In early 2004, when Dr. Michael DeVita informed me that he was cons- ering a textbook on the new concept of Medical Emergency Teams (METs), I was surprised. At Presbyterian-University Hospital in Pittsburgh we int- duced this idea some 15 years ago, but did not think it was revolutionary enough to publish. This, even though, our fellows in critical care medicine training were all involved and informed about the importance of “C- dition C (Crisis),” as it was called to distinguish it from “Condition A (Arrest). ”We thought it absurd to intervene only after cardiac arrest had occurred,because most cases showed prior deterioration and cardiac arrest could be prevented with rapid team work to correct precluding problems. The above thoughts were logical in Pittsburgh, where the legendary Dr. Peter Safar had been working since the late 1950s on improving current resuscitation techniques, ?rst ventilation victims of apneic from drowning, treatment of smoke inhalation, and so on. This was followed by external cardiac compression upon demonstration of its ef?ciency in cases of unexpected sudden cardiac arrest. Dr. Safar devoted his entire professional life to improvement of cardiopulmonary resuscitation. He and many others emphasized the importance of getting the CPR team to o- of-hospital victims of cardiac arrest as quickly as possible.

Avoidable errors cause as many as 98,000 hospital deaths annually. Failure of clinical providers to respond and take appropriate action in a timely manner in patients hospitalized on medical units is a clinical concern that can ultimately lead to respiratory and cardiac arrest. Rapid Response Teams have been implemented to avoid this failure to rescue. Studies indicate that early recognition and team design with a provider is inversely correlated with reduced cardiac arrest and mortality rates. Aim: To reduce the number of unplanned medical / surgical inpatient transfers to higher level of care areas through early recognition of patient deterioration utilizing a standardized early warning scoring tool. Setting: Six inpatient medical and surgical units in moderate sized, Magnet designated, community care medical center in the Southwestern United States, with an existing but underutilized process of establishing early patient risk for clinical decline associated with the Rapid Response Team. Participants: Nursing staff on the project participants unit were the focus of the quality improvement practice change. Methods: An evidence-based approach was used to develop a structured process designed to improve the effectiveness of an inter-professional Rapid Response Team and to increase the use of a trigger-driven system to mandate calls based on physiologic criteria. Since the events of September 11, 2001, the United States has invested billions of dollars and immeasurable amounts of human capital to develop and enhance public health emergency preparedness and infrastructure to respond to a wide range of public health threats, including infectious diseases, natural disasters, and chemical, biological, radiological, and nuclear events. Despite the investments in research and the growing body of empirical literature on a range of preparedness and response capabilities and functions, there has been no national-level, comprehensive review and grading of evidence for public health emergency preparedness and response practices comparable to those utilized in medicine and other public health fields. Evidence-Based Practice for Public Health Emergency Preparedness and Response reviews the state of the evidence on PHEPR practices and the improvements necessary to move the field forward and to strengthen the PHEPR system. This publication evaluates PHEPR evidence to understand the balance of benefits and harms of PHEPR practices, with a focus on four main areas of PHEPR: engagement with and training of community-based partners to improve the outcomes of at-risk populations after public health emergencies; activation of a public health emergency operations center; communication of public health alerts and guidance to technical audiences during a public health emergency; and implementation of quarantine to reduce the spread of contagious illness.
