voyeuristic observation. Through techniques such as hypnosis, electroshock therapy, and genital manipulation, he instigated the hysterical symptoms in his patients, eventually giving rise to hatred and resistance on their part. Didi-Huberman follows this path from complicity to antipathy in one of Charcot's favorite "cases," that of Augustine, whose image crops up again and again in the Iconographie. Augustine's virtuosic performance of hysteria ultimately became one of self-sacrifice, seen in pictures of ecstasy, crucifixion, and silent cries.
