Book excerpt: African American (AA) adolescents are currently at disproportionate risk for HIV infection, with AA youth ages 13 through 19 accounting for 56% of all new HIV cases, a rate of infection 8 times that of Caucasian youth. Prevention researchers have targeted this population with a sense of urgency, resulting in numerous studies that have investigated predictors of sexual risk behaviors among AA youth. The results of these studies have yielded findings in a number of domains including environmental, individual, and interpersonal factors. However, reported findings are often inconsistent and synthesis of findings in this area is lagging. The present study synthesized data from 79 studies examining 11 domains of variables in association with 3 outcome variables: Unprotected Sex, Condom Use, and Multiple Sex partners. Intentions and partner variables were most predictive of sexual risk behaviors in this population. Further research is needed to investigate the role of partner-related influences in predicting sexual risk behaviors.

Book excerpt: The study was aimed at determining if cultural beliefs are salient factors contributing to the spread of HIV/AIDS among African born adolescents living in southern California.A cross-sectional design was used and collected data in two phases.The first phase was qualitative in which data were collected from 25 participants.The second phase utilized a questionnaire which was developed from the results of first phase, using the theory of planned behavior.Of the 150 teenagers completed the questionnaire,62% were male and 38% female.75%indicated they were attending high school. Multiple regressions were done using versions 14 of SPSS software as follows; behavior was regressed on intention and perceived control. Intentions to have sex and use condom when having sex were regressed against attitude, subjective norm, and perceived control, and attitudes on outcome beliefs. Subjective norm was regressed on normative beliefs and perceived behavioral control on control beliefs. The strongest predictor of intention to have sex was attitude, followed by subjective norm.The responses of enjoying sex,"wanting to be a parent"and to "keep sexual partners" predicted the adolescents' attitudes.

Much of the HIV epidemic response had been at the individual level in the hope that this narrow focus would provide the key to containment and resolution of spread. However, over the ten years since the epidemic had taken hold, it was clear that paradigms were limited, input was uncritical and large cohorts were overlooked. In this text a series of contributions have been compiled to explore adolescent issues ranging from sexual behaviour and health education campaigns to HIV prevention and HIV/AIDS care. The chapters begin by giving an overview of adolescent problems, such as homelessness, pregnancy and gender, and explore why these problems are so often overlooked. We then move on to an examination of the facts and fictions associated with adolescent risk, challenging some of the basic current notions underpinning approaches to the subject at the time. Also included are particular focused studies of Australian adolescents’ beliefs about HIV and STDs and also the American adolescents’ perceptions of drug injection. Finally, the volume gives a focused view of those with HIV infection, with a review of findings of the time, neuropsychological and psychological factors. This overview provided some comments on merging issues and future directions. Today it can be read in its historical context.

It provides an intellectual context for empirically-based and theory-supported interventions geared toward this population. African American women are disproportionately burdened by Human Immunodeficiency Virus (HIV) and Acquired Immunodeficiency Syndrome (AIDS). Statistics show that African American women account for 64% of all HIV/AIDS cases reported in 2005 compared to White women at 19% and Hispanic women at 15% (CDC, 2005). Typically, the majority of HIV/AIDS research focuses on prevention for lowincome, substance abusing minority women, adolescents, and men who have sex with men (MSM), while young Black college women are ignored as a risk group. Though this group does not have some of the common risk factors commonly associated with HIV such as poverty, injection drug use, or low levels of education, they still engage in behaviors that place them at risk for contracting HIV. This study consisted of convenience sample of 189 young Black women from Clark Atlanta University between the ages of 18 and 24. Participants were recruited through various campus student organizations. A hierarchical multiple regression analysis was used to test each research hypothesis. Results indicated that type(s) of sexual relationship was the strongest predictor of condom use among young Black college women and accounted for 2.5% of the variance in their condom use. HIV/AIDS knowledge, condom use self-efficacy, substance use nor HIV/AIDS perceived risk predicted this sample's condom use.

