Book excerpt: The purpose of this study was to determine if prophylactic ankle tape and/or ankle braces improve dynamic stability in TTS measure. All subjects were healthy and had no prior history of ankle injuries. Data collection consisted of each subject performing a single leg jump-landing with ankle tape, ankle brace, combination of the two, and control (no tape or brace) conditions. Dynamic stability was assessed with time to stabilization force plate measure. Significant plane by ankle tape interaction (p=0.045) was found. No significant plane by ankle tape by ankle brace interaction (p=0.637), no significant ankle tape by ankle brace interaction (p=0.483), or plane by ankle brace interaction (p=0.697) were found. A notable finding was that subjects took longer to stabilize in the anterior/posterior direction than medial/lateral direction. In conclusion ankle tape, ankle brace, and the combination of ankle tape and ankle brace did not statistically improve dynamic stability in healthy ankles.

Book excerpt: It is unknown whether the application of an ankle brace can increase dynamic stability to overcome the effects of fatigue in those individuals with CAI. Ankle dynamic stability was assessed before and after a functional fatigue protocol using the Time to Stabilization (TTS) method. Prior to the functional task, dynamic stability was measured using a single leg jump landing at 50% of the subjects' maximum vertical jump height. The functional fatigue circuit was repeated until the time it took to complete the circuit increased to 50% of the baseline time. Immediately after fatigue was reached, the single leg jump landing measures were repeated. This protocol was performed with and without an ankle brace. For APTTS, the main effect for Group was statistically significant (F= 4.15; p= .05). Subjects in the CAI group (1.291"0.179s) took significantly longer to stabilize than those in the healthy group (1.191"0.10s). There were no other significant results found. The results of our study provide questions regarding the use of ankle braces for improving dynamic stability measured with TTS in both a healthy population and those with CAI. Further research, with a larger sample size should be conducted to make a better decision regarding the effectiveness of ankle braces in improving single-leg jump landing stabilization.

Book excerpt: Objective: The purpose of this study was to compare a lace-up ankle brace with a no-brace condition on the ability to return an athlete with CAI to a stable state by measuring the TTS following a jump-landing task. Design: Repeated measures within-subject design. Setting: Athletic Training Research Laboratory. Patients or Participants: Ten subjects, five males and five females (18.5625±.814yr; 174.0963±7.191cm; 69.2937±8.277kg) that fit the criteria for chronic ankle instability participated in this study. Measurements: Subjects participated in two separate testing sessions during which a jump-landing task was performed with one of two conditions: lace-up ankle brace or no ankle brace. The brace condition was randomized. During each session, five trials of a jump-landing task were performed on each leg. The jump-landing task consisted of a single-leg landing from a double-leg jump height equivalent to 50% of the subject's maximum jump height. Subjects jumped off of both feet from a distance 70cm away from the center of the force plate, reached up and touched the indicated marker, and landed on the force plate on a single test limb. The landing leg was randomized. Ground reaction forces were used to calculate TTS in the medial/lateral (MLTTS) and anterior/posterior (APTTS) directions. The independent variables were Condition (brace, no brace) and Side (injured, non-injured). A separate two-within (Condition, Side) repeated measures ANOVA was performed for APTTS and MLTTS. Main Outcome Measure(s): APTTS and MLTTS. Results: For APTTS, the Side by Condition interaction was not statistically significant (F1,9 = .004; p=.952). For MLTTS, the Side by Condition interaction was not statistically significant (F1,9 = .023; p= .884). No measurements were found to be statistically significant in either direction. Conclusion: This study was designed to show if the use of an ankle brace, like the Swede-O Universal would assist those subjects with CAI in reducing the amount of time it takes for them to stabilize after a jump landing task. Although the results of this study do not support these conclusions with statistical significance, it may provide an important direction in examining the connection between TTS, ankle bracing, and those with CAI. There is a definite need for further testing on more subjects with CAI so we can make a better conclusion about the use of the Swede-O Universal and its ability to reduce time to stabilization after a jump-landing task.

Book excerpt: Our primary purpose in this study was to compare the effects of wearing the five-toed socks with grippers alone (FSG), regular socks alone (RS), both five-toed socks and an ankle brace (FSGB), and both regular socks and an ankle brace (RSB) on dynamic postural control as measured with Time to Stabilization Resultant Vector (RVTTS) after landing from a jump in chronic ankle instability (CAI) and matched control subjects. The secondary purpose was to compare the effects of wearing FSG, RS, FSGB, and RSB on subjective measures concerning comfort, stability, and confidence in preventing the ankle from a potential injurious situation during the jump-landing procedure in CAI and matched control subjects. Forty eight subjects between the ages of 18 and 30 years old were recruited from the university community, and the subjects were separated into those with (n=24) and without unilateral CAI (n=24). RVTTS and subjective measures were recorded with jump-landing procedure. There was no significant difference in any of the outcome measures in RVTTS. The result of the subjective measurement revealed that, the regardless of the sock conditions, wearing an ankle brace improves comfort, stability, and confidence during a jump-landing procedure among CAI subjects. Future study should determine how enhanced foot gripping and plantar tactile sensation provided by the FSG plays a role in dynamic postural control among CAI subjects.

