Book Effects of Flash Free Technique on Plaque Retention White Spot Lesions and Bracket Failure A Randomized Clinical Trial

Download or read book Effects of Flash Free Technique on Plaque Retention White Spot Lesions and Bracket Failure A Randomized Clinical Trial written by Payam Ishani Afousi and published by . This book was released on 2016 with total page 151 pages. Available in PDF, EPUB and Kindle. Book excerpt: A side effect of orthodontic treatment is the development of white spot lesions (WSLs). This thesis attempted to evaluate the effects of the use of APCTM Flash-Free (FF) and conventional adhesives in bonding of orthodontic brackets on plaque formation and retention, WSL development, and bracket failure in-vivo. This was accomplished by collecting plaque samples from around orthodontic appliances and analyzing them using ATP-driven bioluminescence; scanning enamel surfaces using the Canary SystemTM to evaluate WSL development, and counting the number of spontaneous debonding of brackets of the 4 maxillary incisors in the first 6 months of treatment. It was found that there were no significant differences in plaque formation and retention between brackets bonded with FF adhesive and those bonded with conventional adhesives. Furthermore, there were no significant differences in WSL development between FF adhesives and conventional adhesives during the same study period; however, demineralization of enamel surfaces took place in our sample with mesiofacial and distofacial surfaces being most affected. Failure rates of 5.26% and 0% were observed for FF and conventional brackets during the first 6 months of orthodontic treatment, respectively. The present study revealed that the presence or absence of excessive adhesive (flash) around fixed orthodontic appliances may not play a significant role in plaque retention and white spot lesion development in patients undergoing orthodontic treatment during the first 6 months of orthodontic treatment with fixed appliances. Furthermore, FF brackets may fail more often than conventional brackets during the first 6 months of orthodontic treatment with fixed appliances.

Book The Effect of Light Cured Fluoride Varnish to Remineralize White Spot Lesions and Change the Salivary Microbiome

Download or read book The Effect of Light Cured Fluoride Varnish to Remineralize White Spot Lesions and Change the Salivary Microbiome written by Joseph Michael Mullen and published by . This book was released on 2022 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Orthodontic treatment impedes the oral hygiene of patients, leading to plaque accumulation and the development of white spot lesions (WSL), which are visible areas of demineralized enamel. Our study aims to investigate the efficacy of different commercially available fluoride varnishes to treat these white spot lesions after the completion of orthodontic treatment and evaluate their effect on the oral microbiome. The present pilot study used a split-mouth design, with each of the eight subjects receiving different treatment on the left and right sides of their mouth. Subjects were assigned to one of three groups using stratified permuted block randomization, with each group receiving two of the following three options: placebo varnish (petroleum jelly), 5% sodium fluoride varnish (Acclean, Henry Schein), and a resin-modified glass ionomer light-cured fluoride varnish (Vanish XT Extended Contact Varnish, 3M). There are four total visits for this study: T1 (baseline taken within one month of braces removal), T2 (day 30), T3 (day 90), and T4 (day 180). Several series of photographs were taken at each visit to assess their oral health and the appearance of the WSL. Stimulated saliva was collected, and the severity of the WSL was assessed using laser fluorescence (DIAGNOdent pen, Kavo), after which the varnishes were applied according to their group allocation. Clinical photographs were assessed by a trained and blinded scorer using the Turesky-modified Quigley Hein Plaque Index (TQHPI) and the International Caries Detection and Assessment System (ICDAS). These index measurements, along with the DIAGNOdent scores, were used to assess the efficacy of the different interventions at remineralizing WSLs. Salivary loads of total bacteria, Candida albicans, and Streptococcus mutans were determined by plating saliva samples on agar plates containing Brain Heart Infusion (BHI), Sabouraud Dextrose (SAB), and Mitis Salivarius-Bacitracin (MSB) respectively. Although the small sample size of this pilot study made it difficult to achieve statistical significance, several notable trends were identified. Average TQHPI levels stayed relatively stable, with modest decreases seen in the placebo and light-cured fluoride varnish (LCFV) groups. DIAGNOdent scores decreased in all groups, with the 5% varnish group showing a significant decrease from T1-T3 and the LCFV showing the greatest percent change from T1-T4. ICDAS scores increased in the 5% and placebo groups and decreased in the LCFV group. CFUs representing total bacterial load, S. mutans, and C. albicans all decreased by an average of 60.9 - 99.9 percent, showing decreased oral colonization in the 6 months after completion of orthodontic treatment. These preliminary trends need to be verified in future studies that include more subjects to reach statistical significance.

