Private and government organizations are developing programs to enhance the school readiness of all young children, especially children from economically disadvantaged homes and communities and children with special needs. Well-planned and effective assessment can inform teaching and program improvement, and contribute to better outcomes for children. This book affirms that assessments can make crucial contributions to the improvement of children's well-being, but only if they are well designed, implemented effectively, developed in the context of systematic planning, and are interpreted and used appropriately. Otherwise, assessment of children and programs can have negative consequences for both. The value of assessments therefore requires fundamental attention to their purpose and the design of the larger systems in which they are used. Early Childhood Assessment addresses these issues by identifying the important outcomes for children from birth to age 5 and the quality and purposes of different techniques and instruments for developmental assessments.

Monitoring children's development in relation to this continuum through developmental screening, to identify delays, and to individualize approaches to development and learning, is an important part of providing high-quality child care and early education. Developmental screenings, which indicate whether a child is meeting expected developmental milestones or may have a developmental delay that requires further assessment, are part of a broader set of preventive health care practices recommended by experts, including the American Academy of Pediatrics (AAP). Early, regular, and reliable screening can help identify problems or potential problems that may threaten the child's developmental foundation and lead to additional delays and deficits later in childhood. Properly trained and supported professionals in early childhood settings can use information from screening to help inform practice, shape individual child interactions, and refer children and families for additional services. This paper looks at the degree to which children currently receive developmental screening, the role of child care and early education programs in connecting children to developmental screening, national efforts and funding streams to support developmental screening and its relationship to early childhood, and state policy examples and recommendations stakeholders can draw on when considering expanding access to developmental screening in early childhood settings.

Book excerpt: In the past decade, American and Canadian pediatric societies have recommended that pediatric care clinicians follow a schedule of routine surveillance and screening for young children to detect conditions such as developmental delay, speech and language delays and disorders, and autism spectrum disorder. The goal of these recommendations is to ensure that children with these developmental issues receive appropriate referrals for evaluation and intervention. However, in 2015 and 2016, the US Preventive Services Task Force (USPSTF) and the Canadian Task Force on Preventive Health Care issued recommendations that did not support universal screening for these conditions. This occasional paper is designed to help make sense of the discrepancy between Task Force recommendations and those of the pediatric community in light of research and practice. To clarify the issues, this paper reviews the distinction between screening and surveillance; the benefits of screening and early identification; how the USPSTF makes its recommendations; and what the implications of not supporting screening are for research, clinical practice, and families.
