Book Effectiveness and Complications of Sedation Regimens Used for Pediatric Dental Patients

Download or read book Effectiveness and Complications of Sedation Regimens Used for Pediatric Dental Patients written by Rachel Callie Gentz and published by . This book was released on 2015 with total page 73 pages. Available in PDF, EPUB and Kindle. Book excerpt: Data was available for a total of 701 sedation appointments. Practitioner rating of effectiveness for all sedation regimens combined is as follows: Effective (54.11%), somewhat effective (28.89%), ineffective (16.57%), and overly sedated (0.44%). Oral midazolam was the regimen most often rated as either effective or somewhat effective (88.8%). Most components of the medical history and physical examination were not significantly associated with sedation outcome or complications. However, increased weight of the patient and obesity both were significantly associated with lower dosages given of both oral and nasal midazolam. This study showed that practitioners rate the majority of oral and nasal sedation appointments as effective for the most common sedation regimens used at NCH. This suggests that conscious sedation is a useful clinical adjunct for patients who require advanced behavior guidance techniques.

Book Assessing Sedation Using Patient centered Outcomes

Download or read book Assessing Sedation Using Patient centered Outcomes written by Kristin D. Sweeney and published by . This book was released on 2022 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this project was to evaluate pediatric dental sedation success using patient-centered outcomes for behavior, safety, and treatment. A cross-sectional retrospective cohort study used electronic health record data of children ages 24-60 months undergoing first-time sedations for dental treatment (2015-2020) at Nationwide Children’s Hospital, Columbus, OH. Outcomes were measured: behavior success was overall-visit Frankl score 3-4; safety success was maintenance of vital signs within 2 standard deviations of age- and gender-based norms; treatment success was completion of planned dental treatment. Overall success was defined as success in 2 of the 3 measures. Descriptive statistics and bivariate analyses compared medication regimens. Of 824 subjects (52.3% female, median age 36 months), behavior success was 49.0%, safety success was 79.5%, and treatment success was 62.1%. Overall success was 69.3%. Number of medications (excluding nitrous oxide) did not affect overall success (1 medication: 68.5%; ≥2 medications (e.g. midazolam+hydroxyzine): 75.6%, P=0.17). Subjects receiving intranasal midazolam (n=543) had lower overall success (66.5% versus 74.1%, respectively, P=0.05) and treatment success rates (63.2% versus 58.2%, P=0.23) than oral (n=189), although differences were not significant. However, behavior success and safety success were significantly different for intranasal versus oral administration (44.6% versus 59.8%, P

Book Pediatric Procedural Sedation Outcomes Using Midazolam and Hydroxyzine with and Without Meperidine

Download or read book Pediatric Procedural Sedation Outcomes Using Midazolam and Hydroxyzine with and Without Meperidine written by Kathie H. Wang and published by . This book was released on 2020 with total page 24 pages. Available in PDF, EPUB and Kindle. Book excerpt: Background: Procedural sedation is an advanced pharmacological behavior guidance technique frequently used in the pediatric dental setting. It can be offered as an option for dental treatment for a young, potentially uncooperative pediatric patient to safely and effectively complete dental restorative needs. However, there are no standard regimens or drug dosages used among pediatric dentists, and sedation outcomes vary widely. Purpose: The primary objective of this randomized trial was to assess the effects of oral sedation using midazolam and hydroxyzine with and without meperidine on sedation outcomes in pediatric dental patients. The relationship between child temperament and sedation outcomes was also assessed. Methods: This pilot study recruited 14 children between the ages of 3-7 who met study eligibility criteria and were planned to undergo dental treatment with oral sedation at the University of Washington Center for Pediatric Dentistry. The children were randomly assigned to receive a regimen of midazolam and hydroxyzine with or without meperidine. Parents completed the Child Behavior Questionnaire Short Form (CBQ-SF) to assess temperament. Results: Out of 14 participants, there were no significant differences in sedation outcome in regard to age, sex, insurance status, sedation regimen, isolation method, treatment complexity, and procedure. There were also no significant differences in the effect of child temperament on overall sedation outcome, although within the non-narcotic regimen children with sedation failures displayed higher levels of both effortful control and extraversion/surgency. Conclusion: Further data collection is needed to assess the effect of procedure variables and child temperament on sedation outcomes. Conclusions regarding these relationships cannot be drawn at this time due to the small sample size. However, results from this study will contribute to identification of best practices when choosing a sedation regimen and improve our ability to identify optimal candidates for pediatric procedural sedation.

