Book excerpt: The ability to supply cellular energy in the form of adenosine triphosphate (ATP) by oxidative phosphorylation is critical to maintaining muscle function and health. While conventional endurance exercise training has proven effective at increasing the maximal capacity for oxidative phosphorylation (Vmax), it is not known whether exercise training stimulates mitochondrial ATP synthesis in resting muscle (Vrest). Recently, short-term high-intensity interval training (HIT) training has shown potent effects on Vmax . However, little is known about (1) the effects of short-term HIT on Vrest, and (2) the time course of adaptations in V rest and Vmax following short-term HIT. Healthy young males were recruited to participate in a 2-week training intervention (6 training sessions). Each session consisted of 4-6 bouts of 30-s sprints on a cycle ergometer. Phosphorus magnetic resonance spectroscopy was used to measure Vrest and Vmax in vivo in vastus lateralis: (i) prior to, (ii) 15 hours after a single session of exercise training, and (iii) after completing 6 sessions of exercise training. Two weeks of training increased peak whole-body oxygen consumption (35.8 ± 1.4 to 39.3 ± 1.6 ml·min-1 ·kg-1, p=0.01) and exercise capacity (217.0 ± 11.0 to 230.5 ± 11.7 W, p=0.04) on the cycle ergometer. Book excerpt: In the past, ‘traditional’ moderate-intensity continuous training (60-75% peak heart rate) was the type of physical activity most frequently recommended for both athletes and clinical populations (cf. American College of Sports Medicine guidelines). However, growing evidence indicates that high-intensity interval training (80-100% peak heart rate) could actually be associated with larger cardiorespiratory fitness and metabolic function benefits and, thereby, physical performance gains for athletes. Similarly, recent data in obese and hypertensive individuals indicate that various mechanisms – further improvement in endothelial function, reductions in sympathetic neural activity, or in arterial stiffness – might be involved in the larger cardiovascular protective effects associated with training at high exercise intensities. Concerning hypoxic training, similar trends have been observed from ‘traditional’ prolonged altitude sojourns (‘Live High Train High’ or ‘Live High Train Low’), which result in increased hemoglobin mass and blood carrying capacity. Recent innovative ‘Live Low Train High’ methods (‘Resistance Training in Hypoxia’ or ‘Repeated Sprint Training in Hypoxia’) have resulted in peripheral adaptations, such as hypertrophy or delay in muscle fatigue. Other interventions inducing peripheral hypoxia, such as vascular occlusion during endurance/resistance training or remote ischemic preconditioning (i.e. succession of ischemia/reperfusion episodes), have been proposed as methods for improving subsequent exercise performance or altitude tolerance (e.g. reduced severity of acute-mountain sickness symptoms). Postulated mechanisms behind these metabolic, neuro-humoral, hemodynamics, and systemic adaptations include stimulation of nitric oxide synthase, increase in anti-oxidant enzymes, and down-regulation of pro-inflammatory cytokines, although the amount of evidence is not yet significant enough. Improved O2 delivery/utilization conferred by hypoxic training interventions might also be effective in preventing and treating cardiovascular diseases, as well as contributing to improve exercise tolerance and health status of patients. For example, in obese subjects, combining exercise with hypoxic exposure enhances the negative energy balance, which further reduces weight and improves cardio-metabolic health. In hypertensive patients, the larger lowering of blood pressure through the endothelial nitric oxide synthase pathway and the associated compensatory vasodilation is taken to reflect the superiority of exercising in hypoxia compared to normoxia. A hypoxic stimulus, in addition to exercise at high vs. moderate intensity, has the potential to further ameliorate various aspects of the vascular function, as observed in healthy populations. This may have clinical implications for the reduction of cardiovascular risks. Key open questions are therefore of interest for patients suffering from chronic vascular or cellular hypoxia (e.g. work-rest or ischemia/reperfusion intermittent pattern; exercise intensity; hypoxic severity and exposure duration; type of hypoxia (normobaric vs. hypobaric); health risks; magnitude and maintenance of the benefits). Outside any potential beneficial effects of exercising in O2-deprived environments, there may also be long-term adverse consequences of chronic intermittent severe hypoxia. Sleep apnea syndrome, for instance, leads to oxidative stress and the production of reactive oxygen species, and ultimately systemic inflammation. Postulated pathophysiological changes associated with intermittent hypoxic exposure include alteration in baroreflex activity, increase in pulmonary arterial pressure and hematocrit, changes in heart structure and function, and an alteration in endothelial-dependent vasodilation in cerebral and muscular arteries. There is a need to explore the combination of exercising in hypoxia and association of hypertension, developmental defects, neuro-pathological and neuro-cognitive deficits, enhanced susceptibility to oxidative injury, and possibly increased myocardial and cerebral infarction in individuals sensitive to hypoxic stress. The aim of this Research Topic is to shed more light on the transcriptional, vascular, hemodynamics, neuro-humoral, and systemic consequences of training at high intensities under various hypoxic conditions.