Book excerpt: BACKGROUND: Ankle sprains are the most common injuries among physically active people, with 40-70% developing functional ankle instability (FAI), characterized by frequent episodes of "giving way" and sensations of joint instability. Kinesio Taping, ® an intervention that has recently gained popularity may help decrease postural control deficits related to FAI. This study examined the immediate and prolonged effects of Kinesio Taping® on postural control in subjects with FAI and investigated differences between healthy, coper, and unstable ankles as measured through single-limb balance on a forceplate. METHODS: Sixty physically active, college-aged participants (height: 172.5 ± 9.7 cm, mass: 74.2 ± 16.2 kg, age: 21.5 ± 2.6 years) were sorted into three separate groups: healthy control, copers, and FAI. Subjects were stratified using the Cumberland Ankle Instability Tool (CAIT) combined with their history of ankle injury. Dependent variables were time to boundary (TTB) measures and traditional center of pressure (COP) measures in both the mediolateral (frontal) and anteroposterior (sagittal) planes. Testing was performed at baseline, immediately after application of the tape, 24 hours post-tape application, and immediately after removal of the tape. RESULTS: Significant group main effects for COP standard deviation (F2,57 = 4.309, p = 0.018) and range (F2,57 = 4.918, p = 0.011) in the sagittal plane were noted. Significant condition main effects for TTB absolute minima (frontal plane, F 3,159 = 1.607, p = 0.015) and standard deviation (frontal, F3,138 = 5.710, p = 0.002 and sagittal plane, F3,141 = 0.889, p = 0.029) were also noted. Post hoc testing revealed decreased COP standard deviation and range for the ankle instability group compared to controls and significant improvements at 24 hours post-tape in all groups for TTB absolute minima (p = 0.025) and standard deviation (frontal plane, p = 0.002). CONCLUSIONS: Deficits in COP variables were seen in unstable ankles as compared to both healthy ankles and copers. Minor improvements in TTB measures after 24 hours of continuous wear of Kinesio® tape were observed in healthy, coper, and unstable ankles. Since between group differences were seen in COP measures, but improvements post-tape application were seen only in TTB variables, we can conclude that Kinesio® tape did not improve the specific postural control deficits experienced by unstable ankles. Therefore, while Kinesio® tape may provide a small benefit to postural control at the ankle, it does not appear to be effective at improving ankle instability deficits. Book excerpt: Lateral ankle sprains are among the most common orthopedic injuries and often lead to ankle instability; a condition characterized by pain, weakness, and most problematically, recurrent ankle sprains. Ankle Instability is generally attributed to neuromuscular and proprioceptive deficits; however the pathoetiology behind the condition remains unknown. This work aimed to further understand preparatory and reactive neuromuscular control strategies in individuals with ankle instability (AI group), individuals who have suffered a lateral ankle sprain, but did not develop ankle instability (LAS group), and uninjured controls (CON group) via the use of a novel ankle supinating device, which was created to simulate the mechanism of a lateral ankle sprain during landing. We hypothesize that individuals with ankle instability would demonstrate altered preparatory and reactive neuromuscular control relative to the other two groups, which may predispose them to episodes of their ankle giving way. Contrary to our hypotheses, the AI group was not significantly different than the CON group. In this work, it was the LAS group that differed significantly in their preparatory neuromuscular control patterns from the other two groups, specifically demonstrating a significantly increased TA activation prior to landing. While contrary to traditional logic, we speculate that this is a neuromuscular control pattern in LAS subjects that allows them to control dynamic ankle stability after damage to the lateral ankle ligaments; including controlled kinematics and/or force attenuation. While the device used effectively mimicked the mechanism of a lateral ankle sprain in a safe environment, there were not statistically significant differences in reactive neuromuscular control between the groups. Future work should aim to better categorize individuals with ankle instability, as well as monitor the role of the entire lower extremity in controlling dynamic stability at the ankle joint. Furthermore, future studies should aim to evaluate individuals who have suffered a lateral ankle sprain, but did not develop ankle instability. These individuals may hold the key to understanding neuromuscular control strategies in ankle instability, which could lead to the development of more appropriate treatment and rehabilitation paradigms to reduce the incidence of ankle instability.
