Book excerpt: Extinguishing a public health threat is difficult under any condition, let alone during a sweeping national revolution. In this first comprehensive study of tuberculosis in modern Cuba, Kelly Urban analyzes the medical, social, and governmental responses to the highly contagious disease as the island was heading into and emerging from the Revolution of 1959, providing a window onto broad questions of citizens' rights, biomedicine and public health, and political change. Drawing on a diverse range of sources revealing the perspectives of those at the center of power and those on the margins, Urban finds that the Cuban republican state intervened to confront the tuberculosis problem only after coming under intense grassroots pressure. Cuban citizens forged an activist political subculture around tuberculosis, rejecting discourses that blamed the sick for their own illness. This loose coalition of sanatorium patients, tenement dwellers, black public intellectuals, labor organizers, and reform-minded physicians won entitlements to state health care and pressed for other social rights that influenced health. Their critiques of the state's politicized and inefficient tuberculosis program contributed to the declining legitimacy of the Batista government, helping to spur the Revolution and an innovative restructuring of the public health system.

Operation Pedro Pan is often remembered within the U.S. as an urgent “rescue” mission, but Deborah Shnookal points out that a multitude of complex factors drove the exodus, including Cold War propaganda and the Catholic Church’s opposition to the island’s new government. Shnookal illustrates how and why Cold War scare tactics were so effective in setting the airlift in motion, focusing on their context: the rapid and profound social changes unleashed by the 1959 Revolution, including the mobilization of 100,000 Cuban teenagers in the 1961 national literacy campaign. Other reforms made by the revolutionary government affected women, education, religious schools, and relations within the family and between the races. Shnookal exposes how, in its effort to undermine support for the revolution, the U.S. government manipulated the aspirations and insecurities of more affluent Cubans. She traces the parallel stories of the young “Pedro Pans” separated from their families—in some cases indefinitely—in what is often regarded in Cuba as a mass “kidnapping” and the children who stayed and joined the literacy brigades. These divergent journeys reveal many underlying issues in the historically fraught relationship between the U.S. and Cuba and much about the profound social revolution that took place on the island after 1959. The scope of this book, though, does not end with 1961 and is not limited to Cuba and its historical relations with Spain, the United States, and the former Soviet Union. Nearly 50 years after the Year of Education in Cuba, the Literacy Campaign's legacy is evident throughout Latin America and the 'Third World.' A world-wide movement today continues against neoliberalism and for a more humane and democratic global political economy. It is spreading literacy for critical global citizenship, and Cuba's National Literacy Campaign is a part of the foundation making this global movement possible. The author collected about 100 testimonies of participants in the Campaign, and many of their stories and perspectives are highlighted in one of the chapters. Theirs are the stories of perhaps the world's greatest educational accomplishment of the 20th Century, and critical educators of the 21st Century must not overlook the arduous and fruitful work that ordinary Cubans, many in their youth, contributed toward a nationalism and internationalism of emancipation.
