Les professionnels de santé et par conséquent les chirurgiens-dentistes, sont très souvent en première ligne dans le dépistage de ce phénomène. Méthodes : Nous avons réalisé un questionnaire articulé autour du dépistage, de la législation ainsi que la thérapeutique que nous avons par la suite envoyé à 700 praticiens de l'Hérault avec la participation du Conseil Départemental de l'Ordre des Chirurgiens-Dentistes de l'Hérault. Résultats : Nous avons eu 108 réponses mais nous avons pu en garder seulement 99. Une grande majorité de dentistes (83,7%) ne pensent pas être capable de dépister les violences conjugales quand ils y sont confrontés cependant 68,3% d'entre eux estiment que nous avons un rôle important dans le dépistage. Discussion : Malgré un manque de connaissance sur le sujet, les chirurgiens-dentistes se sentent impliqués dans la prise en charge des victimes de violences conjugales.

En France, 223 000 femmes sont victimes de violence conjugale et 1 femme décède tous les 2,7 jours. Les conséquences générales touchent à la santé physique, sexuelle et génésique, mentale et comportementale. La sphère oro-faciale peut être aussi traumatisée par les violences physiques et la santé bucco-dentaire, affectée. De nombreux outils ont été développés pour aider les professionnels de santé à prendre en charge les femmes victimes de violences, y compris pour les chirurgiens-dentistes. La Normandie est aussi touchée que les autres régions françaises en matière de violences faites aux femmes. Pourtant, les chirurgiens-dentistes paraissaient peu impliqués dans cette prise en charge spécifique. Une enquête a donc été menée dans les départements de l'Eure et de Seine-Maritime, visant à décrire les connaissances et attitudes des praticiens à l'égard des femmes victimes de violences, ainsi qu'à d'identifier les principaux obstacles au dépistage de ces victimes. Ce travail a pour but d'exposer les résultats de cette étude. Le plan s'organise de la façon suivante : généralités et introduction à l'étude, matériel et méthodes, résultats, discussion et conclusion.

Étant donné la fréquence importante des lésions oro-faciales et des récentes politiques publiques de protection de l'enfance, nous nous sommes interrogés sur le rôle primordial du chirurgien-dentiste dans la détection et la prise en charge de ces violences au cabinet dentaire. Une revue de la littérature a de ce fait été réalisée pour appréhender les connaissances et l'attitude du chirurgien-dentiste face à des cas de maltraitance infantile. Pour illustrer cette revue, nous avons réalisé une étude auprès des praticiens d'Ile-de-France afin d'évaluer la sensibilisation face à ce problème de santé publique. Alors même que 49 % des praticiens interrogés ont eu des soupçons ou des cas avérés de maltraitance au cours de leur carrière, seulement 8 % d'entre eux ont décidé d'agir. Plus de 90 % des dentistes se déclarent incompétents pour déceler une maltraitance infantile. De ce fait, de nombreuses lacunes subsistent et il est nécessaire de mettre en place des formations et des actions de sensibilisation pour que les chirurgiens-dentistes deviennent des acteurs de la protection de l'enfance.

Chaque professionnel de santé se doit d'être compétent dans la détection, la prise en charge et l'orientation des victimes. En particulier les chirurgiens dentistes puisque les lésions sont en majorité centrées au niveau de la face, de la tête et du cou. En Europe, la formation reste très disparate sur le sujet. En France, le besoin en formation a été mis en évidence. L'objectif de l'étude est d'élaborer un enseignement aux étudiants de sixième année de chirurgie dentaire et d'évaluer son impact sur la connaissance de ces derniers et sur leur perception de leur compétence. Un questionnaire a été auto-administré avant et après la dispense de l'enseignement. Les résulats mettent en évidence l'intérêt de dispenser un enseignement sur les violences conjugales. Avant la dispense du cours, le taux moyen de bonnes réponses était de 48 % ce qui est relativement faible. Après le cours, le taux s'élève alors à 95.4%. La dispense de cet enseignement à également eu un impact sur la perception que les étudiants ont de leurs compétences, en effet, avant ce cours, 95% d'entre eux se disaient incompétents dans le domaine, à la suite du cours, 69.4% se percoivent compétents. Cet enseignement a donc permis une augmentation du niveau de connaissance des étudiants ainsi qu'une amélioration de la perception de leurs compétences. Cependant, cette formation ne peut pas rester purement théorique pour être totatlement efficace. En effet, la complexité du phénomène des violences conjugales rend la prise en charge délicate. Ces situations restent difficiles à gérer pour tout professionnel de santé malgré une formation complète.

Les chirurgiens-dentistes sont des professionnels de santé de premier recours devant être formés au dépistage et à l'accompagnement des femmes victimes de violences. Ce dépistage peut toutefois être compliqué par les difficultés qu'éprouvent les femmes à aborder cette situation avec leur praticien. Pourtant, aucune attention n'a été portée à la connaissance, aux perceptions et aux attentes de ces femmes quant à l'aide que les chirurgiens-dentistes sont à même de leur apporter. Compte-tenu de la dimension bilatérale de la relation soignant-soigné, ce travail avait pour but de combler ce manque en donnant la parole aux femmes concernées. Les objectifs de notre étude étaient ainsi d'analyser les perceptions et attentes des femmes victimes de violences conjugales quant aux rôles du chirurgien-dentiste et de décrire les freins à l'ouverture du dialogue et les perspectives d'amélioration proposées. Méthodes : Étude qualitative par entretiens individuels semi-dirigés réalisés de novembre 2021 à mars 2022 auprès de 6 femmes ex-victimes de violences conjugales vivant en France. Résultats : Les perceptions des rôles du chirurgien-dentiste dans le contexte des violences conjugales restaient majoritairement limitées au rôle de soignant. En comparaison, de fortes attentes existaient quant au développement des compétences relationnelles. Les femmes étaient favorables à l'amélioration de la visibilité des rôles du chirurgien-dentiste et au fait de pouvoir se confier à leur chirurgien-dentiste. Conclusion : Les perceptions des rôles du chirurgien-dentiste ne correspondaient pas aux attentes exprimées par les femmes victimes de violences. Cette étude soulève la nécessité de continuer à apporter des solutions pour répondre à l'ensemble de ces attentes et faire face aux freins encore nombreux entravant la libération de la parole des femmes victimes de violence au cabinet dentaire.

She argues that Vichy wanted to return France to an illustrious and largely mythical past of harmony, where citizens all knew their places and fulfilled their responsibilities, where order prevailed. The National Revolution, according to Pollard, replaced the ideals of liberty, equality, and fraternity with work, family, and fatherland, making the acceptance of traditional masculine and feminine roles a key priority.