Book excerpt: African-American and Latino men who have sex with men (MSM) bear a disproportionately large burden of the Human Immunodeficiency Virus (HIV) epidemic in the United States. To further enhance HIV prevention efforts among men of color, a survey was conducted within New York City's house ball community; a community largely comprised of racial and ethnic minority persons. Time-space sampling was adapted to recruit participants for the survey from venues frequented by members of the house ball community. Using logistic regression analysis, this study examined the effects of perceived stigma, enacted stigma, sense of community and self-esteem on unprotected anal intercourse (UAI) among a sub-sample of men in the survey. Both perceived and enacted stigma had a modest direct effect on engaging in UAI. The direct effect on UAI was significant even after controlling for covariates in the model. The magnitude of the effect on UAI did not vary by race/ethnicity or sexual identity. In addition, perceived and enacted stigma correlated negatively to both sense of community and self-esteem scores. Although sense of community did not buffer the effect of perceived or enacted stigma on UAI, both sense of community and self-esteem were protective against engaging in UAI. However, while the direct effect of sense of community on UAI remained after controlling for covariates in the model, the effect self-esteem had on UAI diminished after adding variables to the model. Further, self-esteem was negatively correlated with both perceived and enacted stigma, but it did not mediate perceived and enacted stigma's effect on UAI. Implications for HIV prevention strategies given these findings are discussed. Implications include developing multilevel interventions, including structural interventions, to reduce the stigma that is perceived and experienced by men of color as well as building stronger communities for African American and Latino MSM.

Data has been gathered from a young adult sample of African-American males and females. Information is presented in a readily accessible manner so the reader can understand the variability of risky sexual behaviors. The author offers factual information to draw conclusions that can be used to develop HIV/AIDS prevention programs specifically tailored for the African-American community. The first chapter provides an introduction, rationale, and overview of the study. Basic information about the prevalence of AIDS among various African-American populations are presented. Then, Johnson describes information about the subjects, measures of sexual behaviors, drug use, attitudes about the use of condoms, knowledge about AIDS, and perceived susceptibility of being exposed to HIV/AIDS. Next, Johnson describes the sexual attitudes and behaviors of African-American males and females who are currently involved with multiple partners and those who have been previously treated for sexually transmitted diseases. He then describes the characteristics of African-Americans with HIV/AIDS. The epilogue summarizes the major findings and presents suggestions for AIDS prevention activities for African-American young adults.

The sub sample used was representative of first and second generation Mexican, Puerto Rican, and Cuban adolescents attending U.S. schools in the mid nineties. Logit regression models were used to analyze data. The four outcome variables included being sexual active, having had four or more lifetime sexual partners, condom use at most recent sexual intercourse, and drug or alcohol use at most recent sexual intercourse. Predictor variables included race, ethnicity, parental level of education, and family structure. Proxy measures for the acculturation patterns described in segmented assimilation theory were used that included four different combinations of language spoken at home (Spanish vs. English) and family bonds (Strong vs. Weak). Results: Puerto Rican adolescents were more likely to ever have had four or more lifetime sexual partners (OR = 3.102) and to have used a condom at most recent sexual intercourse (OR = 3.569) than the other Latino youth in the sample. Adolescents who were from two parent households were less likely to be sexually experienced (OR = 0.439) and to ever have had four or more lifetime time sexual partners (OR = 0.397) than those from single parent households. Results of acculturation patterns suggest that adolescents who speak English at home and who have disruptive family bonds have the highest predicted probability of being sexually active, 47.76%, and having four or more lifetime sexual partners, 18.24%. Inversely, adolescents who speak Spanish at home and have strong family bonds have the lowest predicted probability of being sexually active, 22.14%, and ever having had four or more lifetime sexual partners 3.87%. Discussion: The probability of HIV sexual risk vary in Latino youth by ethnicity, family structure, and acculturation patterns. HIV prevention efforts targeting Latino youth should strive to support single headed households, promote bicultural competencies, strengthen parent-adolescent relationships, improve supervision capability for Latino parents, and promote condom use in sexual active youth, especially Mexican and Cuban adolescents.

Book excerpt: The growing number of HIV infections among young black women is staggering and representative of a domestic, epidemiological crisis. Thus far, there is a paucity of HIV prevention programs whose specific focus encompasses the socio-cultural characteristics of African Americans. To address this shortcoming, a socio-cultural model of risk behavior was developed to examine factors associated with sexual risk behavior among young African American women using a cross-sectional survey design. A total of 51 female undergraduate college students were recruited from the Internet via the website Facebook.com and complete online survey items that included demographics, AIDS knowledge and attitudes, self-efficacy, sexual power, family communications, and sexual behavior. Multivariate linear regression analysis found that relationship power, specifically decision making control in relationship was a significant predictor of safe sex behavior. Furthermore, the multivariate linear regression analysis also demonstrated that peer support for safe sex practices was a significant predictor of condom use for this population of African American, female, college students. In conclusion, it appears that communication among peers and with male partners is the most salient factor in determining engagement in safe sex behavior for this population.
