Les lactobacilles en particulier transforment le glucose en acide acétique, en éthanol et surtout en acide lactique. Un geste de prévention bucco-dentaire consiste à substituer ce glucose cariogène par des produits considérés à faible potentiel acidogène tel que le xylitol. Le xylitol est un polyol en C5 auquel ont été attribuées depuis 30 ans de nombreuses propriétés : non acidogène, anticariogène, réducteur de plaque, reminéralisant et inhibiteur de la croissance bactérienne.Nous étudions in vitro le comportement de 20 souches de lactobacilles en présence de xylitol. Nous montrons qu'en fait les lactobacilles peuvent métaboliser le xylitol comme le montre la chute de pH consécutive à la culture de ces bactéries sur un milieu riche en xylitol. Nous expliquons ce fait par une sélection de souches résistantes au xylitol chez lesquelles a eu lieu une dé-répression enzymatique. Cette adaptation est fonction de la durée de culture préalable sur un milieu normalement concentré en xylitol ou en glucose. Enfin nous obtenons 100% de réponses positives, y compris chez des souches du groupe I (homofermentaires) décrites comme incapables d'un tel métabolisme. Nous concluons que si le xylitol est un produit aujourd'hui utile dans la prévention dentaire, à terme, l'adaptation des lactobacilles pourrait le rendre inefficace.

Its consumption by humans has gone largely unremarked until recently, when clinical trials indicated possible beneficial health effects. This is best illustrated by the dramatic reduction in dental caries when xylitol is substituted for sucrose, but medical effects have also been noted. The physical, chemical, and biological properties of xylitol, its role in dentistry, medicine and dietetics are covered in detail. Dental topics include the metabolism of xylitol by oral and other microorganisms, its effects on enzymes, its facilitation of mineralization, and its association with plaque, oral glycosidases, peroxidase activity, and sugar transport through bacterial cell membrane. Medical aspects of xylitol range from the glucuronate-xylulose cycle, infusion therapy, erythrocyte metabolism and exocrine gland function to nutritional and safety considerations, and animal and human studies.

Nous conclurons en proposant des pistes en vue de l'élaboration d'un enseignement destiné aux étudiants de la Faculté d'odontologie de Lille.
