A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. This report will be of interest to local and national policymakers, school officials, teachers, and the education community, researchers, professional organizations, and parents interested in physical activity, physical education, and health for school-aged children and adolescents.

Book excerpt: Exercise training is known to decrease fat mass and increase lean mass in adolescents and adults. In adults, the consumption of dairy foods, as part of a lifestyle intervention, can promote favourable body composition changes. However, results of the few studies examining the combined effects of exercise and dairy consumption on body composition in adolescents are inconclusive. The purpose of our study was to determine whether increased dairy consumption, along with structured exercise training and dietary guidance within a weight management program, can promote favourable changes in body composition, anthropometry and cardiovascular fitness in overweight/obese adolescent females. Sixty-one adolescent females (age: 14.8±2.2 y; BMI: 29.3±5.1 kg/m2) were randomized to 3 groups: recommended dairy (RDa; n=24); low dairy (LDa; n=22); control (GCon; n=8), and 54 participants completed the study. The RDa and LDa groups participated in a 12-week, individualized, eucaloric, lifestyle modification intervention consisting of mixed-mode exercise 3x/week, and 5 nutritional counselling sessions with a registered dietitian. RDa group was provided 4 servings/day of dairy (as milk, Greek yogurt and cheese), while LDa maintained habitually low intakes of 0-2 servings/day. Seven-day food records were collected at weeks 0 and 12. Body composition, waist/hip circumference, and VO2peak were assessed for all groups at weeks 0 and 12. Both RDa and LDa decreased body fat (-1.7±1.5%, -1.2±1.1%, respectively), fat mass (-1.3±2.1kg, -1.1±2.0kg, respectively) and subcutaneous fat thickness (-12.5±10.0mm, -9.1±9.4mm, respectively) compared with the GCon (0.3±1.3%, 0.8±1.8kg, 3.0±8.0mm, respectively) (p ≤0.002 for all). RDa decreased fat mass (kg) more than LDa (p=0.001). Both RDa and LDa gained more lean mass (1.5±1.9kg, 0.7±1.6kg, respectively) than GCon (0.5±1.4kg). RDa increased lean mass more than LDa (p≤0.001). VO2peak (ml/min/kg) did not differ between groups following the intervention (p=0.093). These findings suggest that the inclusion of a variety of dairy foods in the diet of overweight/obese adolescent girls, as part of a weight management intervention program, is beneficial to the overall improvement in body composition.

Book excerpt: The increasing incidence of obesity and nutrition related chronic diseases is becoming a public health concern. It is important for health professionals to understand how they can best educate the public in order to prevent this epidemic from worsening. The aim of this study was to evaluate whether an interactive nutrition education program is effective in prompting dietary behavior changes in adolescent females. The subjects of this study were 9 Caucasian adolescent females from Reading, Pennsylvania. The mean age was 11.6 years old. The subjects were interviewed to obtain twenty-four hour recalls to use as baseline data and as a nutrition needs assessment. They were also asked a set of general nutrition knowledge questions. An interactive nutrition education program was designed to meet the nutrition concerns of the particular audience. The initial twenty-four hour recalls showed the average adolescent participant's diet was deficient in milk, fruits, vegetables, and complex carbohydrates. Subjects participated in a nutrition education intervention that consisted of learning about nutrition, planning snacks, and goal setting. The intervention was designed to facilitate goal setting and behavior changes that would increase the consumption of the deficient food groups. Twenty-four hour food recall data was collected after the nutrition education program and compared to the initial data to determine whether the students implemented behavior changes that would affect their nutritional intake. After the nutrition education program, the average number of grain and meat servings increased. The average number of fats, oils, sweets, and dairy products decreased. There was a slight decrease in the average number of servings from the fruit group and a slight increase the average number of vegetable servings. The nutrition intervention resulted in 8 out of the 9 subjects creating and remembering their individual nutrition intake goaL Five of the students met their individualized nutrition goals on the day the food recall data was collected. Nutrition knowledge did not change due to the intervention. The majority of subjects were able to name the food groups and state the recommended number of servings for the food groups before and after the intervention. The difference in number of servings from the food groups before and after the intervention was slight; therefore, it was difficult to relate the change in intake to the effectiveness of the education session. This intervention was effective in motivating students to think about nutrition goals. The majority of students actually implemented their individual goals. An expansion of the sample size for this study would help to improve the analysis of individualized goals. A larger sample of goals would enable the nutrition goals to be categorized and evaluated based on the food group or food choices they influence.

Book excerpt: In the past few decades obesity rates in the US have reached epidemic proportions in both adults and children. Nearly one-third of American adults are obese, and since 1980, the obesity rates for children 6-11 years and 12-19 have nearly tripled (Centers for Disease Control and Prevention, 2012). Obesity in adults and children is associated with abnormal lipid profiles, insulin resistance and low cardiorespiratory endurance (CRE). The link between disease risk and low CRE has been well established in adults and to a lesser extent in children and adolescents. The PURPOSE of this study was to evaluate the effect of a mentor-led vigorous aerobic exercise program on insulin resistance, body composition, and peak VO2 in overweight and obese adolescents. METHODS Subjects (n=23) for the study were randomly assigned to two groups. Eleven participants were assigned exercise mentors and exercised approximately three times per week for 45 to 60 minutes each session. An additional 12 adolescents were randomly assigned to a control group. Body composition, Peak VO2, and HOMA-IR levels were assessed at pre-, post-, and follow-up testing. A 2 (control vs. experimental) x 3 (pre-, post-, follow-up) repeated measures analyzes of variance (ANOVA) was used to analyze the data using the MANOVA procedure in JMP®. RESULTS No significant intervention effect was observed for body composition measures. Significant effects were observed in Peak VO2max (p = 0.0066) and total treadmill time (p = 0.0116). In the experimental group, effect size, measured by Cohen's d, showed a high effect size (d = 0.88) for Peak VO2max from pre to post test and a moderate decrease in effect size (d = -0.44) from post to follow-up. Similar findings were shown with total treadmill time with a small positive effect size from pre to post (d = 0.29) and a small decrease from post to follow-up (d = -0.33). No significant between group differences were found in HOMA-IR. However, the intervention group's HOMA-IR, improved between baseline and post testing period. The current findings show that independent of weight loss and change in body composition, positive changes in fitness were significantly different from the control group, and the fitness improvements were maintained throughout the duration of the study. Insulin resistance improved for the experimental group from baseline to post testing and was maintained over the 12 week non-intervention follow-up period.CONCLUSION A mentor-led exercise intervention does have potential to be effective at increasing health outcomes independent of weight loss, which can be sustainable after the cessation of the program.