Book excerpt: Considering the increasing global prevalence of overweight and obesity and their propensity for disease, this study was undertaken in an attempt to optimize exercise prescription for this at-risk group by determining if the benefits associated with interval training occur in an intensity dependent manner. 19 sedentary, overweight males (Age: 22.7 ± 3.9 yrs, BMI: 31.4 ± 2.6 kg/m2, WC: 106.5 ± 6.6 cm) performed interval training for three weeks at either 70% or 100% of their peak work rate on a cycle ergometer. Aerobic capacity measurements, time to completion trials, muscle biopsies, and fasted blood samples were all performed pre and post training. Analyses of aerobic capacity and exercise performance demonstrate greater improvements made in the 100% compared to the 70% group, while measures of skeletal muscle oxidative capacity indicate equivalent changes between groups. Taking into account the similar increases in mitochondrial content in both groups and understanding the influence of both oxygen supply and demand in determining maximal oxygen consumption, the greater increases in aerobic capacity achieved by the 100% group may be the result of enhanced cardiovascular adaptations. These findings suggest that some of the health benefits associated with interval exercise may be intensity dependent. Therefore, there may be additional benefit to exercise at higher intensities.

Book excerpt: Children and adolescents are thought to use oxidative metabolism to a greater extent than adults during high intensity exercise. The studies reported in this thesis examine the nature and implications of age-related differences in muscle metabolism during high intensity exercise and recovery. Chapter 4 concluded that during heavy intensity exercise, phosphocreatine (PCr) kinetics did not differ with age or sex, while Chapter 5 revealed that during very heavy intensity exercise, the fundamental t was slower and slow component amplitude greater in men compared with adolescent boys, indicating that exercise intensity might play a role in determining age-related differences in muscle metabolism. In Chapter 6, two bouts of very heavy intensity exercise were completed, and prior exercise reduced the PCr slow component amplitude in men but not boys. Deoxyhaemoglobin (HHb) kinetics was faster in adolescents compared with adults during both heavy and very heavy intensity exercise, indicating that matching of oxygen delivery to oxygen utilisation is less precise at the onset of exercise in adolescents compared with adults. PCr recovery from high intensity exercise was faster in boys than men, but not different in girls and women, as described in Chapter 7. The speed of PCr recovery was correlated with maturity in adolescents, but was not correlated with end-exercise [PCr] or pH. Two different tests to measure mitochondrial capacity in adolescents were evaluated in Chapter 8, and a fitted curve and gated test were both used to determine PCr recovery kinetics. Finally, in Chapter 9, age-related differences in muscle metabolism and oxygenation during fatiguing exercise were examined; a strong trend for greater fatigue in adults compared with adolescents was accompanied by greater metabolic perturbation in adults. Overall, these data show that muscle metabolism and oxygenation differs between adolescents and adults during and following very high intensity exercise.
