La fin du XXème siècle a connu une hausse spectaculaire de l’incidence de diabète de type 2 chez les enfants et les adolescents. Encore rare il y a quelques décennies, cette forme de diabète croît rapidement aux États-Unis, où la prévalence est actuellement la plus importante : dans certaines régions, son incidence devance même celle du diabète de type 1. Elle augmente également au Canada, au Japon, dans les pays d’Europe (France, Italie, Royaume-Uni) et d’autres pays du monde, le plus souvent en cours de transition économique. Dans les sociétés occidentalisées, l’incidence du diabète de type 2 chez les enfants et les adolescents fait suite, sans surprise, à l’actuelle épidémie d’obésité, résultat d’une plus grande sédentarité et d’une alimentation trop riche et déséquilibrée. À l’obésité s’ajoutent d’autres facteurs de risque tels que l’ethnicité, les antécédents familiaux de diabète de type 2, l’âge pubertaire, le syndrome métabolique, l’acanthosis nigricans, le syndrome des ovaires polykystiques. Ces facteurs de risque ont une caractéristique commune : l’insulinorésistance. Maximale à la puberté, elle joue un rôle majeur dans l’intolérance glucidique qui précède le diabète de type 2. Le traitement consiste en l’application de mesures hygiéno-diététiques. Si ces mesures s’avèrent insuffisantes, un traitement médicamenteux par metformine et insuline, éventuellement transitoire, peut être initié. Les conséquences du diabète de type 2 chez le sujet jeune sont dramatiques car la survenue des complications micro- et macrovasculaires est plus précoce. Leur traitement, jusqu’alors réservé à l’adulte, n’a été envisagé que partiellement en pédiatrie et doit faire face à de nombreux obstacles, qui ne sont pas sans retentissement sur l’espérance de vie de ces jeunes patients.
