Book excerpt: Chronic and severe maltreatment of children in the form of abuse and neglect can provoke a host of neuro/psycho/emotional/physiological problems that can manifest in developmental delays, learning disorders including attention deficit hyperactivity disorder (ADHD), behavioral disorders such as conduct disorder, depression, and posttraumatic stress disorder (PTSD). Due to the multidimensional landscape of PTSD, together with the differing treatment needs of the various stages of the disorder, the ages of victims and considering the non-verbal and symbolic language of children, experts agree that research is needed to determine alternative treatment interventions best suited for victims of severe maltreatment. Equine facilitated psychotherapy (EFP) is one example of a comprehensive, child-friendly, experiential treatment for PTSD. The EFP model employed in this study incorporated a mind-body-emotion-spirit approach that invited the creation of metaphor to mirror the client's life. To test the efficacy of using EFP with traumatized youth, a quasi-experimental study was conducted using a purposive convenience sample of eleven youth ages 10-18 who presented with PTSD symptomotology. Participants received eight 1.5-2 hour EFP sessions. The impact of the intervention was examined by the Children's Revised Impact of Events Scale -13 (CRIES-13, a standardized measure of PTSD in children). The strength of the human-animal bonding between equines and participants was measured by the Human-Animal Bond Scale (HABS). Data analysis conducted via nonparametric sign tests, using weighted measures (that later measures were weighted more heavily, theorizing that the clinical improvements would be increasing and cumulative over time) indicated identical significant results both for the CRIES-13 (9 positive pairs, 2 negative pairs, p = 0.033) and the HABS (9 positive pairs, 2 negative pairs, p = 0.033). Interestingly, there was no correlation between the CRIES-13 and HABS instruments (Spearman's Rho = -0.228, p =.25), possibly implying that the EFP treatment effects are multi modal, working in multiple directions at the same time. Significant results in both measures suggest that EFP may be a viable psychotherapy for traumatized youth suffering PTSD symptomotology. However, given a limited sample size and the absence of a control group, further investigation is warranted.

Book excerpt: Posttraumatic stress disorder (PTSD) is a major concern for military veterans in the United States and current research on women's experiences in the military and the effects of their postmilitary PTSD are limited. Currently, there are a variety of evidence-based and alternative treatment options for veterans with PTSD, and one alternative treatment option includes equine-facilitated psychotherapy (EFP). EFP is a form of animal-assisted therapy (AAT) that integrates the human–animal bond as part of treatment and recovery. The purpose of this exploratory study was to explore in-depth experiences of the horse–human relationship in EFP with female veterans to elucidate how the horse–human relationship may assist in reducing impacts of PTSD. The research question was, "What is the lived experience of EFP in female veterans with PTSD?" The research was designed as an exploratory qualitative inquiry study that utilized semi-structured interviews with 10 female veterans. The findings from this study included six theme groupings with a total of thirty-three themes found. The most significant themes common to five or more participants included: Emotional discomfort with the horse, Spiritual experience with the horse, Experience of safety with the horse, Visual contact with the horse, Emotional connection with the horse, Beneficial impact of therapy, Increased connection with others, Reduced anxiety, Reduced withdrawal, EFP advocacy for others, and Delayed awareness of PTSD. Through exploring the nature of the human–animal bond in EFP, this research revealed invaluable insights into the potential healing power of horses and alternative mental health treatment options and served to honor women's experiences in the military. In addition, this qualitative study provided an opportunity to gather rich detail and general information as to the effects of EFP for sufferers of PTSD.

Book excerpt: The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition [DSM-IV; American Psychiatric Association (APA), 1994] alludes to the importance of cultural influence in establishing diagnoses. Nevertheless, culture has received little attention in studies of posttraumatic stress symptomatology (PTSS) in maltreated children. Additionally, some studies have shown that sexually abused (SA) children may be at greater risk for developing PTSS than victims of physical abuse (PA) or combined abuse (PA and SA). However, interpretations are complicated by the frequent overrepresentation of girls to boys in SA groups, small sample sizes, and inconsistent maltreatment classification methods. To address these issues, this study aimed to establish concurrent validity for the CBCL-PTSD scale and its youth self-report version (YSR-PTSD), examine the unique effects of SA on PTSS, and determine the effects of gender by type of maltreatment on PTSS across ethnicity. Hypotheses: (1) Hispanics would evidence greater PTSS than non-Hispanics; (2) Girls would evidence greater PTSS than boys; (3) SA Hispanic girls would evidence greater PTSS than children of other ethnic groups; and (4) SA would contribute significantly to PTSS (controlling for age, gender, and foster care placement). Preliminary Findings: Due to low representation, SA boys were dropped from the sample, therefore, Hypothesis 2 could not be tested. Study 1: Archival data were analyzed for 175 girls from an outpatient mental health clinic. Additionally, it is ascertained that complete healing can only be achieved if a multi faceted and all inclusive approach that would effectively heal the childrens mind, body and soul is assumed. Particularly, the psychological needs of the children that are often ignored need to be addressed. Finally the conclusions and recommendations call for more research and intrinsic focus by major stakeholders, respective administrators and higher institutions of learning to design a more effective approach of ensuring that the overall rights of the children are accorded the required attention.

