Cette pathologie, désormais chronique, est traitée par des thérapeutiques parfois complexes et associées à des effets indésirables perçus comme invalidants. Or, une observance optimale est seul garant du succès immunovirologique. Mais cette variable est dynamique et fluctue au cours du temps, en fonction des événements surgissant dans la vie du patient. Afin de prévenir et résoudre toute rupture d'observance, des consultations d'éducation thérapeutique ont été mises en place, depuis octobre 2008, au sein du service de Maladies Infectieuses et Tropicales. Une interface dynamique existe au sein d'une équipe multidisciplinaire d'éducation thérapeutique regroupant 2 médecins, 3 infirmières et 2 pharmaciens. Les consultations ont lieu deux fois par semaine. La prise en charge est différente en cas de mise en place d'un traitement ARV ou suite à un échec thérapeutique. Dans un premier temps, l'équipe d'éducation thérapeutique établit un diagnostic éducatif à l'aide de deux méthodes complémentaires (méthode MOTHIV et IPCEM) puis détermine avec le patient des objectifs éducatifs adaptés. Au cours des entretiens, différents supports sont utilisés. Certains ont été développés par l'industrie pharmaceutique et d'autres créés par l'équipe d'ETP comme un livret patient ou des planches auto-collantes de photos des spécialités anti-rétrovirales existant sur le marché... Nous avons évalué notre programme d'éducation thérapeutique en envisageant aussi bien l'aspect quantitatif (charge virale VIH, taux de lymphocytes CD4 des patients suivis) que qualitatif (qualité de vie, observance, niveau deconnaissances des patients et enquête de satisfaction) lors de la première et dernière consultation d'éducation thérapeutique. D'octobre 2008 à fin août 2010, 299 consultations ont été réalisées pour 94 patients (37 naïfs et 57 en échec thérapeutique). Elles concernaient 34 femmes et 60 hommes. Le pourcentage de charge virale indétectable de notre population étudiée à la première consultation était de 15 % et 4 mois après la dernière consultation de 73 %. De plus, nos résultats ont montré que l'effet semblait perdurer dans le temps car les résultats virologiques favorables observés, 4 mois après le programme d'ETP, étaient encourageants. Les causes de non-observance les plus fréquentes étaient : les oublis, le nombre important de comprimés, les horaires de prise ou encore la dépression. On observait aussi une amélioration des résultats du test de connaissance entre la première et dernière consultation grâce à l'acquisition de connaissances concernant essentiellement les thèmes « traitement » (+ 10 %) et « maladie » (+ 32 %). L'ensemble des patients et des intervenants étaient totalement satisfaits par le programme d'éducation thérapeutique. Les résultats obtenus dans cette étude mettent en avant les avantages de la mise en oeuvre de ce programme. Grâce à cette étude, des actions ciblées peuvent donc être organisées afin d'améliorer encore notre programme d'ETP.

When we first had the idea for this meeting, Ralph Nelson, who had organized a previous course at the "Ettore Majorana" Centre for Scientific Culture, suggested that Erice would be the ideal place for such a meeting. Nahum, Nelson and Rogers became Co-Directors of the Course, with the help of Alessandro Rindi, the Director of the School of Radiation Damage and Protection, and Professor Antonino Zichichi, Director of the "Ettore Majorana" Centre. The course was an outstanding success, both scientifically and socially, and those at the meeting will carry the marks of having attended, both intellectually and on a personal level where many friendships were made. The scientific content of the course was at a very high caliber, both because of the hard work done by all the lecturers in preparing their lectures (e. g. , complete copies of each lecture were available at the beginning of the course) and because of the high quality of the "students", many of whom were accomplished experts in the field. The outstanding facilities of the Centre contributed greatly to the success. This volume contains the formal record of the course lectures.