Book Current Technology and Techniques in Re mineralization of White Spot Lesions

Download or read book Current Technology and Techniques in Re mineralization of White Spot Lesions written by Susan Podray and published by . This book was released on 2012 with total page 55 pages. Available in PDF, EPUB and Kindle. Book excerpt: White Spot lesions are a common iatrogenic occurrence on patients who are treated with fixed orthodontic appliances. There is a dynamic chemical interaction between enamel and saliva at the tooth surface that allow a lesion to have phase changes involving demineralization of enamel and remineralization. This is due to calcium and phosphate dissolved in saliva that is deposited onto the tooth surface or removed depending on the surrounding pH. Caseinphosphopeptide-amorphous calcium phosphate (CPP-ACP) is gaining popularity in dentistry as a way to increase the available level of calcium and phosphate in plaque and saliva to improve the chemical gradient so that if favors remineralization. The aim of our investigation is to search the available current literature and formulate a recommendation for use of CPP-ACP in orthodontics. Publications from the following electronic databases were searched: PubMed, Web of Science, Cochrane Library and Science Direct. Searches from August 2010 to April 1st 2012 were performed under the terms "MI Paste OR Recaldent OR caseinphosphopeptide-amorphous calcium phosphate OR CPP-ACP or tooth mousse". The searches yielded 155 articles, These were reviewed for relevance based on inclusion and exclusion criteria. Articles with inappropriate study design or no outcome measures at both baseline and end point were also excluded. 13 articles were deemed of relevance with a high quality study design and were included in this study for evaluation. The current literature suggests a preventative treatment regimen in which MI Paste Plus is used. It should be delivered once daily prior to bed after oral hygiene for 3 minutes in a fluoride tray, throughout orthodontic treatment. It should be recommended for high risk patients determined by poor oral hygiene, as seen by the inability to remove plaque from teeth and appliances. This protocol may prevent or assist in the remineralization of enamel white spot lesions during and after orthodontic treatment.

Book The Effects of a Fluoride Releasing Orthodontic Primer on Demineralization Around Brackets

Download or read book The Effects of a Fluoride Releasing Orthodontic Primer on Demineralization Around Brackets written by Daniel Ryan Pennella and published by . This book was released on 2011 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this study was to investigate the effects of a fluoride releasing orthodontic primer on demineralization adjacent to brackets. Twenty-two patients were recruited for this study. One premolar was randomly chosen as the experimental tooth, the contralateral tooth was the control. Teeth were visually analyzed for white spot lesions (WSLs). Knoop microhardness was used to determine hardness. Visual examination results showed no significant difference in the number of WSLs observed between Opal Seal and Transbond XT over the duration of this study. Solid conclusions could not be drawn from the results of microhardness testing. Therefore, it cannot be concluded that there is a difference in enamel hardness between Opal Seal and Transbond XT. However, prior to 90 days, teeth showed a significant difference in WSLs. Suggesting a protective effect of Opal Seal that diminished with time. Future studies are necessary to determine the clinical performance of this product.

