Available in PDF, EPUB and Kindle. Book excerpt: The Military Health System (MRS) has approximately 8.7 million eligible beneficiaries. These beneficiaries include active duty military personnel and their family members, retired military personnel and their family members, and surviving family members of deceased military personnel. In 2001, the Department of Defense (DoD) spent just over $2 billion on pharmacy benefits. Much like the private health care sector, the MRS has experienced a rapid growth in pharmaceutical expenditures, which have increased an average of 17 percent a year over the past six years. Both the DoD and the U.S. Congress have identified the MRS pharmacy benefit as an area for reform. To this end, Section 701 of the National Defense Authorization Act for Fiscal Year 2000 requires the Secretary of Defense to establish an effective, efficient, and integrated pharmacy benefits program. According to the legislation, titled the Pharmacy Benefits Redesign Program, "The pharmacy benefits program shall include a uniform formulary of pharmaceutical agents which shall assure the availability of pharmaceutical agents in the complete range of therapeutic classes. The Act further specifies that "The uniform formulary will be applicable to all prescribers within the facilities of the uniformed services (i.e., military treatment facilities MTFs) and the TRICARE program. The pharmaceutical agents on the formulary will be available through the MTFs and retail pharmacies designated or eligible under the TRICARE program, as well as the National Mail Order Pharmacy program."

To this end, the DoD is required to establish a single uniform formulary (UF) of covered drugs. The legislation also requires two surveys of prescribers-a baseline survey prior to UF implementation and a post-implementation survey. In the baseline survey, most direct-care prescribers reported a high degree familiarity with formularies and perceived formulary management as contributing toward quality of care, whereas most purchased-care respondents reported less familiarity with formulary lists and did not believe that formulary management contributes to quality of care.

In 2002 alone, the Department of Defense spent about $3 billion on outpatient pharmacy benefits. As part of an effort to redesign the TRICARE pharmacy benefit to save costs, the Department of Defense is considering moving from a two-tiered to a three-tiered co-payment system, which will increase the co-payment for some classes and brands of drugs. Providers (acting in the interest of their patients) would, theoretically, have an incentive to prescribe less-costly options. To predict how changing to a three-tiered system will affect costs and pharmacy utilization, the authors use an existing data resource to determine how beneficiaries age 45 to 64 in private-sector health plans responded to similar changes in pharmacy benefits. In this analysis, the authors assess, among other potential outcomes, how changing to a three-tiered system would affect aggregate costs and pharmacy utilization and how it would affect the utilization of specific (high-cost) classes of medications.

The John Warner National Defense Authorization Act (NDAA) for Fiscal Year 2007 required GAO to examine DOD's pharmacy benefits program. Specifically, as discussed with the committees of jurisdiction, GAO examined DOD's prescription drug spending trends from fiscal years 2000 through 2006 and DOD's key efforts to limit its prescription drug spending. To conduct this work, GAO analyzed DOD's data on spending trends, including trends in beneficiary pharmacy use. GAO also assessed DOD's cost avoidance data and the agency's efforts to limit spending through its uniform formulary, which is a list of preferred drugs available to all beneficiaries. GAO interviewed DOD officials about these and other efforts to limit spending.

Book excerpt: The rapid rise in health care costs, the closure of military treatment facilities, and the rising number of retired military beneficiaries have prompted the Defense Department (DOD) to continually reengineer its health care delivery system. DOD's TRICARE health care system provides most of its care at Army, Navy, and Air Force facilities, supplemented by civilian health care services arranged by regional TRICARE contractors. Among health care services, the pharmacy benefit is most in demand by military beneficiaries. As in the private sector, DOD's pharmacy costs have continued to grow relative to total health care costs. GAO estimates that DOD's pharmacy costs rose 13 percent between 1995 and 1997, while its overall health care costs increased two percent during that same period. This report discusses (1) the adequacy of the information that DOD and its contractors use to manage the pharmacy benefit; (2) the merits and the feasibility of DOD and its contractors applying commercial best practices, including a uniform formulary, in managing its pharmacy programs; (3) the merits or limitations of recent mail-order and retail pharmacy initiatives to secure discounted DOD drug prices; and (4) the potential effects that military treatment facility's funding and formulary management decisions can have on beneficiaries' access to pharmacies and TRICARE contractors' costs.

Book excerpt: Thanks to remarkable advances in modern health care attributable to science, engineering, and medicine, it is now possible to cure or manage illnesses that were long deemed untreatable. At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugsâ€"coupled with the broader trends in overall health care costsâ€"is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. This report explores structural and policy factors influencing drug pricing, drug access programs, the emerging role of comparative effectiveness assessments in payment policies, changing finances of medical practice with regard to drug costs and reimbursement, and measures to prevent drug shortages and foster continued innovation in drug development. It makes recommendations for policy actions that could address drug price trends, improve patient access to affordable and effective treatments, and encourage innovations that address significant needs in health care.

This Institute of Medicine committee was pleased to assist the Congress with this review, in part because the committee saw in the VHA example an opportunity to understand and anticipate problems that all publicly funded programs are likely to encounter in this new age of pharmaceuticals. The Congress asked the committee to review the restrictiveness of the National Formulary, its impact on the costs and quality of care in the VHA, and how it compared to formularies and drug management practices in the private sector and in other public programs, especially Medicaid. Detailed in the pages that follow, the committee's findings and conclusions on these questions are, the committee believes, highly instructive, though not always in the ways that we anticipated.
