Book excerpt: The endodontic triad; consisting of biomechanical preparation, microbial control and complete obturation of the canal space; remains the basis of endodontic therapy. Complete obturation of pulp space as efficiently and three dimensionally as possible is considered to be an important part of root canal treatment. Endodontic literature reports the need to seal the root canal in a hermetic way i.e. the canal system should be sealed apically, coronally and laterally. Leonardo and Leal (1991) affirmed that to seal a root canal means to fill it in all its extension with an inert, antiseptic material, obtaining the most hermetic seal possible. Various endodontic materials have been advocated for obturation of the radicular space. Most techniques employ a core material and a sealer. All core materials require a sealer. There are no exceptions to this rule. Although there are a large number of filling materials, the combination of gutta-percha and a sealer is most widely used in clinical practice. The purpose of sealing root canals is to prevent periapical exudates from diffusing into the unfilled part of the canal, to prevent re-entry and colonization of bacteria .

Book excerpt: The main objective of this book is to reduce endodontic failure, increase the success rate of endodontic treatment and decrease the percentage of tooth extraction so as to conserve a naturally compromised tooth structure as opposed to having a dental implant. The first study focuses on the possibility of having a filling material which could provide a tight seal with the desired properties. The in Vitro study targeted on elimination of microleakage by measuring the ability of the existing materials to obturate the root canal, 3 dimensionally using different obturation techniques. This was done by correlating the reading using the electrochemical machine and Micro CT Scan. This study adds value to the fact that the currently developed Nano HA based endodontic sealer is being produced locally in Malaysia thereby reducing the cost as compared to its imported counterpart. The third study consisted of testing the possibility of reinforcing over-flared root canal with Mineral trioxide aggregate (MTA) and also illustrated the pattern of root fracture. This development will definitely help economize the treatment thereby providing a good solution for young patients under 18 years of age.

Book excerpt: Shorttitle: The development and maintenance of apical seal is desirable and considered to be a major prerequisite to improve the outcome of root canal treatment. Basically, two materials are used in combination for the root canals filling; a semisolid material (gutta-percha) and a sealing cement. Many types and brands of endodontic sealers are commercially available, but none of them fully satisfy all requirements and able to provide a fluid- tight seal. To address this problem the School of Dental Sciences, USM, has prepared a new experimental nano HA-filled epoxy resin based endodontic sealer using a simple and inexpensive technique. Nano-HA is biocompatible, reactive and has capability to adhere to the dentinal tubules. Moreover, it promotes apical healing and is able to produce hermatic apical seal. Different filling techniques based on heated or preheated gutta-percha have been introduced to enhance three-dimensional filling of the root canal. Little is known regarding the apical sealing ability of the new experimental nano-HA sealer when used with different obturation techniques.
