Book Ensuring Innovation in Diagnostics for Bacterial Infection

Download or read book Ensuring Innovation in Diagnostics for Bacterial Infection written by Morel C. and published by World Health Organization. This book was released on 2016-04-18 with total page 338 pages. Available in PDF, EPUB and Kindle. Book excerpt: The inappropriate use of antibiotics is a primary cause of the ongoing increase in drug resistance amongst pathogenic bacteria. The resulting decrease in the efficacy of antibiotics threatens our ability to combat infectious diseases. Rapid point-of-care tests to identify pathogens and better target the appropriate treatment could greatly improve the use of antibiotics. Yet there are few such tests currently available or being developed despite the rapid pace of medical innovation. Clearly something is inhibiting the much-needed development of new and more convenient diagnostic tools. This study delineates priorities for developing diagnostics to improve antibiotic prescription and use with the goal of managing and curbing the expansion of drug resistance. It calls for new approaches particularly in the provision of diagnostic devices and in doing so outlines some of the inadequacies in health science and policy initiatives that have led to the dearth of such devices. The authors make the case that there is a clear and urgent need for innovation not only in the technology of diagnosis but also in public policy and medical practice to support the availability and use of better diagnostic tools. This book explores the complexities of the diagnostics market from the perspective of both supply and demand unearthing interesting bottlenecks some obvious some more subtle. It calls for a multifaceted and broad policy response and an overhaul of current practice so that the growth of bacterial resistance can be stemmed.

Book Global Infectious Disease Surveillance and Detection

Download or read book Global Infectious Disease Surveillance and Detection written by Institute of Medicine and published by National Academies Press. This book was released on 2007-11-11 with total page 284 pages. Available in PDF, EPUB and Kindle. Book excerpt: Early detection is essential to the control of emerging, reemerging, and novel infectious diseases, whether naturally occurring or intentionally introduced. Containing the spread of such diseases in a profoundly interconnected world requires active vigilance for signs of an outbreak, rapid recognition of its presence, and diagnosis of its microbial cause, in addition to strategies and resources for an appropriate and efficient response. Although these actions are often viewed in terms of human public health, they also challenge the plant and animal health communities. Surveillance, defined as "the continual scrutiny of all aspects of occurrence and spread of a disease that are pertinent to effective control", involves the "systematic collection, analysis, interpretation, and dissemination of health data." Disease detection and diagnosis is the act of discovering a novel, emerging, or reemerging disease or disease event and identifying its cause. Diagnosis is "the cornerstone of effective disease control and prevention efforts, including surveillance." Disease surveillance and detection relies heavily on the astute individual: the clinician, veterinarian, plant pathologist, farmer, livestock manager, or agricultural extension agent who notices something unusual, atypical, or suspicious and brings this discovery in a timely way to the attention of an appropriate representative of human public health, veterinary medicine, or agriculture. Most developed countries have the ability to detect and diagnose human, animal, and plant diseases. Global Infectious Disease Surveillance and Detection: Assessing the Challenges-Finding Solutions, Workshop Summary is part of a 10 book series and summarizes the recommendations and presentations of the workshop.

Download or read book Microbial Threats to Health written by Institute of Medicine and published by National Academies Press. This book was released on 2003-08-25 with total page 397 pages. Available in PDF, EPUB and Kindle. Book excerpt: Infectious diseases are a global hazard that puts every nation and every person at risk. The recent SARS outbreak is a prime example. Knowing neither geographic nor political borders, often arriving silently and lethally, microbial pathogens constitute a grave threat to the health of humans. Indeed, a majority of countries recently identified the spread of infectious disease as the greatest global problem they confront. Throughout history, humans have struggled to control both the causes and consequences of infectious diseases and we will continue to do so into the foreseeable future. Following up on a high-profile 1992 report from the Institute of Medicine, Microbial Threats to Health examines the current state of knowledge and policy pertaining to emerging and re-emerging infectious diseases from around the globe. It examines the spectrum of microbial threats, factors in disease emergence, and the ultimate capacity of the United States to meet the challenges posed by microbial threats to human health. From the impact of war or technology on disease emergence to the development of enhanced disease surveillance and vaccine strategies, Microbial Threats to Health contains valuable information for researchers, students, health care providers, policymakers, public health officials. and the interested public.

Book The Development of a Rapid Diagnostic Test for Bacterial Infection

Download or read book The Development of a Rapid Diagnostic Test for Bacterial Infection written by Yuqian Ma and published by . This book was released on 2010 with total page 426 pages. Available in PDF, EPUB and Kindle. Book excerpt: Critically ill patients in the intensive care unit (ICU) frequently develop an inflammatory response, but current diagnostic tests cannot accurately distinguish a bacterial from a non-infectious aetiology, particularly within the first 24 hours. A significant number of these patients are treated with antibiotics and then subsequently turn out either not to have had a bacterial infection or the diagnosis remains uncertain. This overuse of antibiotics increases pressure on resistance. This thesis tests the hypothesis that a rapid diagnostic test based on Flow cytometric analysis of activation markers on monocytes and neutrophils can more accurately distinguish infected from non-infected patients, thereby leading to more targeted use of antibiotics. The project was also prompted by previous findings of elevated Toll like receptor 2 (TLR2) and TLR4 levels on neutrophils from septic patients with Gram positive and Gram negative infection respectively. -- Preliminary studies recruiting patients from accident and emergency (A&E) and the ICU led to development of robust patient recruitment and laboratory protocols incorporating use of a large panel of antibodies recognising surface proteins involved in phagocytosis, adhesion and chemotaxis. In vitro studies were performed to identify host and bacterial factors that increase TLR2 and TLR4 expression on neutrophils. TLR2, together with CD64 and CD 14, were increased on neutrophils after stimulation with interferon-[gamma] for 12-22 hours. No in vitro stimuli were identified that increased expression of neutrophil TLR4. -- Two clinical studies were performed in A&E recruiting 49 patients with a community-acquired bacterial infection and 41 with a non-infectious illness. Neutrophil CD64 was shown to be the best single diagnostic marker for community-acquired bacterial infection.