Book A Comparison of Moderate Oral Sedation Drug Regimens for Pediatric Dental Treatment

Download or read book A Comparison of Moderate Oral Sedation Drug Regimens for Pediatric Dental Treatment written by Ojas A. Parikh and published by . This book was released on 2017 with total page 33 pages. Available in PDF, EPUB and Kindle. Book excerpt: Purpose: Compare moderate oral sedation of pediatric patients using Hydroxyzine and Meperidine with either Diazepam or Midazolam in management of pediatric dental patients. Methods: Randomized, double-blind, crossover pilot study of patients 3 to 7 years of age requiring two sedation visits. Frankl and Houpt behavior scores recorded at injection time, initiation of treatment and 100% oxygen at end of treatment. Postoperative phone call surveys conducted within eight hours and within 24 hours of discharge. Wilcoxon Signed-Rank tests, Fisher's Exact Chi-squared test and 0.10 significance level. Results: 25 subjects completed 35 sedations. Eight participants completed both treatments and demonstrated significantly higher total Houpt Scores with Diazepam at all treatment stages. Frankl scores favored Diazepam at injection time. More abnormal behavior was found with Midazolam, less memory of the visit with Diazepam, but longer sleep time with Diazepam. Conclusions: Sedation with the Hydroxyzine, Meperidine and Diazepam regimen may allow for a better overall sedation experience. Postoperative monitoring is essential. The results are promising and demonstrate the value of a larger study on sedation with Diazepam.

Book Comparison of Triple Combination Oral Sedation Regimens for Pediatric Dental Treatment

Download or read book Comparison of Triple Combination Oral Sedation Regimens for Pediatric Dental Treatment written by Brett Henderson and published by . This book was released on 2019 with total page 30 pages. Available in PDF, EPUB and Kindle. Book excerpt: Purpose: Compare the efficacy of two benzodiazepines (diazepam or midazolam) in combination with meperidine and hydroxyzine for pediatric dental sedation. Methods: A randomized, double blind observation study of behaviors and outcomes related to two sedation groups. Frankl and Houpt behavior scores were recorded at three time points: injection time, initiation of treatment and at the end of treatment. Postoperative phone call surveys were conducted within eight hours of discharge to assess sleep, activity, and behavior. Results: A total of 40 sedation subjects were included in the study, of which 20 were treated with diazepam triple Combination (Di+M+H) and 20 with midazolam triple regime (Mi+M+H). Treatment was successful for 45% of cases with midazolam and 70% with diazepam (P value=.20). Houpt sleep scores were significantly higher for diazepam than midazolam at injection (P-value=.0043) and during treatment (P-value=.0152). Although Frankl scores, Houpt move and Houpt cry scores tended to favor diazepam, none were statistically significantly different. More abnormal behavior was reported with midazolam, though not statistically significant (35% vs 6%, P-value=.0854). Postoperative sleep time was longer for midazolam, but not significantly different (median sleep time: 61 vs 45 minutes, P-value=.2071). Conclusion: The diazepam, meperidine, hydroxyzine triple combination sedation regimen shows promising results as a successful alternative to midazolam triple combination. Longer postoperative monitoring may be required with diazepam, but this study has shown postoperative sleep times to be less than previously reported. Larger sample size is needed to determine if the current trend will be maintained.
