Book excerpt: There are approximately 1.4 million trans-identified individuals in the US alone, many of whom will undergo gender-affirming medical or surgical interventions to better align their appearance with their gender identity. Multiple major medical societies recommend fertility preservation counseling prior to starting any gender-affirming therapies, but data are limited on the reproductive effects of common gender-affirming hormone regimens. The burden of fertility counseling falls to the hormone providers and surgeons that are encountering these patients, many of whom will not have had adequate training or resources to provide evidence-based recommendations and options. Additionally, many reproductive health care providers are not trained in how to care for gender minorities. The purpose of this book is to be a reference for clinicians and researchers in the field of transgender medicine, to provide up-to-date data and resources to properly counsel transgender and nonbinary patients about the reproductive consequences of gender-affirming interventions and their options for family-building, and to educate providers about appropriate and culturally competent reproductive health care. Effects of masculinizing and feminizing hormone therapy, as well as the fertility preservation options available, are discussed in detail for both adults and youth. In addition to these medical considerations, both psychosocial, legal and ethical considerations are highlighted for a more well-rounded presentation. A final chapter describes how to create a welcome and accepting clinical environment. Such a reference does not currently exist, leading to the propagation of misinformation and encouraging patients to seek nonmedical sources, such as social media, for their information. In pregnancy, the uterus acquires this impor tance with the arrival of the fertilized egg, which takes up residence for periods ranging from about 2 weeks in the opossum to about 2 years in the elephant. The arrival of the embryo signals a crucial time for the establishment of pregnancy. For several days the blas tocyst remains free in the uterine lumen, where it depends on uterine secretions for its sur vival and differentiation. During this time, essential changes in the endometrium take place in preparation for attachment of the blastocyst and implantation. Early embryonic loss is an economic problem of global proportions in animal husbandry, where, in pigs and cattle for example, some 30% of all fertilizations fail to result in a pregnancy. In humans this figure may be even higher, and estimates of early spontaneous abortions range from 40 to 60% of all conceptions.

Book excerpt: Although the physiology of the menstrual cycle appears clear and easily explained by a balance in the concentration of various sex steroid hormones, numerous details of its mechanism are still poorly understood and little is known about the relationship among clinical events, plasma hormone concentrations, molecular impacts on target tissues and their regulation. In the following chapters, the authors have attempted to establish a correlation between endometrial histology and well-understood physiologic events of the menstrual cycle. They have provided up-to-date information on the effects of various hormones and combinations of hormones on the endometrium. The interdependence of endometrial morphology, molecular biology, endocrinology and physiology, provides grounds for a better understanding of the complex mechanism of the menstrual cycle, and sheds some light on its pathophysiology. Such an approach adds ariother dimension to interpretation of many menstrual abnormalities and numerous aspects of infertility in women with normal physiognomies and apparently regular menstrual cycles. The pathologist must be aware of these new concepts since a knowledge of functional changes reflected in hormone serum levels and sex steroid receptor concentrations allows a mare detailed analysis and a better interpretation of the structural features of the endometrium. This information placed in the proper clinical context can help the gynecologist provide optimal therapy. The reader will find a valuable reference for a synthesis of clinical, morphological, and biochemical data related to the menstrual cycle and its aberrations.

Book excerpt: This book presents an overview of the diagnostic performance of non- or semi-invasive tests for endometriosis in peripheral blood, endometrium, saliva, peritoneal fluid and urine. The value of existing and emerging systems biology technologies for biomarker development is addressed in several chapters on genetics, microarrays, proteomics and metabolomics. Although tests with high sensitivity and acceptable specificity have been developed, sometimes validated in independent populations and seem promising, more research is needed to translate these data into clinical benefit for patients and coordinate efforts internationally to standarize analysis, reports and operating procedures. The gold standard to diagnose endometriosis is currently through laparoscopic inspection with histological confirmation, a surgical procedure with rare but significant potential risks for the patients. A non-invasive test for endometriosis would be critical for the early detection of endometriosis of symptomatic women with pelvic pain and/or subfertility with normal ultrasound. This would include nearly all cases of minimal-mild endometriosis, some cases of moderate-severe endometriosis without a clearly visible ovarian endometrioma and cases with pelvic adhesions and/or other pelvic pathology, who might benefit from surgery to improve pelvic pain and/or subfertility. Such a test would also be useful in symptomatic women with ultrasound imaging suspicious for endometriosis, since it may be difficult to differentiate an ovarian endometrioma from other ovarian cysts and since the quality of ultrasound imaging is highly variable worldwide.
