Book excerpt: Qu'est ce que l'Infirmité Motrice Cérébrale? Quelle est son origine? Quelles sont ses conséquences ? La première partie de ce mémoire traite de la déficience motrice, en la replaçant dans un contexte général. D'après la définition que donne Tardieu, l'Infirmité Motrice Cérébrale est la conséquence de lésion au cerveau avant, autour ou peu après la naissance, qui devient la cause de perturbations du contrôle moteur sur la posture et les mouvements du corps, à des degrés divers. Suivant l'étendue de leur lésion, les enfants présentent une quadriplégie, une diplégie, une hémiplégie... La spasticité, l'athétose et l'ataxie sont des troubles neuro-moteurs qui affectent les mouvements des membres et sont à l'origine d'une déficience motrice. Celle-ci place la personne qui en est atteinte en situation de handicap. Son autonomie étant soit retardée, soit impossible. Comment cette situation est-elle appréhendée par la famille? Comment s'établissent les premiers échanges, puis la communication? Comment se développe le langage des enfants IMC ? Les conséquences de l'Infirmité Motrice Cérébrale touchent surtout le versant expressif du langage: troubles de la parole (dysarthrie), troubles pneumo-phoniques, dyspraxies... Le développement du langage est ensuite particulier à chaque enfant. Je me suis intéressée dans la deuxième partie à la rééducation et notamment sur ce que les différents professionnels intervenant auprès de l'enfant pouvaient lui proposer, à lui et à sa famille. Travail éducatif, rééducatif, thérapeutique, portant sur la motricité, sur les fonctions cognitives, le langage, l'autonomie, tout cela se conçoit en équipe et après une évaluation globale de l'enfant. En orthophonie, deux approches ont été proposées aux enfants, elles allient des gestes, des massages, des traces de couleur, du rythme, des pictogrammes. .. pour soutenir et favoriser le langage. Les effets sont significatifs.

Des systèmes de communication alternative et augmentée peuvent alors leur être proposés. Ce mémoire a pour objectif d’étudier la manière dont quatre enfants IMC/IMOC non verbaux découvrent et s’approprient leur outil de communication bénéficiant d’une technologie de synthèse vocale. Dans la partie théorique, nous avons présenté les différentes caractéristiques de l’Infirmité Motrice Cérébrale ou d’Origine Cérébrale. Nous avons également rappelé les étapes majeures du développement normal du langage oral et de la communication. Enfin, nous avons évoqué les grands principes de la communication alternative et augmentée, ainsi que les multiples outils qui peuvent être proposés à ces sujets privés de parole. Dans la partie pratique, nous avons observé le langage de jeunes enfants qui s’expriment à l’aide d’un code pictographique installé sur une synthèse vocale. Nos observations confirment l’idée que la synthèse vocale n’est que le support de la parole des sujets et ne dispense pas d’un apprentissage long et complexe des structures du langage, qui doit être étayé par la prise en charge orthophonique.
