He on Eve Began to cast lascivious eyes; she hirn As wantonly repaid; in lust they burn. " Not only did Milton regard the "forbidden fruit" as an aphro disiac, he also identified it as an apple, and an apple it has re mained until this day. Sexual behavior has always been one of the most fascinating and attention-arresting activities in human history and there has been no decrease in the fascination and curiosity it still arouses in the human psyche. 1 2 Introduction As timeless as the topic of sexual behavior is that of aphro disiacs. Workshop participants discussed challenges in neuroscience research for enabling faster entry of potential treatments into first-in-human trials, explored how new and emerging tools and technologies may improve the efficiency of research, and considered mechanisms to facilitate a more effective and efficient development pipeline. There are several challenges to the current drug development pipeline for nervous system disorders. The fundamental etiology and pathophysiology of many nervous system disorders are unknown and the brain is inaccessible to study, making it difficult to develop accurate models. Patient heterogeneity is high, disease pathology can occur years to decades before becoming clinically apparent, and diagnostic and treatment biomarkers are lacking. In addition, the lack of validated targets, limitations related to the predictive validity of animal models - the extent to which the model predicts clinical efficacy - and regulatory barriers can also impede translation and drug development for nervous system disorders. Improving and Accelerating Therapeutic Development for Nervous System Disorders identifies avenues for moving directly from cellular models to human trials, minimizing the need for animal models to test efficacy, and discusses the potential benefits and risks of such an approach. This report is a timely discussion of opportunities to improve early drug development with a focus toward preclinical trials.

Book excerpt: An ideal health care system relies on efficiently generating timely, accurate evidence to deliver on its promise of diminishing the divide between clinical practice and research. There are growing indications, however, that the current health care system and the clinical research that guides medical decisions in the United States falls far short of this vision. The process of generating medical evidence through clinical trials in the United States is expensive and lengthy, includes a number of regulatory hurdles, and is based on a limited infrastructure. The link between clinical research and medical progress is also frequently misunderstood or unsupported by both patients and providers. The focus of clinical research changes as diseases emerge and new treatments create cures for old conditions. As diseases evolve, the ultimate goal remains to speed new and improved medical treatments to patients throughout the world. To keep pace with rapidly changing health care demands, clinical research resources need to be organized and on hand to address the numerous health care questions that continually emerge. Improving the overall capacity of the clinical research enterprise will depend on ensuring that there is an adequate infrastructure in place to support the investigators who conduct research, the patients with real diseases who volunteer to participate in experimental research, and the institutions that organize and carry out the trials. To address these issues and better understand the current state of clinical research in the United States, the Institute of Medicine's (IOM) Forum on Drug Discovery, Development, and Translation held a 2-day workshop entitled Transforming Clinical Research in the United States. The workshop, summarized in this volume, laid the foundation for a broader initiative of the Forum addressing different aspects of clinical research. Future Forum plans include further examining regulatory, administrative, and structural barriers to the effective conduct of clinical research; developing a vision for a stable, continuously funded clinical research infrastructure in the United States; and considering strategies and collaborative activities to facilitate more robust public engagement in the clinical research enterprise.

Book excerpt: From running with the infamous Calton Tongs to running Calton Athletic, David Bryce's life story is a remarkable account of crime, violence, alcoholism and drug addiction in Glasgow's gangland. A respected 'hard man', Bryce worked his way through most of HM's prisons in Scotland before an epiphany in 1977 made him realise that he was a hopeless alcoholic who needed help. A five-year battle followed, during which he sank into the abyss of heroin addiction before finally getting clean. In 1985, in an effort to help others who were struggling against substance abuse, Bryce set up Calton Athletic, a football team and social group made up solely of recovering drug addicts. Reformed gangster Jimmy Boyle was one of the first to have faith in Bryce and Calton Athletic, but the club eventually won widespread respect. Gordon Brown, then a young MP, was so impressed that he wrote an article for The Observer which led to a TV film starring Lenny Henry and Robbie Coltrane. In the '90s, Ewan McGregor, Irvine Welsh and the Trainspotting crew sought Bryce's advice and friendship during the making of the internationally acclaimed cult film, while Robbie Williams begged to switch nationality and play for Calton Atheltic in an 'England v. Scotland' celebrity charity match. Bryce's uncompromising belief that the only way to come off drugs is to go cold turkey and stay completely clean saw him clash with government agencies over the 'harm-reduction' policy of recent years. The club's statutory funding was withdrawn in 1998, but today Calton Athletic are still providing an invaluable lifeline while the 'official' drugs policy has become increasingly discredited. Alive and Kicking is an inspirational tale of survival and success against the odds.

NPSs encompass a wide range of different compounds and drug classes but had been dominated by synthetic cannabinomimetics and psychostimulatory synthetic cathinones, so-called β-keto amphetamines. Book excerpt: Drug dependence-according to the 1969 W orId Health Organization definition-is 'A state, psychic and sometimes also physical, resulting from the interaction between a living organism and a drug, characte rized by behavioural and other responses that always include a com pulsion to take the drug on a continuous or periodic basis in order to experience its psychic effects, and sometimes to avoid the discomfort of its absence. Tolerance mayor may not be present. A person may be dependent on more than one drug'. The substitution of the term 'Drug Dependence' in place of the previously employed terms 'Drug Addiction' and 'Drug Habituation' was recommended by a WHO Expert Committee in 1964. It was felt that there had been ongoing confusion in the use of the terms addiction and habituation and in particular, misuse of the term addiction. In 1965 the WHO Expert Committee 'was pleased to note the generally favourable reaction to the recommendations' made in the previous year and the gradual acceptance of the new term. In general, in subsequent years the term 'Drug Dependence' has gained increasing acceptance. Yet it would seem that difficulties may arise whatever term and definition may be used (Glatt, 1974), and although by and large the newer term seems more appropriate than the older definitions, one still often comes across the term' Addiction', and this not only when discussing the matter with laymen or drug misusers themselves.
