The most frequent cause of stroke is a localized disturbance of cerebral circulation, i.e., cerebral ischemia. Less common are spon taneous intracerebral and subarachnoid hemorrhages and sinus ve nous thromboses. The introduction of new diagnostic procedures such as cranial computed tomography, magnetic resonance imaging, digi tal subtraction radiologic techniques, and various ultrasound tech niques has led to impressive advances in the diagnosis of stroke. Through the planned application of these techniques, it is even possible to identify the pathogenetic mechanisms underlying focal cerebral ischemia in humans. However, these diagnostic advances have made the gap between diagnostic accuracy and therapeutic implications even greater than before. This fact can be easily explained. In the past, therapeutic studies had to be based on the symptoms and temporal aspects of stroke; it was impossible for early investigations to consider the various pathogeneses of cerebral ischemia. Inevitably, stroke patients were treated as suffering from a uniform disease.

The digital anatomy correlation used in this work is the natural continuation of efforts to apply the University of Colorado Visible Human data set to gastroenterology. The Visible Human data set was created by Dr. Vic Spitzer and colleagues at the University of Colorado and is currently housed at the universitys Center for Human Simulation. The data set consists of high resolution transaxial digital images captured as cadavers were abraded away at 1 mm or less depths. These images are compiled into blocks of data and each structure is identified. This information can be used to pull out and manipulate 3-D structures as well as allowing one to review planar anatomy in any orientation. Using the Visible Human dataset, one should be able to find a normal anatomy correlate to any image found during a EUS examination. However, as important as normal anatomy is, it is the abnormal features which are the crux of an EUS examination. Endosonographers are asked to define lumps, bumps, cysts to find correlates for symptoms and abnormal laboratory findings. Accuracy requires a tremendous amount of skill and experience. To help in this task, we have assembled chapters from a world-wide group of expert endosonographers. These authors have shared their insight and images to help the readers of this work better see and understand some of the complexities uncovered during a EUS evaluation. Since 1987 the Legacy raised by my Man in Motion World Tour (24901 miles wheeled around the world March 1985-May 1987) has pro $13 million dollars to research and rehabilitation in the areas vided over of spinal cord injury. I hope that in some small way this funding has contributed to the development of the vital programmes that supported me and many others. The effects of spinal cord injury are traumatic and life-shattering and require a skilled interdisciplinary approach. I congratulate those who have contributed to this book and challenge each one of you to never give up on your dreams to find the answers to the optimum treatment of spinal cord injury and disease.
