According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendation stresses the importance of pest diagnosis in underpinning many activities involved in the implementation of the International Plant Protection Convention (IPPC). The recommendation encourages regional plant protection organizations and contracting parties to share knowledge and expertise, and support laboratory capacity in diagnostic protocols. Encouraging national plant protection organizations to maximize the utilization of diagnostic protocols as official procedures to adequately fulfil their obligations under the IPPC is essential. There remain, however, some strategic issues associated with pest diagnosis that should be integral to all aspects of the IPPC work program. It is in this context that a survey was conducted within the framework of the Implementation Review and Support System (IRSS) on IPPC diagnostic protocols, under the remit of the Implementation and Capacity Development Committee. This report presents the results of the survey, reflects the data received from participants, and illustrates the analysis and recommendations for optimizing the use of diagnostic protocols.

Book excerpt: "The Unified Protocols for Transdiagnostic Treatment of Emotional Disorders in Children and Adolescents (UP-C/UP-A; Ehrenreich-May et al., 2018) are transdiagnostic interventions designed to address multiple diagnoses, diagnostic categories, and/or problem types within a single treatment protocol. It might be said that modern transdiagnostic interventions originated within the early years of the twenty-first century with theoretical and empirical work by Barlow and colleagues conceptualizing a "unified approach" to understanding and treating emotional disorders (e.g., Barlow, Allen, & Choate, 2004) and, subsequently, the initial publication of the Unified Protocols for Transdiagnostic Treatment of Emotional Disorders (Barlow et al., 2011). However, up until the mid-twentieth century, psychotherapeutic approaches were nothing if not transdiagnostic, in that they addressed underlying psychodynamic and interpersonal processes theorized to lead to the development of broad psychological neuroses. This approach changed during the second half of the twentieth century, in accordance with two parallel and mutually informative developments. First, with each new iteration of the American Psychiatric Association's Diagnostic and Statistical Manual Mental Disorders (DSM) from the publication of the original DSM in 1952 to DSM-IV in 1994, the number of diagnoses proliferated, and diagnostic criteria became increasingly fine-grained. During the same historical period, new treatments for these highly specified diagnoses were developed based on new empirical and theoretical evidence (e.g., exposure for phobias), and more rigorous outcomes research on cognitive-behavioral therapy (CBT) and other interventions began to emerge (Barlow et al., 2004). Psychotherapy research was conducted using newly-developed treatment manuals addressing symptoms of highly specific diagnoses (e.g., panic disorder, specific phobias) to help ensure scientific rigor and replicability. The turn back toward transdiagnostic models of psychopathology and treatment at the turn of the twenty-first century was based on growing recognition of the commonalities among psychiatric diagnoses, the shared features of psychotherapeutic interventions for distinct diagnoses, and the beneficial effects of treatments for one diagnosis on other diagnoses, which will be further discussed below"--

Book excerpt: Multidetector-row computed tomography (MDCT) has become a fundamental imaging technique for the study of many anatomical districts in different clinical situations, as it provides a fast, reliable, and accurate simultaneous evaluation of different organs, including parenchyma, hollow viscera, vessels, and bony structures. It has also equipped the radiologist with the ability to explore areas that in the last decade were largely ignored by CT, especially the coronary arteries and the colon, and to limit invasive diagnostic tests (e.g., catheter angiography) to those cases in which interventional procedures are required. The examination quality and the consequent diagnostic accuracy of MDCT are the results of an optimized study technique, which nonetheless needs to be adapted to the particular clinical situation of the patient, while bearing in mind the radiation exposure. Another fundamental parameter in the optimization of MDCT is the protocol for the intravenous injection of iodinated contrast material; in these studies, multiple variables, some modifiable by the operator and others patient-dependent, must be considered based on the aims of maximizing arterial and venous enhancements whilst minimizing the dose of iodine injected, saving money, and increasing patient safety. Through the presentation of cases addressing different anatomical regions and various clinical indications, including emergency and neuro-imaging, this volume seeks to provide the general radiologist and trainee specialist with a guide to the main study protocols to be implemented in order to optimize examination quality and, consequently, facilitate the diagnostic process.

Never before has the make up of cancer tissues and individual cells been so exhaustively researched and char- terized. We are now capable of producing molecular “fingerprints” that ch- acterize the expression of all known and unknown genes within tumors and their surrounding tissues. More than 30,000 different genes may be measured in each patient’s tumor in a single experiment. Simultaneously, novel therapies that exploit the molecular roadmap have been developed and are now being offered to patients. These novel agents, such as Glivec, Herceptin, Iressa, and others, specifically target individual genes within tumors and can produce d- matic responses in some patients. These drugs are only the forerunners of a coming tidal wave of novel therapeutics that individually target specific m- ecules within cancer cells—more than 300 such agents are currently in phase I or II clinical trials. This is an exciting time for cancer specialists and patients alike. However, if we have learned anything from the past 50 or more years of research into cancer, it is that Lord Beaverbrook, in founding the British national health service in the 1950s, was frighteningly prescient when he defined the primary goal of health care to be “Diagnosis, Diagnosis, Diag- sis. ” Now, more than ever, it is essential that appropriate diagnostic methods and approaches are applied to the selection of patients for treatment.
