Care has been exercised to reflect the latest progress in this rapidly advancing department. Because of recent discoveries in the physiology of digestion, a chapter has been added upon this subject, but from the Viewpoint of the clinician rather than that of the physiologist. The chapter on Examination of the Stomach Contents includes those tests which, in the Opinion of the author, will best assist the practitioner in diagnosis and treat ment. With regard to surgical treatment, no more has been attempted than to give the indications. Although surgery is indicated in many cases of stomach disease, the utmost caution is necessary to avoid useless or injurious intervention. It appears to me strange, that whereas at the present time, bacteriological and pathological examinations are used so extensively in the diagnosis of disease, chemical methods are so rarely made use of in practice, and dealt with so briefly in text-books. The stomach-tube should be part of the armamentarium of every practitioner. It is not essential that he should perform all the quantitative analyses to which I refer, but I venture to think that he would be repaid amply, if he carried out the ordinary qualitative tests, and the estimation of the total acidity of the gastric contents. The practitioner, be he consultant or otherwise, who, without the aid which such investigations afford, continues to treat a patient suffer ing from persistent indigestion, or gastric pain, is acting unfairly to his patient and unjustly to himself. Book excerpt: Excerpt from The Surgical Treatment of Ulcer of the Stomach The reason of this discrepancy is simply that the condition of the mucous membrane of the stomach during life, especially when it is in amed, is absolutely different from what it is after death. During life, and especially while digestion is going on, the mucous mem brane is exceedingly well supplied with blood. It is soft, bright red, thick, and full of uid. After death it is thin, bloodless, accid, and in colour a pale slaty gray - it hardly looks like the same structure, the difference is so great; and in cases of acute ulceration, owing to the in ammatory changes in the surrounding tissues, the difference is greater still. Immediately around the ulcer the mucous membrane is swollen, con gested and raised above the level of the rest. Its colour is bright red, though the interior of the stomach may be paler than natural from loss of blood. Here and there I have seen the surface dotted with little blood-stained points from vessels that have given way. The sides of the ulcer are thickened and oedematous, and so soft and swollen that if the ulcerated surface is small, and does not extend deeper than the mucous membrane, they meet together over it and convert it into a narrow fissure. After death, when the blood pressure has fallen, especially in cases in which the vessels have been emptied by previous hemorrhage, all this thickening disappears, and then the ulcer assumes the crater-like form which is usually described; but I have never seen this during life. With characteristic absence of posing, in simple, strong, and dignified language, the author has presented his subject with such sincerity and clearness that his views will meet almost invariably with ready acceptance. It has been a pleasure to give the work the careful examination necessary, and to compare his statements with those in current literature. The text has required but few modifications and additions to make it thoroughly represent the thought of the year. This is so for the reason that, although many contributions have been pub lished since the appearance of the German edit-ion, some of these have been the thrashing out of old straw, and others have been anticipated by the far-sightedness of the author. To the subjects of periodic vomit ing in children, syphilis of the stomach, the etiology of gastric ulcer, the blood in ulcer and cancer, diet in hyperacidity, acute dilatation of the stomach, digestion leukocytosis, benign tumors of the stomach, achylia gastrica, hematemesis of hepatic cirrhosis, congenital stenosis of the pylorus, operative procedure in stomach diseases, and to a few others, some words have been added, but pains have been taken not to change the author's own statements.
