Book excerpt: This case study reports on a two-stage empirical ethics research project, focusing on the second stage of the research where Delphi method was used as a form of consensus building. Conducted as part of my MA-PhD, the goal of this project was to better understand empirical ethics in long-term care practices and the interaction between public policy and the ethical environment in long-term care. Findings from the first stage of the project revealed that frontline staff frequently experience moral distress being aware of the ethical thing to do, although feeling powerless to act accordingly as a result of either real or perceived institutional constraints. The emerging theme of moral distress informed the second stage of the research where Delphi methodology was used to facilitate consensus building among stakeholder experts, through an iterative process of knowledge production, reflection, and translation. The goal of this case study is to provide a better understanding of the Delphi method generally, and specifically as a consensus building method in empirical ethics research. The case study will (a) provide a brief overview and context of the project; (b) share my experiential account of designing the research project, examining methodology, empirical ethics, and consensus building; (c) explore the method in action-a step-by-step account of how the Delphi method was carried out; (d) provide a candid report of the research practicalities I encountered; and (e) share some practical lessons I have learned, which I hope will be helpful to you in some way as you journey through your research.

While defending the concept of autonomy, the author argues that the standard view of autonomy as non-interference and independence has only a limited applicability for long-term care. He explains that autonomy should be understood as a comprehensiveness that defines the overall course of a person's life rather than as a way of responding to an isolated situation. Agich distinguishes actual and ideal autonomy and argues that actual autonomy is better revealed in the everyday experiences of long-term care than in dramatic, conflict-ridden paradigm situations such as decisions to institutionalize, to initiate aggressive treatments, or to withhold or to withdraw life-sustaining treatments. Through a phenomenological analysis of long-term care, he develops an ethical framework for it by showing how autonomy is actually manifest in certain structural features of the social world of long-term care. Throughout this timely work, the rich sociological and anthropological literature on aging and long-term care is referenced and the practical ethical questions of promoting and enhancing the exercise of autonomy are addressed.
