Cela a pour conséquence de favoriser les risques iatrogènes. Parmi ces pathologies chroniques, le diabète de type 2 est en plein essor et sa prévalence est plus élevée dans les régions d'outre mer et notamment à la Réunion. La pharmacie clinique est une discipline à part entière dont le but est d'optimiser l'observance thérapeutique des patients. Aujourd'hui, le monde de l'officine prend une nouvelle tournure et le pharmacien se voit confié des nouvelles missions qui le recentre comme acteur central du parcours de soins ; c'est dans ce contexte que la pharmacie clinique trouve toute sa place. La pharmacie clinique s'exerce autour de trois axes principaux : la Dispensation des produits de santé, le Bilan Partagé de Médication et le Plan Personnalisé de Soins. C'est au cours du bilan partagé de médication que l'on peut déceler des lacunes ou des problèmes amenant le patient à ne pas être observant. Une simple intervention pharmaceutique auprès du médecin peut suffire mais nous pouvons également mettre en place des actions éducatives qui ont pour but de favoriser l'observance des patients. Au cours de mon stage officinal à la Réunion, j'ai réalisé 7 bilans de médication partagés auprès de 7 patients diabétiques de type 2. Il s'agit de réaliser 7 étapes dont, entre autres, l'anamnèse des traitements, l'entretien avec le patient, l'analyse, la transmission au médecin, un entretien conseil pour communiquer au patient les modifications apportées ainsi qu'un entretien de suivi. Je me suis aidée du questionnaire fourni par la Société Française de Pharmacie Clinique.

Book excerpt: Le diabète de type Il est une pathologie chronique est plein essor ; son incidence et sa prévalence ne cessent d'augmenter, malgré le fait que 80 % des cas de diabètes de ce type pourraient être évités par des mesures hygiéno-diététiques bien conduites. Dans la même tendance, pourtant considérée comme pathologie du sujet adulte après 40 ans, on remarque que le nombre de cas de diabète de type Il augmente chez les jeunes, enfants et adolescents, notamment en raison de la sédentarité, fléau du XXI ème siècle.Ici, l'enjeu a consisté en l'élaboration d'outils pédagogiques et en leur évaluation dans le cadre de l'accompagnement pharmaceutique des patients atteints de pathologies chroniques, en ciblant spécifiquement la pathologie du diabète de type Il. L'objectif était de s'investir dans cette nouvelle mission afin de comprendre le rôle du pharmacien dans les entretiens pharmaceutiques (EP), et plus généralement dans l'Education Thérapeutique du Patient (ETP). Dans un premier temps a donc été étudiée la place que le pharmacien occupe dans le système de santé, avec l'évolution qu'a connue cette profession grâce à diverses lois. Par la suite, nous nous sommes focalisés sur I'ETP et les EP en tant que nouvelle mission confiée aux professionnels de santé en incluant les pharmaciens. Puis, dans l'objectif de comprendre l'intérêt d'élargir les thèmes d'accompagnement pharmaceutique autorisés, ia pathologie du diabète a été étudiée plus en détails, de la physiopathologie jusqu'à la prise en charge et le suivi médical. Finalement, la dernière partie résume le projet mis en place au sein d'une officine rurale sur la période d'avril à juin 2020.Grâce à l'analyse bibliographique, les obstacles à la mise en place d'entretiens pharmaceutiques ont été mis en évidence pour comprendre les éventuelles pistes d'amélioration. Après avoir construit trames et outils pédagogiques, ceux-ci ont pu être exploités avec plusieurs patients de profils différents lors d'entretiens individuels, et ce malgré le contexte sanitaire exceptionnel avec l'épidémie mondiale de la Covid-19. Dans une démarche d'amélioration, une analyse critique du projet a été réalisée : il en ressort que la pluridisciplinarité et l'investissement avec une posture éducative sont des éléments clés de la démarche d'ETP et d'EP, mais aussi que le pharmacien a toute légitimité dans cette nouvelle mission à condition de s'investir

Une étude de besoins, à l’aide d’entretiens semi-directifs, a été réalisée auprès de 16 patients diabétiques de type 2 et 13 professionnels de santé (hospitaliers et libéraux), afin d’identifier les difficultés et attentes de ces acteurs. Cette analyse montre la différence de besoins entre les patients et les soignants et met en évidence la nécessité de prendre en compte ceux des patients pour proposer des offres éducatives cohérentes. L’approche multiprofessionnelle garant d’une démarche d’éducation thérapeutique du patient de qualité nécessite une coordination des soignants autour du patient. Le partage d’outils de communication simple, créés et partagés par les acteurs (cahier d’éducation « porté » par le patient...) ainsi que des actions de formations communes peut permettre de créer et soutenir cette coordination et harmoniser le discours transmis aux patients.

Well-to-do townspeople displayed a German novelty, the decorated fir tree, in their parlors; an enterprising printer discovered the money to be made from Christmas cards; and a hodgepodge of year-end celebrations began to coalesce around December 25 and the figure of Santa. The homecoming significance of the holiday increased with the Civil War, and by the end of the nineteenth century a full- fledged national holiday had materialized, forged out of borrowed and invented custom alike, and driven by a passion for gift-giving. In the twentieth century, Christmas seeped into every niche of our conscious and unconscious lives to become a festival of epic proportions. Indeed, Restad carries the story through to our own time, unwrapping the messages hidden inside countless movies, books, and television shows, revealing the inescapable presence--and ambiguous meaning--of Christmas in contemporary culture.
