Book excerpt: The objective of this comparative report is to present the model of future health care system revenues and expenditures in selected Central and Eastern European countries which are now the new EU member states, and to discuss projection assumptions and results. Health expenditure analysis and projections are based on the ILO social budget model, a part of which is the health budget model. The model covers health care system revenues and expenditures. It is suitable for the analysis of impact exerted by demography (especially ageing) on health care system revenues and expenditures. The objective of AHEAD project is to examine those factors. Up to date, data and information sources in new member states that could be used for the long-term comparative projections have been limited.

Book excerpt: Following the transition from central planning toward market-based economies, the formerly communist states of Central and Eastern Europe introduced a number of reforms in the finance, management, and organization of the health sector. While health sector reforms in these countries have involved deep structural changes, they have generally been less successful in improving efficiency, enhancing equity in healthcare financing and delivery, and managing clinical quality of health services. Total health expenditures have increased in almost all countries, especially in recent years, and with revenues not keeping pace, huge debts have accumulated in the health sector. Efficiency gains have been few and far between, and with the dynamic nature of technology and demographic changes increasing the complexity of health services and the health marketplace, further reforms are becoming even more difficult. This report takes stock of recent trends in health expenditure aggregates in the public sector and identifies specific areas of health expenditure reform consistent with the objectives of stabilizing the fiscal situation in these countries (the Czech Republic, Estonia, Hungary, Latvia, Lithuania, Poland, Slovakia, and Slovenia) without adversely affecting the production, delivery and utilization of health services.

In its health care system, two negative effects have combined: - impact of transformation recession on financial perspective of the health system and - public sectors (with health system as a typical example) traditionally did not participate in the transformation process at all, or only made a partial transformation and were the last sectors to start transformation process. This situation brings many challenges for Slovakia's healthcare system for the future. In basic setting receipts and expenditures of healthcare system will be one of the greatest challenges, especially with best healthcare possible for citizens. The goal of this work was to construct a macroeconomic model of revenues and expenditures of healthcare system in Slovakia and to apply this model in quantification of a basic forecast of the country's healthcare budget and for sensitivity analysis. Aim of work is based on prediction of: - life expectations - real wage projection - employment rate projection.

Book excerpt: This country report aims to give an estimation of the development of public health care expenditures in Hungary during the first half of the 21st century. The model used for analysis is the ILO social budget model. Since this model is a full budget model, it includes not only the expenditure side, but the revenue side also. The structure of the model is presented in Section 2 of this report following a discussion of the long-term projections recently prepared for the Hungarian health care system (Section 1). The third section contains a review of main databases used by the model, while the fourth describes the underlying assumptions and development of main variables of the baseline scenario. The second part of the same section discusses alternative assumptions, aiming to provide sensitivity analyses for the baseline scenario. Results of both the baseline scenario and sensitivity analyses are presented in Section 5. Section 6 summarises these results and draws conclusions, while the final section formulates some policy recommendations based on the findings of our model (Section 7).

Expenditure analysis is based on ILO social budget model, part of which is health budget model. The model takes into account the revenue side of health care system as well, which is consistent with the above-mentioned discussion on health care system funding and its sustainability. The first part of the Report is dedicated to social, and especially health-related, expenditure models and projections applied in Poland. Following, detailed description of data and information used in the current projection is presented. Baseline projections of main demographic and macro-economic variables and indicators used in the model are shown, and the assumptions for the development of these indicators and their inter-relations are discussed. Three scenarios are presented: the baseline scenario, death-related costs scenario, and the scenario with different longevity improvements. Projection results cover both the revenue and the expenditure side of health care system. Finally, conclusions are made and policy recommendations are formulated, based on projection results.

Book excerpt: The analysis aims to describe processes of demographic and epidemiological change, as well as health status self-assessment in selected Central and Eastern Europe countries (CEECs), including both the new member states and the candidate countries. The analysis is presented in the context of the use of medical services and the structure of services used. Special attention is given to those demographic and epidemiological changes that have direct impact on the frequency of medical services used and, as such, determine increases in health care costs. The ageing process and health status improvement are the main hypothethical determinants of health care costs and are therefore presented in more detail. Additionally, changes in health behaviour - mainly in the use of medical services - are discussed in the context of institutional changes in the health care sector. Comparative analysis covers the countries representing groups characterised by similar tendencies and specific health and demographic characteristics. Estonia respresents the Baltic states, Bulgaria, the Balkan countries, and Slovakia represents countries of Central Europe. Poland is a specific country, with demographic and epidemiological characteristics similar to Slovakia, but it is much larger, with a high share of rural population. In Hungary, demographic processes related to the second demographic transition began much earlier and are still dynamic, and as such, constitute a reference for other CEECs.