Available in PDF, EPUB and Kindle. Book excerpt: Equine-facilitated psychotherapy (EFP) is widely used as an alternative or adjunctive treatment for individuals with mental health difficulties. However, research supporting the efficacy of EFP for mental health outcomes lacks both quantity and rigor. Therefore, we conducted meta and content analyses to systematically examine the relationships between EFP and mental health outcomes and to assess the features of EFP intervention studies. We included quantitative-based EFP intervention studies dated between 2006-2017 focusing on neurotypical participants with DSM-5 recognized psychological symptoms. The meta-analysis (Study 1) was based on 27 study samples and 984 participants. The results indicated a positive relationship between the use of EFP and mental health outcomes, with a moderate effect size of .27. Based on the subsequent moderator analysis, three study characteristics were found to moderate the effectiveness of EFP: age, target outcome, and program duration. Specifically, interventions targeted at mood disorders (e.g., anxiety and depression) or children/adolescents produced stronger EFP effect than other modalities or populations. Considering the cost of interventions, brief, standardized, and unmounted interventions were also preferable as both produced similar effects to longer and mounted interventions. The content analysis (Study 2) was based on 33 study samples and 1,197 participants. We systematically examined the features of EFP intervention studies in terms of reporting standards, research design, and program design and delivery to identify sound practices and to detect areas where improvements can be made in order to benefit future research and practices. Noteworthy strengths as well as features that warrant growth in terms of reporting, program procedure, and research design are discussed.

Previous scholarship has demonstrated that animals can provide therapeutic benefits for trauma victims, both as companions and in various psychotherapies. Recent research suggests that the addition of equines to the psychotherapy process may be beneficial for those who have experienced repeated traumatic stress. Trauma-focused Equine-Assisted Psychotherapy (TF-EAP) focuses on using horses to help clients consistently regulate their own physiology and psychology. The current study examines the effects of six months of sessions of TF-EAP on children and adolescents that had experienced severe abuse of neglect. Child and Adolescent Functional Assessment Scale scores were analyzed for 15 youths (mean age = 11.1 years) at the start of TF-Eap sessions and again after 6 months of treatment.

Psychological problems treated include emotional and behavioral problems, attachment issues, trauma, and developmental disorders. An influential 1970s survey suggests that over 20 percent of therapists in the psychotherapy division of the American Psychological Association incorporated animals into their treatment in some fashion. Anecdotal evidence suggests that the number is much higher today. Since Yeshiva University psychologist Boris Levinson popularized the involvement of animals in psychotherapy in the 1960s, Israel has come to be perhaps the most advanced country in the world in the area of animal-assisted psychotherapy (AAP). This is true especially in the areas of academic training programs, theory-building, and clinical practice. Great effort has been put into understanding the mechanisms behind AAP, as well as into developing ethical guidelines that take into account the therapist's responsibility toward both client and animal. This book exposes the world to the theory and practice of AAP as conceived and used in Israel. It emphasizes evidence-based and clinically sound applications with psychotherapeutic goals, as differentiated from other animal-assisted interventions, such as AAE (animal-assisted education) and AAA (animal-assisted activities), which may have education or skills-oriented goals. Not just anyone with a dog can call him-or herself an animal-assisted therapist. This volume demonstrates not only the promise of animal-assisted psychotherapeutic approaches, but also some of the challenges the field still needs to overcome to gain widespread legitimacy.

Book excerpt: Equine Assisted Psychotherapy (EAP) is a non-traditional form of psychotherapy that addresses treatment goals with the use of horses. This small pilot study consisting of 5 veterans examines the effectiveness of EAP in the treatment of veterans with posttraumatic stress disorder (PTSD). The Equine Assisted Growth and Learning Association's model of EAP was used. With the vast majority of available literature being qualitative in nature and consisting largely of participant anecdotes, this study has aimed to provide a more controlled, quantitative approach. Results from this small sample study tentatively suggest that EAP was an effective form of treatment for veterans with PTSD. Results indicated that 2 out of 5 participants experienced a statistically significant reduction in PTSD symptoms and 3 out of 5 participants no longer met DSM-IV diagnostic criteria for PTSD by follow-up. All participants demonstrated a statistically significant increase in positive affect and 4 out of 5 participants demonstrated decrease in negative affect following each session. Clear evidence of treatment effects in social and interpersonal functioning was not found with only minor benefit reported for improvement in social functioning for 2 of 5 veterans and minor improvement reported in only 1 out of 5 veterans in interpersonal functioning. These data were acquired using the Posttraumatic Stress Disorder Checklist - Civilian version (PCL - C), The Patient-Reported Outcomes Measurement Information System (PROMIS) Social Health measures, Outcome Questionnaire 45 - Interpersonal Roles subscale, and a visual analog adaptation of the Positive and Negative Affect scale (VPANAS). Recommendations for future research are also discussed including a need for a more comprehensive theoretical understanding of how change occurs in EAP and stronger study designs. Book excerpt: Approximately 402,378 of children in the United States received foster care services, and over 1 million received in-home services in 2013 (U.S. Department of Health and Human Services, Administration for Children and Families, Administration on Children, Youth and Families, Children's Bureau, 2015). All of these children are considered child welfare involved, and adolescents are a sub-group of this population at increased vulnerability. Youth experience multiple adversities prior to entering the foster care system, challenging experiences while in the foster care system, and difficulties related to aging out of care (Miller, 2009; Stott, 2013). Building upon developmental psychopathology and resiliency theory, this study utilized structural equation modeling (SEM) to analyze relations among risk and protective factors in predicting outcomes for adolescents involved in the child welfare system. It was hypothesized that post-traumatic stress and protective factors would mediate the effects of trauma and foster care involvement on adolescents' scores for school achievement and independent living skills. Using a large national survey of child welfare involved youth, a sample of 818 adolescents between the ages of 12-16 years old at baseline was assessed. Results of this study were consistent with resiliency research highlighting the influence of protective factors (e.g. school effort and engagement; closeness, positive relationship, spends time with and talking about school with caregiver) on adolescents' school achievement and independent living skills. The presence of protective factors significantly directly impacted adolescent outcomes and mediated the effect of post-traumatic stress symptoms on the outcome variables. These results have significant implications for research and practice with adolescents involved in child welfare.
