A sizeable body of evidence supports the role of both excessive endogenous estrogen and exogenous estrogen in the etiology of endometrial cancer. There is growing evidence that inadequate progesterone has the opposite effect, in that progesterone supplementation of postmeno pausal estrogen therapy reduces the incidence of endometrial cancer. Despite this new awareness of the hormonal role that is played in carcino ma of the endometrium, the disease still plagues the oncologist. The general approach to carcinoma of the endometrium in the United States is that of primary surgical staging. This provides the maximum amount of information to best plan postoperative radiation therapy and/or chemotherapy for these patients. In general, patients who are considered candidates for surgical staging are those with advanced disease or high-risk stage I endometrial carcinoma. High-risk endometrial carcinoma is defined as those patients with moderately differentiated lesions with deep myometrial invasion, poor ly differentiated carcinoma of the endometrium, and the high-risk histolo gies such as papillary carcinoma and clear-cell carcinomas. It tends to develop after menopause, when a woman is between the ages of 50 and 60. It is the sixth most common cancer in women worldwide (fourteenth most common cancer overall), with 320,000 new cases diagnosed in 2012, and it is the 13th most common cause of death from cancer with nearly 86,000 deaths worldwide. Endometrial cancer occurs when cells of the endometrium undergo a transformation and begin to grow and multiply without the control mechanisms that normally limit their growth. As the cells grow and multiply, they form the cancerous lesion. Cancerous tumors may encroach on and invade neighboring organs or lymph nodes, or they may enter the bloodstream and spread to the bones or distant organs, such as the lungs. This process is called metastasis. Metastatic tumors are the most aggressive and serious of all tumors. Two main types of endometrial cancers exist. Nearly all endometrial cancers are endometrial adenocarcinomas, as they originate from glandular (secreting) tissue. The other type of endometrial cancer, uterine sarcomas, originates in the connective tissue or muscle of the uterus. A subtype of endometrial adenocarcinomas, adenosquamous carcinoma, includes squamous cells (that is, the type of cells found on the surface of the skin and cervix). Other subtypes of endometrial adenocarcinomas are papillary serous adenocarcinomas and clear cell carcinomas. Multidisciplinary teams of experts including specialists in medical oncology, gynecologic oncology, radiology, urology, radiotherapy, and surgery are essential to utilize sophisticated imaging and laboratory tests, to confirm the diagnosis and identify the characteristics of the tumor. They all work together to determine the best treatment approach for the patient. Recent progress in the development of new surgical techniques has transformed the treatment of uterine cancer, resulting in greater surgical precision and fewer complications. In addition, targeted adjuvant therapy has become useful in improving the oncologic outcome of patients with this disease.

And also, one must reflect on how this book is likely to be utilized in this era of rapid communications. The Editors first met in May 2003 at a stimulating Italian symposium on endometrial cancer (organizers Drs. Luigi Frigerio, Roberto Grassi and Andrea Lissoni, with participation of the deans of Italian Gynecologic Oncology, Ugo Bianchi and Constantino Mangioni) that took place at Bergamo and Caravaggio. The impressive gains in biology and clinical trials were further discussed by the two editors and others that are co-authors in this venture on this side of the Atlantic at a 2004 Educational Session at the American Society of Clinical Oncology (ASCO). The pace of progress in various aspects of management of uterine cancer was no- worthy, not only was tumor biology fueling novel hypotheses such as questioning the mesenchymal origin of carcinosarcomas, but knowledge of molecular pathways was beginning to be applied as prognostic and as predictive factors portending benefit from systemic therapies. Surgical staging and sensitive imaging provided the underpinning for refining our treatment algorithms. Finally, a role for che- therapy had finally become established, principally through phase III studies c- paring chemotherapy to radiation in mostly locally advanced stages III and IV that had undergone resection.
