The full and sustainable development of the territories that are falling behind is not only an obligation in terms of the realization of the rights of their inhabitants; it would also allow these territories to activate their social, economic, environmental and cultural potential, to the benefit of all societies.It is hoped that the 2020 Regional Overview will help to highlight the challenges experienced by the territories with the worst indicators in terms of food and nutrition, and that it will serve to mobilize political commitment and public attention towards those areas that are most highly laggingThe year 2020 will be remembered for many decades as the year of the COVID-19 pandemic. The indicators of the 2030 Agenda that are used in this publication do not yet show the different impacts of COVID-19. However, there are references to the possible implications of the pandemic for the future.

Book excerpt: Colombia’s response to the country’s drug problem has been based on the repression of the weakest links in the drug chain—namely consumers and small farmers—which has led to disproportionate rates of imprisonment and has involved a heavy focus on forced crop eradication. Not only has such an approach failed to effectively control the cocaine market, but it has also unleashed harmful side effects in terms of security, social development, and human rights as they concern communities in coca-growing areas. Moreover, although scholars and practitioners have analyzed Colombia’s drug problem from a variety of perspectives, these efforts have tended to overlook women’s experiences. This report explores the ways that rural norms, gender structures, the armed conflict, and illegal markets have played out in the lives of women coca growers in Colombia’s Andes-Amazon region, an area distinguished by the presence of illegal armed groups, violence, poverty, and weak state institutions. In this region of Colombia, coca cultivation has offered an important source of income for rural families, which in turn has affected women’s roles in society and has placed them in a vulnerable position vis- à-vis armed actors. The Andes-Amazon region is an area where the country’s war on drugs and its armed conflict converged and unmasked the gender structures dominating the countryside. These structures affected rural women in various ways: through everyday violence, the fumigation of illicit and licit crops alike, and women’s stigmatization due to their involvement in an illegal trade. But coca was also a source of livelihood that helped them attain economic independence and gave them the ability to improve their well-being and that of their families. The recent peace accord signed between the Colombian government and the country’s main guerrilla group represents a historic opportunity to learn from past mistakes in terms of the illicit crop problem and the social and political demands of coca-growing communities. Against this backdrop, it is time to recognize the contributions that women coca growers have made in both the public and the private spheres toward the construction of a peaceful countryside in the most remote and forgotten regions of the country.

Book excerpt: Flexible Families examines the struggles among Nicaraguan migrants in Costa Rica (and their families back in Nicaragua) to maintain a sense of family across borders. The book is based on more than twenty-four months of ethnographic fieldwork in Costa Rica and Nicaragua (between 2009 and 2012) and more than ten years of engagement with Nicaraguan migrant communities. Author Caitlin Fouratt finds that migration and family intersect as sites for triaging inequality, economic crisis, and a lack of state-provided social services. The book situates transnational families in an analysis of the history of unstable family life in Nicaragua due to decades of war and economic crisis, rather than in the migration process itself, which is often blamed for family breakdown in public discourse. Fouratt argues that the kinds of family configurations often seen as problematic consequences of migration—specifically single mothers, absent fathers, and grandmother caregivers—represent flexible family configurations that have enabled Nicaraguan families to survive the chronic crises of the past decades. By examining the work that goes into forging and sustaining transnational kinship, the book argues for a rethinking of national belonging and discourses of solidarity. In parallel, the book critically examines conditions in Costa Rica, especially the ways the instabilities and inequalities that have haunted the rest of the region have begun to take shape there, resulting in perceptions of increased crime rates and a declining quality of life. By linking this crisis of Costa Rican exceptionalism to recent immigration reform, the book also builds on scholarship about the production and experiences of immigrant exclusion. Flexible Families offers insight into the impacts of increasingly restrictive immigration policies in the everyday lives of transnational families within the developing world.

Las reformas han venido acompafiadas por un incremento en el gasto publico del sector salud, financiado en gran pa rte por los ingresos fiscales generales que priorizan o estan dirigidos explfcitamente hacia las poblaciones sin capacidad de pago. Los compromisos polfticos se han traducido en general en presupuestos mas elevados yen leyes aprobadas que circunscriben los fondos destinados al sector salud. En la mayorfa de los pafses se ha priorizado la atenci6n primaria en salud por su costo efectividad y han adoptado metodos de adquisici6n que incentivan la eficiencia y la transparencia en los resultados y que les dan a los administradores del sector salud un mayor apalancamiento para dirigir a los proveedores hacia la . consecuci6n de las prioridades de salud publica. Sin embargo, a pesar de los avances. aun subsisten las disparidades en el financiamiento y calidad de los servicios de los subsistemas de salud. Cumplir con el compromiso de la cobertura universal de salud exigira esfuerzos concertados para mejorar la generaci6n de los ingresos fiscales de manera sostenible y de elevar la productividad y efectividad del gasto. En el re po rte Hacia la cobertura universal en salud v la equidad en America Latina v el Caribe: Evidencia de poises seleccionados. los autores sefialan que la evidencia tomada de un analisis de 54 encuestas de hogares corrobora que las inversiones en la ampliaci6n de la cobertura estan generando resultados. A pesar que los pobres aun presentan los peores fndices de salud en comparaci6n con los ricos, las disparidades se han reducido en gran medida, especialmente durante las primeras etapas de vida. Los pafses han alcanzado elevados niveles de cobertura y equidad en la utilizaci6n de los servicios de salud maternoinfantil. El panorama se torna mas lleno de matices y no tan positivo cuando se trata de la salud en adultos y de la prevalencia de condiciones y enfermedades cr6nicas. La cobertura de las intervenciones de enfermedades no transmisibles no es muy alta y la utilizaci6n de los servicios se inclina hacia aquellos que gozan de una mejor posici6n. La prevalencia de las enfermedades no transmisibles no ha mostrado el comportamiento esperado en vista de la cafda en las tasas de mortalidad: un mayor acceso a los servicios de diagn6stico por pa rte de los segmentos mas ricos podrfa encubrir cambios en la prevalencia real. Los gastos de salud provocados por situaciones catastr6ficas han ido en descenso en la mayorfa de los pafses. El panorama en torno a la equidad, sin embargo es mixto, apuntando hacia limitaciones en la medida. Si bien la tasa de empobrecimiento a causa de gastos de salud es baja, con tendencia decreciente en terminos generales, entre dos y cuatro millones de personas en los pafses estudiados aun caen por debajo de la lfnea de la pobreza despues de haber incurrido en gastos de salud. Los esfuerzos para darle un seguimiento sistematico a la calidad de la atenci6n en la region, apenas dan sus primeros pasos. Sin embargo, un repaso a la literatura revela graves deficiencias en la calidad de la atenci6n de salud, asi como sustanciales diferencias entre los diversos subsistemas. Elevar la calidad de la atenci6n y asegurar la sostenibilidad de las inversiones en salud siguen siendo una agenda inconclusa.
