Dans la première partie, nous procéderons à une approche du stress dans sa globalité: son origine, les mécanismes mis en place quand nous sommes soumis au stress, les manifestations qui en découlent et la façon dont on peut le gérer, le traiter et l'éviter. Dans un second temps, nous nous intéresserons à la place qu'il tient dans notre métier que ce soit pendant les études ou tout au long de notre carrière. La dernière partie sera consacrée à l'enquête réalisée. Après l'avoir présentée, nous la détaillerons et ferons l'analyse des résultats obtenus et des conclusions que l'on peut en tirer.

Book excerpt: En odontologie, l'empathie s'inscrit dans le modèle de prise en charge biopsychosociale centré sur la personne et ses besoins, elle est un élément clé dans la relation thérapeutique. Le stress est présent chez les étudiants d'odontologie au cours de leur formation et il peut affecter leurs capacités à développer une attitude empathique. Cette étude a pour but d'évaluer les niveaux de stress et d'empathie des étudiants avant et après leurs premières prises en charge de patients de façon autonome. Elle aussi pour objectif de vérifier s'il existe une corrélation entre ces deux facteurs. Les réponses des étudiants de 4ème année de chirurgie dentaire de Clermont-Ferrand aux questionnaires JSPE, TEQ, et PSS ont été étudiées. Les résultats n'ont pas mis en évidence d'évolution de l'empathie et du stress, ni de corrélation entre ces deux facteurs. Les filles ont un niveau d'empathie plus élevé que celui des garçons. Les étudiants qui choisissent le métier de chirurgien-dentiste pour des raisons financières sont moins empathiques, tandis que ceux qui choisissent cette profession dans l'optique d'aider autrui sont plus empathiques. Enfin, la présence aux cours d'éthique a tendance à être liée avec le niveau d'empathie.

L'auto-hypnose a montré ses effets dans la réduction du stress chez les patients. Son application concernant la gestion du stress chez les étudiants en odontologie semble être possible également. La question du stress chez les étudiants en odontologie n'a été que très peu abordée dans la littérature et encore moins en France. Nos études menées au sein de l'UFR Odontologie de Nantes ont permis de mettre en exergue un stress sévère chez les étudiants de 2ème, 3ème, 4ème et 5ème année. Dans ce contexte, l'auto-hypnose semble apporter un outil de gestion du stress. Notre étude clinique comparative montre la diminution de l'anxiété-trait et l'augmentation du sentiment d'auto-efficacité et de la résilience des étudiants.

La première partie de ce manuscrit s'attache à synthétiser le concept de stress et les données épidémiologiques disponibles sur la santé des étudiants en santé en France et à l'international. La santé des étudiants en France est qualifiée de préoccupante par l'Observatoire national de la vie étudiante. Plus spécifiquement, les étudiants en santé sont très concernés par les fragilités psychologiques qui peuvent s'expliquer par la pression et le stress auxquels ils sont soumis durant leurs études. Concernant les étudiants en odontologie, les enquêtes « Bien-être » conduites en 2018 (4) et en 2021 (5) par l'Union Nationale des Étudiants en Chirurgie Dentaire (UNECD), rapportent qu'ils qualifient leurs études par le mot « stress ». Le stress perçu par les étudiants en odontologie en France n'est pas un phénomène isolé. La littérature internationale sur le sujet est abondante. Les études d'odontologie sont connues pour être difficiles et exigeantes, de par le volume de connaissances et la diversité des compétences que les étudiants doivent acquérir. Créer un environnement d'apprentissage positif constitue un défi pour les Facultés de chirurgie- dentaire et implique d'identifier les sources de stress perçues par les étudiants. La deuxième partie présente les sources de stress identifiées dans les études en odontologie, rapportées dans la littérature. La troisième partie vise à proposer, à partir de la littérature, des pistes de réflexions pour diminuer le stress des étudiants au cours de leurs études. Ces pistes sont articulées autour des méthodes pédagogiques, de la formation clinique et de son évaluation, et de la formation des étudiants à la gestion du stress.