Book Effect of Acidulated Phosphate Fluoride Gel on the Prevention of White Spot Lesions in Patients Undergoing Active Orthodontic Treatment

Download or read book Effect of Acidulated Phosphate Fluoride Gel on the Prevention of White Spot Lesions in Patients Undergoing Active Orthodontic Treatment written by Corneil Kimberly Hutto Fretty and published by . This book was released on 2015 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Introduction: The purpose of this study was to evaluate the effect of 1.23% acidulated phosphate fluoride (APF) gel on white spot lesion (WSL) formation utilizing a typical orthodontic treatment interval for the applications. Methods: In this double-blind, placebo controlled trial, 55 patients starting orthodontic treatment, either at Texas A&M University, Baylor College of Dentistry or at a private practice, were randomly divided into two groups (a treatment and placebo). Every eight weeks, the treatment group received an application of Nupro 1.23% APF gel (Dentsply®) for one minute, while the placebo group received a gel treatment for one minute that was exactly the same composition, but lacked fluoride. Patients were followed for 6 or 8 months depending on when their second set of brackets was placed. Pre and post treatment photographs and Fluorecam® readings were used to quantify the development of WSLs. Results: There was no statistically significant difference in the percentage of patients who developed WSLs, the number of new WSLs that developed on each tooth during treatment, or in the number of new WSLs developed per patient when using photographic analysis or Fluorecam® readings. Based on photographic assessments, 37.5% of patients in the fluoride group and 32.3% of patients in the placebo group developed new WSLs (p=0.685), while with Fluorecam assessments 37.5% of patients in the fluoride group and 58.1% of patients in the placebo group developed a new WSL (p=.130). Five patients had pre-existing WSLs (1 patient in the treatment group and 4 patients in the placebo group). Only patients in the placebo group demonstrated worsening of white spot lesions (p=.375). When using the Fluorecam® to quantify WSL development by measuring the size, intensity, and impact of each lesion, the only measurement which demonstrated a significant difference between the fluoride and placebo group was the intensity of the lower left first premolar (P

Book Comparison of Three Fluoride Dentifrice Products in the Prevention of White Spot Lesions in Orthodontic Treatment

Download or read book Comparison of Three Fluoride Dentifrice Products in the Prevention of White Spot Lesions in Orthodontic Treatment written by Anna B. Palombini and published by . This book was released on 2015 with total page 69 pages. Available in PDF, EPUB and Kindle. Book excerpt: Introduction:. During the course of orthodontic treatment, enamel decalcification is a common problem. Demineralized enamel, the precursor to caries formation, can be attributed to fixed orthodontic appliances and prolonged exposure to bacterial plaque. Progression to clinically detectable white spot lesions may occur as early as one month following the placement of orthodontic appliances. Such problems have influenced clinicians to search for a solution. Because fluoride treatment immediately upon debonding is not advocated, clinicians have proposed fluoride treatment and fluoride-releasing materials at the commencement of the therapy. The two new anti-cavity toothpastes, ClinproTM 5000 with 1.1% Sodium Fluoride and ClinproTM Tooth Créme with 0.21% Sodium Fluoride have been shown to be useful in reduction of white spot lesions. Both the Clinpro TM products (3M ESPE) contain fluoride as well as tri-calcium phosphate, which are components naturally found in saliva. The aim of this study is to determine if ClinproTM 5000, ClinproTM Tooth Créme, or MI-Paste Plus has an effect on the formation and resolution of white spot lesions for patients undergoing orthodontic treatment. Methods: Three prospective groups of 40 patients undertaking routine orthodontic treatment were evaluated (total recruitment of 120 subjects). Patients must have their permanent dentition, be 12 years or older, and have not used extensive fluoride regimes before. Patients were chosen if they appear that they would be compliant as judged by the investigator. The selected product was brushed on the teeth by the patient for two minutes, twice daily for a total of 4 months. After brushing, the patients were instructed to expectorate only, not rinse, eat or drink anything for at least 30 minutes. The subjects were observed every 4 weeks. At each visit, 3 intraoral photos were taken (frontal and buccal views) and measurements based on the Enamel Decalcification Index (EDI) will be recorded. The Enamel Decalcification Index was used to determine the number of white spot lesions and caries risk detected in these photographic records. Also, a brushing diary was reviewed. A trained member of the 3M ESPE Clinical Research group did monitor the study by means of visits to the clinic to evaluate patient charts, study data, and study photographs. Results: Altogether, the results provided strong support with regard for ClinproTM 5000 providing superior enamel protection against decalcification when compared to ClinproTM Créme, and MI Paste. Conclusions: The use of ClinproTM 5000, ClinproTM Créme, and MI paste demonstrated less formation of white spot lesions when compared to data in the literature. ClinproTM 5000 had a marginally better affect compared to the other two products.

