Book excerpt: Excerpt from The Nutrition of the Infant IN his endeavour to make his practice as exact and scientific as possible, the author was at one time greatly discouraged by the fact that infant feeding apparently presented almost insuperable obstacles to its rational management in health, and to exact diagnosis and treat ment in the presence of disorder. While this applied, to some extent, to the management of breast feeding, it applied with greater force to substitute feeding. In hospital and private practice he was only too familiar with the various attempts to meet the requirements, and had frequent opportunities of testing the results of. Prao tically every method at all commonly used. But there were many failures, and, what was even more unsatisfactory, the successes were achieved by means so essentially empirical that they taught no lesson for the future. It must be confessed that others were satisfied where the author was greatly discontent. For it was said to be unreasonable to expect that artificial feeding should be so conducted that its results could be at all comparable with those of breast feeding. This attitude seemed to be the more serious in view of the number of women unable or unwilling to nurse their infants. It aims at providing a complete and practical epitome for the dietetic student. It is now many years since the late Sir Henry Thompson, a distinguished authority on dietetics, pointed out that no one can be a really accomplished practitioner who has not made dietetic principles and practice an important part of their professional education, yet the subject of dietetics has still no recognised place in medical teaching, and the facilities for the student acquiring a practical knowledge of it can hardly be described as adequate. The first thing necessary for food and feeding in health and disease is to be well instructed in the elements of physiology, the nature of foods, the normal laws of feeding, as well as in the deviation by which the action of these laws is modified. To this must be added some practical acquaintance with kitchen usage and processes. A certain familiarity with the resources of the cook is essential to provide a suitable daily menu which will be agreeable to the invalid, and as much as possible varied within the narrow limits induced by the circumstances of each case. In the following pages an attempt is made to present this information concisely, and applied, so far as possible, to the everyday requirements of the medical practitioner. In the preparation of the work I have derived much help (more especially with the data of the chemical composition of foods) from many sources, special mention being made of the writings of Atwater, Langworthy, and of the analytical reports published from the Lancet laboratory. Many changes were deemed necessary to aid the general practitioner in working formulae required for home-modification of milk. The author has omitted the greater part of the former chapter on Gaertner mother-milk, inasmuch as the milk is not distributed fresh daily, which is so vital. A new chapter, entitled "Milk Idiosyncrasies in Children," showing how children were fed who could not tolerate milk, is added. Another new chapter, entitled "Buttermilk Feeding," treating of a method of feeding in vogue in foundling asylums, has been added, as has also a chapter on "Scurvy." The dietary for older children has been enlarged. The method of feeding in diphtheria has been rewritten, and the method in use at the Willard Parker Hospital is described. A new chapter on "Feeding Children Afflicted with Cleft Palate" has been added. A great deal of attention has been given to the chapter on "Modified Milk from the Walker-Gordon Laboratory." Much of what had previously been written on this subject has been left out. The author believes that accuracy at the laboratory plus clean utensils is of value in many cases, and cites a successful modified-milk feeding. Some illustrations showing specimens of "Poor breastmilk" have been added. In all, this edition may safely be trusted to aid those in search of practical points in modern methods of breast- and bottle- feeding.
