Here in my organization one of the continuous quality management (CQM) focused area is Diabetes Mellitus. Diabetes Mellitus is associated with a series of underlying conditions including diabetic kidney disease (diabetic nephropathy) which can lead to chronic kidney disease. Diabetic Nephropathy is a condition developed from diabetes and if identified later in the disease state can lead to the complication of diabetic kidney disease (DKD) or chronic kidney disease (CKD). According to the National Kidney Foundation (NKF) 2020, approximately 37 million people in the U.S. are affected by CKD. Approximately 80 million people are at risk. CKD is the 9th leading cause of death in the U.S. (NKF, 2020). In order to better assess and prevent diabetic nephropathy, early screening for microalbuminuria, early diagnosis, and treatment should be implemented to better care for our patient population. It is important to explore the relationship between DM and chronic kidney disease due to it being the 9th leading cause of death in the United States (NKF, 2020). Early detection of diabetic kidney disease (DKD) mitigates the effects of the disease. According to the American Diabetes Association (ADA) and the American Association of Clinical Endocrinologist (AACE) guidelines, early detection and treatment can slow the progression of DKD which can lead to end stage renal disease if not detected early or treated. Having a positive microalbuminuria (30-300 mg per day or greater) is identified as kidney damage or disease. The higher microalbuminuria, the more damage the kidneys are thought to be. Treatment includes controlling of blood pressure by starting an angiotensin converting enzyme inhibitor (ACE) or angiotensin receptor blocker (ARBs) along with glycemic control.
