Leur élimination est un enjeu majeur nécessaire à la réussite du traitement endodontique. But de l'étude : comparer l'efficacité de 3 techniques d'activation laser Er:YAG des solutions d'irrigation : la Photon Induced Photoacoustic Streaming (PIPS) de Fotona®, la Shock Wave Enhanced Emission Photo-acoustic Streaming (SWEEPS) de Fotona® et l'effet Photoacoustique de Syneron® par l'évaluation au microscope électronique à balayage (MEB) de l'élimination des boues dentinaires au tiers apical de canaux courbes. Méthode : 36 canaux courbés à plus de 20° sont préparés à l'aide du système TF AdaptiveTM et répartis en 5 groupes. L'irrigation finale pour le groupe 1 s'effectue avec 2ml d'EDTA à 17% puis 2ml de NaOCl à 2,5% activés par l'effet Photoacoustique du laser LiteTouchTM de chez Syneron® durant 2 fois 30sec chacun. Même protocole pour le groupe 2 avec l'embout PIPS, et le groupe 3 avec l'embout SWEEPS, tous deux montés sur la pièce à main du laser LightWalker ATSTM de Fotona®. Le groupe 4, témoin positif, utilise l'EndoActivator® et le groupe 5, témoin négatif, l'irrigation conventionnelle à la seringue + aiguille 30 gauge à ouverture latérale. Les états de surface sont observés au MEB (x 2000) et les images obtenues sont scorées par 3 évaluateurs indépendants. Résultats : on note une différence significative d'élimination de la boue dentinaire au tiers apical des canaux courbes en faveur de la PIPS de Fotona® (p = 0,0343). Conclusion : l'innovation majeur de la PIPS réside dans l'absence d'insertion de l'embout dans les canaux.
