Book excerpt: Alzheimer disease (AD) is a neurodegenerative disorder characterized by significant cognitive deficits, behavioral changes, sleep disorders and loss of functional autonomy. AD represents the main cause of dementia and has become a major public health issue. In addition, the number of patients suffering from AD is growing rapidly as the population ages worldwide. Memory impairment is usually the earliest clinical and core symptom of this disease. The diagnosis at a late clinical stage is relatively easy. However, a delay in the diagnosis is damageable for the handling of patients in terms of optimal medical and social care. The actual interest of the scientific head-ways is to optimize the diagnosis in prodromal stage of the disease and to propose personalized therapeutic solutions to individual patients. New revised AD diagnostic criteria include early alteration of cerebrospinal fluid (CSF) biomarkers: decrease of amyloïd peptides (Aβ42), and increase in tau and phosphorylated-tau (p-tau) protein concentration. This recognition of CSF biological biomarkers for the diagnosis of AD is a major step towards the “molecular” diagnosis and follow-up of the disease. Many issues are however still subject of debate. This e-book provides a comprehensive overview of the state of the art of fluid biomarkers for AD, e.g. which novel biomarkers should be implemented in clinical practice for diagnosis or for monitoring treatment or side effects, which ones are new for AD or related dementias or what is the potential of peripheral blood markers. All these medical conditions are accompanied by the disruption of blood-brain barrier (BBB). The BBB is an interface, separating the brain from the circulatory system and protecting the central nervous system from potentially harmful chemicals while regulating transport of essential molecules and maintaining a stable environment. Owing to the inability of the neurons to regenerate on their own after neurodegeneration or severe damage to the neural tissue, neurodegenerative disorders do not have natural cures on their own. Neuroregeneration is a viable way to curb neurodegeneration. One of the current approaches is stem cell-based therapy that has been shown to be potentially helpful for the application of neuronal cell replacement for neuroregeneration. It is vital that the neurodegenerative disorder being detected at an early stage as it can provide a chance for treatment that may be helpful to prevent further progression of the fatal disease. Thus, research has focused on developing effective non-invasive diagnostic methods for early detection of these disorders. Molecular diagnostics can provide a powerful method to detect and diagnose various neurological disorders. Such diagnosis can enhance early detection, provide subsequent medical counsel based on medical pathway, as well as to gain better insight of neurogenesis and hopefully eventual cure of the neurodegenerative diseases. With research reports, reviews, mini-reviews and commentary, this research topic covers a wide range of areas in neurodegeneration research, including diagnosis and prognosis; regulating central nervous system; biomarkers and brain injury induced neurobehavioral outcomes among other timely reports on neurodegeneration.

Book excerpt: Background Parkinsonu2019s disease and Alzheimeru2019s Disease and Related Dementia (ADRD) are categories of aging disorders with complex and heterogeneous symptomatology. The clinical manifestations of these neurological disorders are characterized by heterogeneity in age at onset, disease duration, rate of progression, and constellation of cognitive, motor, and non-motor features. Due to these variable presentations, counseling of patients about their individual risks and prognosis is limited. There is an unmet need for predictive tests that facilitate early detection and characterization of distinct disease subtypes as well as improved, individualized predictions of the disease course. The emergence of machine learning to detect hidden patterns in complex, multi-dimensional datasets provides unparalleled opportunities to address this critical need. We have developed predictive models for Alzheimeru2019s and Parkinsonu2019s related disorders. Our efforts have focused on: (1) developing predictive models and identification of biomarkers which are multi-modal (clinical, biological, genetic, and imaging data), and (2) relying on unsupervised machine learning techniques to identify disease subtypes that are multidimensional. Methods and findings We used unsupervised and supervised machine learning approaches for subtype identification and prediction. We used machine learning methods on comprehensive, longitudinal clinical data from the Parkinson Disease Progression Marker Initiative (PPMI) (n=328 cases) and Alzheimer's Disease Neuroimaging Initiative (ADNI) (n= 248) to identify patient subtypes and to predict disease progression. The resulting models were validated in an independent, clinically well-characterized cohort from the Parkinson Disease Biomarker Program (PDBP) (n=112 cases). Our analysis distinguished three distinct disease subtypes with highly predictable progression rates, corresponding to slow, moderate and fast disease progressors, after 24, 48, and 72 months from baseline. We achieved highly accurate projections of disease progression four years after initial diagnosis with an average Area Under the Curve of 0.93 (95% CI: 0.96 u00b1 0.01 for PDvec1, 0.87 u00b1 0.03 for PDvec2, and 0.96 u00b1 0.02 for PDvec3). We have demonstrated robust replication of these findings in the independent validation cohort.Conclusion These data-driven results enable clinicians to deconstruct the heterogeneity within their patient cohorts. This knowledge could have immediate implications for clinical trials by improving the detection of significant clinical outcomes that might have been masked by cohort heterogeneity. We anticipate that machine learning models will improve patient counseling, clinical trial design, allocation of healthcare resources and ultimately individualized clinical care.

It was also conducted under the aus pices of the Alzheimer's Association Ronald and Nancy Reagan Research Institute, USA. The Conference was attended by 550 participants from 28 countries, representing a broad spectrum of research interests; and included a well-balanced representation from aca demia, clinical institutions and pharmaceutical industry. The four-and-one-half day meeting served as an excellent medium for surveying the current preclinical and clinical develop ments in AD, PO, and other related disorders. The scientific program was divided into 24 oral sessions and daily poster sessions. The conference culminated in a round table discus sion. There were 122 talks and 161 posters. This book incorporates a combination of both.
