Por otro lado, se interroga sobre algunos debates y controversias presentes en los casos estudiados ofreciendo, en su conjunto, un contenido extenso y de gran aplicación práctica.

Las hipótesis de una génesis orgánica del autismo explican solamente una mayor fragilidad en estos sujetos a nivel estadístico. Falta explicar por qué la gran mayoría de personas afectadas por cualquier síndrome orgánico al que se atribuye el autismo no son de hecho ni autistas ni psicóticos. Con la hipótesis de la organogénesis, la distinción entre autismo y otras enfermedades con rasgos autistas no tiene ya sentido. El resultado es una verdadera «epidemia» de autismo, es decir, el aumento de casos comprobados al mil por ciento en pocos años. Para afrontar tal «epidemia» se apuesta, sobre todo en Estados Unidos, por importantes inversiones en terapias de tipo educativo-comportamental. Ellos exigen que haya alguien que les dé garantías, un «guardaespaldas» (como lo ha definido un niño autista), que defienda su subjetividad, un «Otro» poco musculoso, que sea curioso y creativo frente a niños tan desarmados y al mismo tiempo tan fuertes.
