Abstract: Objective: To systematically review the effect of dietary isoflavones or soy protein on the blood lipid levels, namely total cholesterol (TC), low-density lipoprotein (LDL-C), high-density lipoprotein (HDL-C) and triglycerides (TG) in healthy postmenopausal women. Methods: A literature search was conducted through PubMed and MEDLINE database to locate studies in English on randomized controlled trials with isoflavones or soy diet interventions on healthy postmenopausal women. Quality assessment criteria were based on the CONSORT statement. Result: 15 studies with a total of 2008 healthy female participants were included in this systematic review. Interventions of soy diets and isoflavones resulted in inconsistent effects on blood lipid levels in postmenopausal women. Only 6 studies showed significant changes in lowering levels of TC, LDL-C or TG, or increasing levels of HDL-C. Conclusion: Overall, the clinical benefits of isoflavones and soy diets alone on blood lipid levels are inconclusive. Further systematic reviews and meta-analyses of randomized controlled trials are needed to evaluate whether isoflavones or soy diet are effective for the prevention of cardiovascular disease among postmenopausal women. Book excerpt: Evidence indicates that oxidative stress increases and oxidative defense status decreases after menopause, contributing to the rise in cardiovascular disease (CVD). Soy protein consumption is increasing due to its purported protective effects on disease. Isoflavones from soybeans have been shown to lower circulating lipids and lipoproteins and may also possess antioxidant properties. Phytate, another component of soy protein, may also offer health benefits, mainly due to its antioxidant properties. The purpose of this study was to determine whether soy protein components, isoflavones and phytate, would reduce the risk of CVD in postmenopausal women. In a double-blind study, 55 postmenopausal women were randomly assigned to 1 of 4 soy protein isolate (SPI) treatments: low phytate, low isoflavone (LP/LI); normal phytate, low isoflavone (NP/LI); low phytate, normal isoflavone (LP/NI); or normal phytate, normal isoflavone (NP/NI). Subjects consumed 40 g soy protein per day for 6 weeks. Blood lipids (total cholesterol [TC], low-density lipoprotein cholesterol [LDL-C], high-density lipoprotein cholesterol [HDL-C], triacylglycerol [TG]) and oxidative stress indices (protein carbonyls [PC], oxidized low-density lipoprotein [oxLDL], and 8-iso-prostglandin-F2[subscript [alpha]] [PGF2[subscript [alpha]]]) were measured at baseline and week 6. Although the normal-isoflavone treatments showed a marginal effect on oxLDL, there was no overall significant effect on oxidative stress indicators. Book excerpt: Dietary changes are part of strategies used in the management of cholesterol, and can be of particular interest in postmenopausal women at higher risk of developing CVD. Milk fat naturally contains polar lipids rich in sphingomyelin. Convincing pre-clinical evidence suggested the potential health benefits of milk polar lipids (MPL), especially on cholesterol metabolism. In humans, however, MPL effects have been rather inconsistent and the underlying mechanisms remain poorly described. We hypothesized that isolipidic enrichment of the diet with MPL may improve the cardiovascular risk factors of postmenopausal women and MPL effects may be mediated in part by reduced intestinal cholesterol absorption. In a double-blind randomized controlled trial, 58 postmenopausal women were subjected to a 4-week intervention with daily consumption of a cream-cheese containing 0g (control, n=19), 3g (n=19) or 5g (n=20) of MPL. Blood and fecal samples were collected before and after each intervention. Fasting blood lipids and associated cardiometabolic markers were measured in all women and fecal lipids including coprostanol (a gut-derived metabolite of cholesterol) were analysed in subgroups (n=7-9 per group). A proof-of-concept mechanistic crossover study was also carried out in 4 ileostomy subjects who performed 8-h postprandial metabolic explorations after consuming 0g-, 3g- or 5g-MPL enriched cheese labelled with 2H-cholesterol. Tracer enrichment was measured in plasma and chylomicrons by GC-IRMS. Cholesterol and sphingomyelin molecular species were measured in the 8-h ileal efflux by ESI-MS-MS.
