Book excerpt: De 1984 à 1993, 33 cas de fentes labiales ou labio-palatines ont été recensées au Centre Hospitalier du Belvédère, dont 24 avaient été diagnostiqués en prénatal. Au terme d'une étude rétrospective et en analysant la littérature, nous avons tenté de dégager tout l'intérêt du dépistage échographiclue des fentes labiales dans la mise en route du bilan étiologique prénatal de malformation, notamment en cas de malformations associées, permettant ainsi de définir le pronostic foetal. Lors de fente labiale isolée, nous avons essayé de mesurer, tout l'impact psychologique du diagnostic prénatal, auprès de parents ayant accepté de participer à notre enquête. Ainsi, nous nous sommes posés la question de savoir si le fait de connaitre la vérité et ainsi de pouvoir se préparer à l'accueil de l'enfant, sur l'angoisse iatrogène qu'une telle annonce peut provoquer chez les parents.

Book excerpt: Objectifs : Le but de ce mémoire était d'évaluer la concordance entre le type de fente dépisté à l'échographie en anténatal et le type de fente diagnostiqué cliniquement en postnatal dans la population d'enfant ayant eu un diagnostic anténatal. Les objectifs secondaires étaient d'identifier les types d'erreurs les plus fréquemment commises. Méthodes : Ont été inclus rétrospectivement tous les enfants nés avec une fente labiale associée ou non à une fente palatine, pris en charge par le Centre des Compétences des Maladies Rares des Fentes et Malformations de la Face entre le 1 janvier 2006 et le 31 décembre 2017. Les données anténatales ont été comparées aux données postnatales. Résultats : 167 enfants ont eu un diagnostic anténatal de fente. Le diagnostic prénatal était en accord avec les résultats postnataux dans 79,6% des cas (133/167). Le calcul de la concordance a permis de calculer un kappa à 0,73 (IC95 : 0,65 - 0,81) et une valeur prédictive positive de 0,80 (IC95 : 73 ; 085). Conclusion : Il existe donc une bonne concordance du type de fente entre les diagnostics anté et post-nataux. Les erreurs les plus fréquemment commises étaient des sous-estimations du type de fente. L'amélioration de la qualité du diagnostic devrait permettre d'apporter une information précise aux parents.

Book excerpt: Introduction : Suite au diagnostic échographique de fente labiale et/ou palatine, on peut s'interroger sur l'opportunité de la réalisation d'un bilan chromosomique prénatal. Objectifs : L'objectif principal était d'estimer la prévalence des anomalies chromosomiques parmi les FL/P diagnostiquées in utero. L'objectif secondaire était d'observer l'attitude du CPDPN de Grenoble vis-à-vis de la proposition de caryotype foetal. Matériel et méthodes : Il s'agissait d'une étude observationnelle descriptive de prévalence, rétrospective, monocentrique. Etaient éligibles tous les foetus porteurs de FL/P diagnostiquée in utero et pris en charge par le CPDPN de Grenoble sur une période de 10 ans (01/01/2000 au 31/12/2009). Résultats : Au total, 147 cas de FL/P diagnostiquées in utero ont été inclus. La prévalence des anomalies chromosomiques parmi ces fentes était de 4,1%. Il s'agissait de trois trisomies 13, d'une anomalie des gonosomes en mosaïque, et de deux anomalies de structure déséquilibrée. Ces dernières n'ont été identifiées que des années plus tard, grâce à la technique de CGH-array pour l'une des deux. Dans aucun cas l'anomalie chromosomique n'avait été retrouvée associée à une fente isolée. On observait que le CPDPN de Grenoble avait proposé un caryotype foetal dans 91,7% des cas de FL/P diagnostiquées in utero. Discussion et conclusion : La technique récente de CGH-array n'est pas encore appliquée en routine, mais dans les années à venir elle devrait remplacer le caryotype en prénatal. La réalisation d'une analyse chromosomique doit être systématiquement proposée devant une fente associée, et devant une fente labio-palatine ou palatine isolée. Devant une fente labiale isolée, la proposition systématique ne paraît pas justifiée. Il serait intéressant de compléter notre étude auprès des autres CPDPN de France pour comparer les attitudes.

Elles sont principalement diagnostiquées lors de l'échographie du deuxième trimestre. La prise en charge de la mère et de l'enfant est pluridisciplinaire. Si le pronostic est favorable, dans le cas d'une fente faciale isolée, l'annonce n'en reste pas moins traumatisante pour les parents. Au cours de ce travail, nous avons évalué le vécu de la grossesse des femmes attendant un enfant porteur d'une fente labiale et/ou palatine. Puis nous avons cherché à comprendre ce que l'échographie 3D apportait à ces femmes. Cette étude a été réalisée auprès de 15 patientes, suivies au sein de dix centres pluridisciplinaires de diagnostic prénatal différents.
