Au cœur de l'actualité, la dispensation à l'unité des médicaments à l'officine est un projet politique porté depuis 2017. J'ai souhaité, au travers de cette thèse, revenir sur l'expérimentation faite en 2014 sur la dispensation des antibiotiques à l'unité, et discuter ses résultats en termes d'observance, en termes écologiques, économiques et organisationnels. Cette expérimentation a entrainé de vives réactions des acteurs du secteur de la santé. La faisabilité de la généralisation de cette pratique à l'officine, avec ses intérêts et ses limites, est présentée dans ce travail. La mise en place d'une telle pratique nécessitera indéniablement une évolution de la législation encadrant la dispensation des médicaments, mais aussi des préparations de doses à administrer, ainsi que du conditionnement proposé par l'industrie pharmaceutique.

Book excerpt: La prise en charge médicamenteuse du patient comporte de nombreuses étapes, de la prescription à l'administration du médicament. Le pharmacien hospitalier et le préparateur en pharmacie hospitalière (PPH) jouent un rôle important lors de la dispensation de médicaments en rétrocession. Cette dernière est une activité critique qui nécessite la maîtrise pharmacologique des traitements et de la réglementation. Dans un objectif d'amélioration de la sécurité et de la qualité des dispensations, un projet de formation des PPH a été élaboré dans le service rétrocession de l'hôpital Nord-Ouest de Villefranche-sur-Saône. Après une analyse de la répartition des dispensations dans le temps et entre les PPH référents et non référents, un état des lieux de la qualité des dispensations a été réalisé à l'aide d'une grille préalablement élaborée. Il a mis en évidence la nécessité de mettre en place une formation orientée sur la maîtrise des médicaments et leurs conseils associés. Des formations initiales et continues à destination des PPH référents ont alors été réalisées. Une seconde phase d'observation a ensuite été menée selon des modalités identiques à l'état des lieux initial et a permis de comparer la qualité des dispensations avant et après la mise en place des formations. Une amélioration des connaissances et des compétences a été observée. Cette étude prospective montre une amélioration de la qualité et de la sécurité des dispensations après la mise en place des formations. Dans une démarche d'amélioration continue, ces formations nécessitent d'être poursuivies au sein du service de rétrocession et d'être étendues à l'ensemble de l'équipe des préparateurs en pharmacie.

En France, la loi « Hôpital, Patients, Santé et Territoires » du 21 juillet 2009 a bouleversé le métier de pharmacien d'officine avec un élargissement de ses missions. A présent, la notion de protocole de coopération permet aux pharmaciens de disposer de délégations d'activités telles que des actes de soins ou de prévention préalablement exercés par les médecins. Cette nouvelle mission contribue à l'élargissement de l'offre des soins, à la réduction des délais d'accès à une prise en charge et donc à une amélioration du parcours de soins. Le pharmacien est en mesure, dans le cadre de ces protocoles, de renouveler ou d'ordonner un traitement médicamenteux, de réaliser des actes simples et de prescrire un arrêt de travail en cas de nécessité. Pour prétendre à la dispensation protocolisée, le pharmacien doit faire partie d'une structure permettant un exercice coordonné. Par ailleurs il doit disposer d'éléments pratiques nécessaires à sa mise en place tels qu'un espace de confidentialité permettant la consultation avec un accès à des sanitaires, un système d'information partagé et avoir suivi une formation préalable réalisée par les médecins de la même structure. L'enquête menée auprès des pharmaciens maîtres de stage de l'ex-région Limousin a permis de faire un état des lieux de leurs connaissances au sujet de la dispensation protocolisée, de jauger leur motivation à la mettre en place au sein de leur officine et d'analyser les facteurs freinant son application à grande échelle. Cette nouvelle mission a été jugée importante et nécessaire par une majorité de pharmaciens mais trop complexe pour être mise en œuvre. Une nouvelle forme de coordination plus simple et plus souple serait une porte d'entrée à l'interprofessionnalité autour du patient.
