Climate change, transportation incidents, increasing numbers of mass gatherings, chemical and technical incidents, increasing number of armed conflicts and terrorism are some of the major reasons for this increasing pattern. Any of these events may result in severe casualties, destruction of infrastructures and create a situation in which the number of victims may exceed available resources. Much of the knowledge in disaster medicine is based on the “lessons learned”. However, it has been proven that no lessons learned theoretically can be of practical use if the knowledge is not tested in an appropriate environment. In this perspective, the major clinical excellence and testing laboratories for disaster response are the scenes where disasters happen. Yet, most of the research and reports in the field are produced in high-income countries, while most of the disasters happen in middle- or low-income countries. There is thus a need to bring these two environments together in order to translate theory into practice and among people who are highly exposed and involved in the multidisciplinary management of a disaster or major incident.

The aim of programs such as this is not to re-educate people in respect to their profession/area of expertise, but rather to train them in elements specific to DM and ways in which they can apply their existing knowledge and professional expertise in the disaster context. Such multidisciplinary programs cover all disaster phases, accommodate the needs of each organization, and offer a safe environment where participants are able to learn by doing in an interactive environment--vertically within their own organization and horizontally together with other organizations--by focusing on shared training, cooperation, and intercultural understanding. Such programs bring some degree of standardization and offer a thorough evaluation of participants and the program by using evidence-based scientific methods. One important part of the program is to offer a standardized handbook in which all necessary information is gathered. This is not intended as a substitute for reference books, but it is intended to make the information accessible and easy to learn so that the information can be combined with the practical parts of the education

Book excerpt: This book is a historical review of international food and agriculture since the founding of the international organizations following the Second World War, including the World Bank and the Food and Agriculture Organization of the United Nations (FAO), the World Food Programme (WFP) and into the 1970s, when CGIAR was established and the International Fund for Agricultural Development (IFAD) was created to recycle petrodollars. Despite numerous international consultations and an increased number of actors, there has been no real growth in international assistance, except for the work of the Bill and Melinda Gates Foundation. The book concurrently focuses on the structural transformation of developing countries in Asia and Africa, with some making great strides in small farmer development and in achieving structural transformation of their economies. Some have also achieved Sustainable Development Goals (SDGs), particularly SDG2, but most have not. Not only are some countries, particularly in South Asia and sub-Saharan Africa, lagging behind, but they face new challenges of climate change, competition from emerging countries, population pressure, urbanization, environmental decay, and dietary transition. Lagging developing countries need huge investments in human capital, and physical and institutional infrastructure, to take advantage of rapid change in technologies, but the role of international assistance in financial transfers has diminished. Book excerpt: In Sub-Saharan Africa, the scale of undernutrition is staggering; 58 million children under the age of five are too short for their age (stunted), and 14 million weigh too little for their height (wasted). Poor diets in terms of diversity, quality, and quantity, combined with illness and poor water and sanitation facilities, are linked with deficiencies of micronutrients—such as iodine, vitamin A, and iron—associated with growth, development, and immune function. In the short term, inequities in access to the determinants of nutrition increase the incidence of undernutrition and diarrheal disease. In the long term, the chronic undernutrition of children has important consequences for individuals and societies: a high risk of stunting, impaired cognitive development, lower school attendance rates, reduced human capital attainment, and a higher risk of chronic disease and health problems in adulthood. Inequities in access to services early in life contribute to the intergenerational transmission of poverty. Recent World Bank estimates suggest that the income penalty a country incurs for not having eliminated stunting when today’s workers were children is about 9†“10 percent of gross domestic product per capita in Sub-Saharan Africa. Much of the effort to date has focused on the costing, financing, and impact of nutrition-specific interventions delivered mainly through the health sector to reach the global nutrition targets for stunting, anemia, and breastfeeding, and interventions for treating wasting. However, the determinants of undernutrition are multisectoral, and the solution to undernutrition requires multisectoral approaches. An acceleration of the progress to reduce stunting in Sub-Saharan Africa requires engaging additional sectors—such as agriculture; education; social protection; and water, sanitation, and hygiene (WASH)—to improve nutrition. This book lays the groundwork for more effective multisectoral action by analyzing and generating empirical evidence to inform the joint targeting of nutrition-sensitive interventions. Using information from 33 recent Demographic and Health Surveys (DHS), measures are constructed to capture a child’s access to food security, care practices, health care, and WASH, to identify gaps in access among different socioeconomic groups; and to relate access to these nutrition drivers to nutrition outcomes. All Hands on Deck: Reducing Stunting through Multisectoral Efforts in Sub-Saharan Africa addresses three main questions: • Do children have inadequate access to the underlying determinants of nutrition? • What is the association between stunting and inadequate food, care practices, health, and WASH access? • Can the sectors that have the greatest impact on stunting be identified? This book provides country authorities with a holistic picture of the gaps in access to the drivers of nutrition within countries to assist them in the formulation of a more informed, evidence-based, and balanced multisectoral strategy against undernutrition.

However, while facile generalizations about the lack of demographic change and lack of progress in meeting the MDGs in sub-Saharan Africa are commonplace, they are often misleading and belie the socio-cultural change that is occurring among a vanguard of more educated youth. Even within Ethiopia, the second largest country at the Crossroads of Africa and the Middle East, different narratives emerge from analysis of longitudinal, micro-level analysis as to how demographic change and responses are occurring, some more rapidly than others. The book compares Ethiopia with other Africa countries, and demonstrates the uniqueness of an African-type demographic transition: a combination of poverty-related negative factors (unemployment, disease, food insecurity) along with positive education, health and higher age-of-marriage trends that are pushing this ruggedly rural and land-locked population to accelerate the demographic transition and stay on track to meet most of the MDGs. This book takes great care with the challenges of inadequate data and weak analytical capacity to research this incipient transition, trying to unravel some of the complexities in this vulnerable Horn of Africa country: A slowly declining population growth rates with rapidly declining child mortality, very high chronic under-nutrition, already low urban fertility but still very high rural fertility; and high population-resource pressure along with rapidly growing small urban places”
