Book excerpt: Dental caries is defined as a multifactorial, transmissible, infectious oral disease caused primarily by the complex interaction of cariogenic oral flora (biofilm) with fermentable dietary carbohydrates on the tooth surface over time. At the tooth surface and sub-surface level, dental caries results from a dynamic process of attack (demineralization) and restitution (remineralization) of the tooth matter. It is essential to understand that carious lesions, or cavitations in teeth, are signs of an underlying condition, an imbalance between protective factors (i.e., those favouring remineralization) and pathologic factors (i.e., those favouring demineralization), balance tilting towards the latter. In clinical practice, it is very easy to lose sight of this fact and focus entirely on the restorative treatment of caries lesions, failing to treat the underlying cause of the disease.

This in-vitro project comprised three studies aiming to investigate the impact of tooth age on ETW susceptibility and prevention. In the first study, un-identified extracted premolars were collected and had their ages estimated using validated dental forensic methods. The premolars were examined to investigate the relationship between age and presence and severity of ETW, as well as other main dental-hard tissues conditions. ETW, dental caries, fluorosis, extrinsic staining and tooth color were evaluated using established clinical indices. In the second study, the tooth age impact on ETW susceptibility and response to preventive treatments (Sn+F, NaF, and de-ionized water control) were evaluated using representative samples from the initial study. Enamel and dentin specimens were prepared and subjected to daily erosion-treatmentremineralization cycling procedure. Surface loss (SL) was determined during and after the cycling, by optical profilometry. Similar protocol was adopted in the third study with the addition of toothbrushing abrasion to the model, in order to explore the interplay between age and toothpaste abrasivity on erosion-abrasion development. SL was measured during and after the erosion-toothbrushing-remineralization cycling. The relationships between age and the investigated variables were assessed using linear regression models. In conclusion: 1. The presence and severity of ETW, dental caries, and extrinsic staining increased with age, while of enamel fluorosis decreased. Tooth also showed to be darker with age. 2. Susceptibility of enamel and dentin to demineralization increased with age. Sn+F showed the highest anti-erosive efficacy, and was not affected by age. NaF showed lower efficacy on dentin, which increased with age. 3. Enamel and dentin SL increased with toothpaste abrasivity level. Dentin SL also increased with age. Age effect on enamel SL was observed only with low abrasive toothpaste. Age-related changes on enamel and dentin affected ETW development.

Book excerpt: Background: Toothpaste abrasivity is considered the major contributor in toothbrushing abrasive wear, while toothbrush stiffness can be considered a secondary factor that may modify the abrasivity of toothpaste. Objectives: To investigate the longitudinal enamel and dentin surface loss caused by the interaction between the abrasives in toothpaste and toothbrush filament stiffness. Study Hypothesis: The amount of enamel and dentin loss depends on the abrasivity of the toothpaste and the filament stiffness of toothbrush. Materials and Methods: The following experimental factors were considered: abrasive suspension, at two levels (L-low: Z113 and H-high: Z103); and toothbrushes at three levels determined by bristle stiffness (soft, medium, and hard) generating 6 testing groups (n = 8). Slabs of bovine enamel and dentin were cut, embedded in acrylic resin, and polished. UPVC tapes were placed on the surface of the specimens, leaving an area of 1 × 4 mm exposed in the center of the each enamel slab. Specimens (n = 8) were subjected to 5 d of erosion/abrasion cycling: erosion (5min, 4×/d, 0.3% citric acid, pH 3.75), abrasion (15 s, 2×/d, 45 strokes each, 150-g load, automated brushing machine), fluoride treatment (15 s with abrasion and 45 s without abrasion; 275 ppm F as NaF in abrasive slurry) with exposure to artificial saliva between erosion and abrasion (1h) and all other times (overnight). Surface loss (SL, in micrometers) was determined by optical profilometry, after the third and fifth days of cycling. Data were analyzed using three-way ANOVA (alpha = 0.05). For enamel, only cycling time was found to affect surface loss with 5 d > 3 d. Overall, there was little SL (mean range: 0.76 μm to 1.85 μm). For dentin (mean SL range: 1.87 μm to 5.91 μm), significantly higher SL was found for 5 d vs. 3 d, with particularly large differences for hard toothbrush high abrasive, and medium toothbrush/low abrasive. Hard toothbrush resulted in significantly higher SL than medium toothbrush for high abrasive after 5 d, with no other significant stiffness differences. High abrasive had significantly higher SL than low abrasive overall with strong effects for all combinations, except medium stiffness after 5 d. In conclusion, the interplay between abrasivity and filament stiffness appears to be more relevant for dentin than enamel.

Book excerpt: The effect of trace elements such as fluoride on multi-factorial tooth wear is poorly understood. This study examined the role of varying fluoride compounds and products in multi-factorial tooth wear models. Dedicated software for surface profile measurement of dental hard tissues using white light confocal profilometry was developed and a measurement uncertainty evaluation completed. Microhardness techniques were validated and the remineralisation protocol developed. Human teeth were donated in line with research ethics and enamel and dentine samples were prepared. A series of three distinct experiments involving varying fluoride preparations and concentrations were completed. Firstly, the effect of an aqueous sodium fluoride solution of increasing concentration on citric acid erosion and attrition of enamel and dentine was investigated. 5000 ppm and 19000 ppm sodium fluoride solutions significantly reduced enamel loss versus control after 15 cycles of erosion-attrition, however no other groups showed significant differences. Secondly, the effect of a single application of highly concentrated sodium fluoride, stannous fluoride and titanium fluoride solutions and a sodium fluoride/calcium fluoride varnish on hydrochloric acid erosion and tooth brush abrasion of enamel was investigated. The titanium fluoride solution and the sodium/calcium fluoride varnish reduced enamel loss significantly after erosion. However, in the erosion-abrasion protocol only the sodium fluoride solution and the sodium/calcium fluoride varnish showed a statistically significant reduction in enamel loss after 15 cycles. Chemical and ultra-structural analyses supported these findings. Finally, the effect of a single application of two highly concentrated sodium fluoride varnishes and a fluoride-free self-etch adhesive on citric acid erosion-attrition of enamel was investigated.
