Abstract: ABSTRACT Abstract of thesis entitled DEVELOPMENT OF RESIDENTIAL CARE FOR OLDER PERSONS IN CHINA: A CASE STUDY OF TIANJIN Submitted by Liu Hong for the degree of Doctor of Philosophy at the University of Hong Kong in September, 2006 China is a country with a long history and strong traditional culture. In the 1950s, although Chinese citizens were not rich, care for the older persons was not perceived as a social issue under the socialist planned economic and social system. However, in recent decades, rapid aging trend rises the urgent need for service support for the care of older persons. This study considers the historical development of residential care for older persons, and presents the status of residential care and the quality of care delivered in institutions. This study adopts qualitative methods of documentary analysis and case study, based on multiple sources of data, such as focus group interviews and in-depth interviews. The documentary analysis aims to describe the historical development of residential care for older persons in China and the transformation of social policy on residential care. After presenting a macro- picture on the historical development, this study also conducts a case study of Tianjin to assess the quality of care at the micro level. Three groups of stakeholders were interviewed, including older residents, staff and family members, to explore their perceptions on the quality of care. China has a long history of development on residential care. In the early stage of development, residential care for older persons was delivered as part of the work of social welfare and social relief. Only the "Three Nos" persons and the "Five Guarantee" receivers were qualified to live in social welfare institutions. After the economic reforms, the policy of socialization of social welfare is adopted to alleviate the burdens of government on social welfare. Fee-charging services are provided to all older persons in need. Besides government, individuals, private organizations and non-government organizations are involved in developing residential care for older persons. Currently, family care is not the only one mode of care for older persons. Residential care and community-based services are also set to share the responsibility of the family to care for the older persons. The objectives of residential care include not only to maintain the frail older persons but also to help older persons to have a better quality of life. Meanwhile, both management mechanism and the role of government, family and community have experienced a lot of changes. In the case study of Tianjin, quality of care was analyzed based on three groups of stakeholders. Generally residential care users were satisfied with the quality of care. But there were differences of perceptions on the quality of care among the three groups. Both family members and staff emphasized more about the material aspects and medical care than older persons do. On the contrary, older persons expressed their dissatisfaction with psychological care from both institutions and family. Finally, this thesis formulates recommendations for residential care development and quality improvement, including personnel development, empowering family and flexible services for older persons. Improvements in life expectancy and the consequences of the decades-old family planning policy have led to a rapid increase in the elderly population. According to the United Nations World Population Prospects, the proportion of older people age 65 and over will increase by about one-fourth by 2030, and the elderly will account for about one quarter of the total population by 2050. Population aging will not only pose challenges for elder care but also have an impact on the economy and all aspects of society (World Bank, 2016a). The government is aware of the need to develop an efficient and sustainable approach to aged care. To this end, the General Office of the State Council issued the 12th Five-Year Plan for the Development of Aged Care Services in China and the Development Plan for a System of Social Services for the Aged (2011-2015). It is now in the process of formulating the 13th Five-Year National Plan on Aging, which will further elaborate and finalize the reform roadmap for 2016 to 2020. The Plan is expected to be finalized and launched by June 2016. The National Development and Reform Commission (NDRC) helped draft these plans and is now leading the development of policy measures for the provision of social services for the elderly. This volume has been prepared to support the translation of the broad ideas on aged care provision expressed in the 12th and 13th Five-Year Plans and other government plans into reality and to help the government tackle the challenges described above. It strives to identify a policy framework that fits the Chinese context and can be put in place gradually. Specifically, it aims to provide an up-to-date understanding of the evolving aged care landscape in China; review international experiences in long-term care provision, financing, and quality assurance and assess their relevance to China’s current situation; discuss implications of current developments and trends for the future of aged care in China; and propose policy options based on available evidence and best practices.

First, a set of statistical indicators are developed for mapping the spatial distribution of the elderly population and residential care facilities (RCFs). Secondly, in-depth, semi-structured interviews are used to understand the socio-cultural meanings of access, the decision making process in relocation, the well-being of elderly residents, as well as the challenges of residential care and social welfare reform. In total, 27 elderly residents, 16 family members, and five RCF managers were interviewed in six RCFs in Beijing. The constant comparative method is used to analyze all the transcribed interview materials. There are several major findings resulting from the research: the distribution of the elderly population and residential care resources is geographically uneven across the districts of Beijing and the supply of resources does not match the potential need. Elderly people and their family members choose residential care because of the shortage of community and home care resources and/or the advantages of residential care. The decision making process is a process of balancing geographical factors, quality of services, and financial affordability. Access to residential care is an interactive process influenced by geographical, economic, and social-cultural factors. The physical and socio-cultural environments of RCFs and individual's sense of place play important roles in their adaptation and well-being after the relocation from the home to a RCF. Building up the active aging model with joint efforts from governments, society, RCFs, and individuals is helpful and effective for promoting the well-being of elderly residents in RCFs. At the end, the study also provides suggestions for the government, organizations, and RCFs on aspects such as administration, policy making, planning, volunteering, and management of RCFs to meet the challenges of residential care in China. The study confirms the importance of healthy living environments to the well-being of elderly residents. It also provides knowledge for understanding the reconfiguration of filial piety in decision making processes and utilization of residential care in current Chinese society. From a health geography perspective, this thesis is one of the first studies on residential care in China.

Book excerpt: China’s senior population is equivalent to over 14% of its total population, so it can be easily concluded that China is an “aging society” (RenMin RiBao, 2014). In the next 20 years, China’s population will begin to increase at the rate of 10 million each year (Hua Er Jie Jian Wen, 2014). Another relevant fact is that more than 260 million peasant workers are currently working in urban metropolitan cities (China’s Employment, 2013). Therefore, the Continuing Care Retirement Community (CCRC) is essential to provide well-being and care to senior citizens in China. However, the development of CCRC in China is not keeping up with the annual increase in population. The codes and standards for senior housing design are also inadequate and inconsistent. The majority of CCRCs in China continue to focus only on physical facilities, not quality of life of seniors (Tao, Wang, & Ma, 2008). However, the resident’s quality of life (QOL) has been considered as a very important factor when designing senior living facilities in other countries. Therefore, this study hypothesized that physical environments and quality of life both play significant roles in seniors’ life and more attention should be paid when designing the senior housing in China. Under these backgrounds, this research reviewed a range of available literature on senior living design including two main categories, which are physical environment and quality of life. The Physical environment part described information about housing and living environment for older people including the continuing care retirement community, senior living communities and building types in China, standards of the residential building design for the aged in China, wayfinding, and universal design. The second part includes information about quality of life domains, such as safety, enjoyment, relationship, autonomy and choice. This study focused on the common areas of the CCRC in Beijing, China. Both qualitative and quantitative methods have been used to collect and analyze the data. The researcher visited the CCRC, observe and measure physical environments of common areas (i.e. space planning, finish materials, furniture, lighting, universal design features) utilizing physical environment checklists. Data was also collected by conducting in-person surveys with residents and their families and friends, and semi-structured interviews with staff and a manager. The collected data were analyzed to investigate the relationship between CCRC common areas and residents’ quality of life...This research will contribute to the Chinese senior housing industry by incorporating the quality of life domains that emphasize human relationship, choice, and enjoyment along with safety in the physical environment. For future research, the researcher will develop a more comprehensive observation checklist and survey and interview questionnaires that can be applied to different types of senior housing in China to examine seniors’ needs and expectations.