Book Testing Amino Acid Toothpaste Against Braces Derived Plaque Two Randomized Double Blind Placebo Controlled Clinical Trials

Download or read book Testing Amino Acid Toothpaste Against Braces Derived Plaque Two Randomized Double Blind Placebo Controlled Clinical Trials written by kendrick park and published by . This book was released on 2020 with total page 50 pages. Available in PDF, EPUB and Kindle. Book excerpt: Increased plaque, calculus, and tartar build-up are some of the worst consequences of braces and orthodontic treatment. Orthodontic brackets and wires act as inadvertent traps that collect food and plaque where bacteria accumulate, and subsequently cause heavy inflammation of the gums, cause pain, and generate subsequent bone loss. Current supplemental oral hygiene aids include floss threaders, "super-floss," interproximal brushes, and most importantly reinforcing proper oral hygiene habits at every orthodontic visit. However, implementing these strategies often render ineffective due to the increased cost of additional appliances and due to patients' non-compliance at such a young age and their unwillingness to change their oral hygiene habits. Prior investigations within the last decade have recently identified arginine, an amino acid, as an active ingredient in oral dentifrices that help prevent dental sensitivity and dental decay. These recent investigations and advancements of amino acids in oral health care products have led to questions of whether they can also help in decreasing plaque that subsequently surround braces once orthodontic treatment starts. A recent study at UCLA (2015) has shown in-vitro bacterial plaque and biofilm that is specifically formed from an orthodontic bracket (Braces). Additionally, different combinations of amino acids were added to evaluate its efficacy in reducing this specific biofilm. Resultant data showed that a combination of the D-forms of arginine, alanine, and lysine proved to be the most effective of any combination tested towards the decrease of the bracket-derived plaque. This previous study in-vitro study and previous notable associations of amino acids in its positive outcomes in oral health care have led us to investigate whether amino-acids (more specifically arginine, alanine, and lysine) in toothpaste can help prevent and reduce plaque in patients with braces. This will be conducted via two randomized, double-blind clinical trials and its analyses using both objective and subjective measures.

Book Dental Composite Materials for Direct Restorations

Download or read book Dental Composite Materials for Direct Restorations written by Vesna Miletic and published by Springer. This book was released on 2017-11-25 with total page 310 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book covers both basic scientific and clinically relevant aspects of dental composite materials with a view to meeting the needs of researchers and practitioners. Following an introduction on their development, the composition of contemporary composites is analyzed. A chapter on polymerization explains the setting reactions and light sources available for light-cured composites. The quality of monomer-to-polymer conversion is a key factor for material properties. Polymerization shrinkage along with the associated stress remains among the most challenging issues regarding composite restorations. A new classification of dental composites is proposed to offer more clinically relevant ways of differentiating between commercially available materials. A review of specific types of composites provides an insight into their key issues. The potential biological issues of dental composites are reviewed in chapters on elution of leachable substances and cariogenicity of resin monomers. Clinical sections focus on material placement, finishing procedures, and the esthetics and clinical longevity of composite restorations. Bonding to tooth tissues is addressed in a separate chapter, as is the efficiency of various composite repair methods. The final chapter discusses future perspectives on dental composite materials.
